Book excerpt: Visual inspection of this dam did not reveal conditions which constitute an immediate hazard to human life or property. However, the dam has some deficiencies which require further engineering investigations. An area of concentrated seepage was noted at the downstream toe of the embankment. This area was approximately in line with the concrete gate house. Seepage was emerging at a rate estimated to be 10 to 20 gallons per minute. While only one area of concentrated seepage was noted, the entire area beyond the downstream toe was wet and swampy. Water was flowing out of the hillside in several areas immediately downstream of the embankment- right abutment contact. The structural stability analysis performed for the spillway section indicates that safety factors are below recommended values for all conditions studied. The sliding safety factors for severe loading conditions such as ice loading or flood flows indicate that the section would be unstable if subjected to these loads. It is recommended that within 3 months of the date of notification of the owner, investigations into the seepage and structural stability problems should be commenced. Remedial measures which are required based on these investigations should be completed within 18 months. The hydrologic/hydraulic analyses performed indicate that the outflows from all storms exceeding 54% of the Probable Maximum Flood (PMF) will result in flow over the emergency relief weir segment of the embankment. The dam does have the sufficient spillway capacity to pass one-half the PMF without the embankment being overtopped. Therefore, the spillway capacity is rated as inadequate.

Information and analysis are based on visual inspection of the dam by the performing organization. Visual inspection of this dam did not reveal conditions which constitute an immediate hazard to human life or property. Structural stability analyses performed for this report indicate that the spillway section is unstable for all conditions studied. The analysis was based on the limited information available and so may not reflect existing conditions. However, the analysis does indicate that there is a serious question concerning the stability of this dam and further investigations are required. It is recommended that within 3 months of the date of notification of the owner, investigations into the structural stability deficiencies should be commenced. These studies should include developing accurate cross sections of the dam, progressing subsurface explorations, and coring the dam. This information should then be incorporated into a detailed stability evaluation and the need for modifications to the structure should be determined. Required changes of the structure should be completed within 18 months.

A structural stability investigation should be commenced within 6 months to determine the characteristics of the uplift forces acting on the dam, the properties of the existing dam and the effect of these conditions on the stability of the dam. The hydrologic/hydraulic analysis indicates that the spillway will pass only 26% of the Probable Maximum Flood (PMF). The dam will be overtopped by 6 feet and 1-1/2 feet by the PMF and 1/2 PMF respectively. Failure of the dam during the 1/2 PMF event would significantly increase the downstream hazard from that which would exist just prior to failure of the dam. Based on an initial screening, and preliminary computations, there appears to be a serious deficiency in spillway capacity so that if a severe storm were to occur, overtopping and failure of the dam would take place, significantly increasing the hazard to loss of life downstream from the dam.

Several deficiencies were noted and these should be corrected within 6 months of the date of final approval of this report. Among the actions which should be taken are refilling the cribs which are missing material, repair of displaced timbers on the downstream face, replace broken timbers, repair of the valve on the drain, and repair of the spalling concrete on the wingwall which extends downstream of the north abutment. In addition, a detailed emergency action plan and warning system should be developed. This dam does not have sufficient spillway capacity to adequately discharge the peak outflow from one- half the PMF. The analysis indicates that the dam would be overtopped by all storms exceeding 15% of the PMF. However, the structural stability analysis indicates that the dam would be stable when subjected to the PMF storm event. Therefore, the spillway is assessed as being inadequate. The water surface level for either the PMF or one-half the PMF would result in flow around the northern end of the concrete wingwall which extends from the abutment of the dam. An accurate topographic survey should be performed where this end around flow is likely to occur.

A structural stability investigation should be commenced within 3 to 6 months to determine the characteristics of the uplift forces acting on the dam, the properties of the existing dam and foundation, and the effect of these conditions on the stability of the dam. Remedial work should be undertaken depending on the results of this investigation and completed within 18 to 24 months. The hydrologic/hydraulic analysis establishes the spillway capacity as 35 percent of the Probable Maximum Flood (PMF). The dam will be overtopped by 0.7 feet by the PMF and 0.25 feet under the 1/2 PMF. However, a dam break analysis indicates that a dam failure would not significantly increase the downstream hazard to loss of life from that which would occur just prior to the failure. Therefore, the spillway is assessed as inadequate according to the Corps of Engineers' screening criteria.

Remedial work should be completed, depending on the results of the investigation, within two years. The hydrologic/hydraulic analysis establishes the spillway capacity as 10.7% of the Probable Maximum Flood (PMF). The dam will be overtopped by 10.0 feet and 4.8 feet during the PMF and 1/2 PMF respectively. The spillway is inadequate since failure of the dam during the 1/2 PMF event will not significantly increase the downstream hazard from that which would occur just prior to the dam failure.

The dam, however, has a number of problem areas which require investigation and remedial action. These areas are as follows: The spillway is inadequate for all storms in excess of 1% of the PMF (Probable Maximum Flood). The spillway capacity for analysis purposes is 110 cfs before overtopping of the dam would occur. The PMF was calculated to be 25,300 cfs. The dam is overtopped by 7.4 feet during the 1.2 PMF and 12.7 feet during the PMF. The ability of the dam to withstand overtopping and the need to provide additional spillway capacity will have to be evaluated. The design and configuration of the dam is such that only a detailed analysis of the structural stability, which is beyond the scope of this report, will provide sufficient information to determine if overtopping of the dam will be detrimental to the stability of the structure. Therefore, a detailed structural stability investigation is required to determine the type and extent of required remedial measures. This analysis should be initiated as soon as possible with remedial action, as a result of this investigation, completed within the following year.
