The dam, however, has a number of problem areas, which if not remedied, have the potential for developing into hazardous conditions. The visual inspection revealed water flowing in a ditch at the toe of the easterly abutment and wet areas on the downstream face of the easterly dam section. THESE AREAS should be monitored to determine the quantity of the flow and the extent of the wet areas. Periodic measurements should be taken to determine if these areas increase in size or flow. Test borings should be taken to determine the source of this flow. Remedial measures, as indicated as a result of this investigation, should be taken to prevent further damage to the dam.

Its spillway has also been adjudged to be seriously inadequate and the dam assessed as unsafe, nonemergency. Due to the appreciable size of Eaton Reservoir, the breaching of that dam would undoubtedly have a serious effect on Clayton's Dam. Similarly, the failure of either of these dams would have a serious effect on two small dams also on Eaton Brook, located 0.5 miles and 1.7 miles downstream of Clayton's Dam.

The maximum spillway discharge capacity is 7.9 percent of the PMF peak outflow. The spillway is, therefore, adjudged as 'inadequate'. The following remedial measures should be performed within 12 months from notification: (1) Missing portions of the upstream stone masonry wall should be repaired. (2) Brush and shrubbery should be removed from the embankment, spillway-reservoir drain structure and downstream channel. Provide a program of periodic cutting and or mowing of the dam surfaces, spillway-reservoir drain structure and the downstream channel. (3) The collected debris at the spillway-reservoir drain structure and in the downstream channel should be removed and hauled away from the dam. (4) A program of periodic inspections and maintenance of the dam and appurtenances should be provided, including yearly operation and lubrication of the reservoir drain and its control facilities. This information should be documented for future reference. (5) An emergency action plan for the project should be developed.

Seepage found in and around the spillway during the inspection was of major concern.

The structural stability analysis indicates unsatisfactory stability would result from loadings which could occur during all investigated conditions. A structural stability investigation should be commenced within 6 months to determine the characteristics of the uplift forces acting on the dam, the properties of the existing dam and the effect of these conditions on the stability of the dam. The hydrologic/hydraulic analysis indicates that the spillway will pass only 26% of the Probable Maximum Flood (PMF). The dam will be overtopped by 6 feet and 1-1/2 feet by the PMF and 1/2 PMF respectively. Failure of the dam during the 1/2 PMF event would significantly increase the downstream hazard from that which would exist just prior to failure of the dam. Based on an initial screening, and preliminary computations, there appears to be a serious deficiency in spillway capacity so that if a severe storm were to occur, overtopping and failure of the dam would take place, significantly increasing the hazard to loss of life downstream from the dam.

This seepage extends approximately 250 feet along the embankment, and reaches are elevations of 17 feet below the crest of the dam. The hydrologic/hydraulic analysis indicates that the spillway system is capable of passing the 1/2 PMF with 1.25 feet of freeboard. But, the PMF will overtop the concrete box at the downstream end of the spillway and run down along the embankment toe. Therefore, the spillway is assessed as inadequate according to the Corps of Engineers' screening criteria.

Book excerpt: The examination of documents and visual inspection of the dam and appurtenant structures did not reveal conditions which constitute an immediate hazard to human life or property. The dam, however, has a number of problem areas which require further investigation and remedial work. The structural stability analysis indicates that the dam is unstable when subjected to forces which could occur under the Probable Maximum Flood (PMF) loading condition. Unsatisfactory stability is indicated for the dam when subject to forces which could occur during the normal winter operations and the 1/2 PMF loading condition. A structural stability investigation should be started within six months to determine the characteristics of the uplift forces acting on the dam and to determine whether the foundation acts integrally with the dam section to resist overturning. Remedial work should be completed, depending on the results of the investigation, within two years. The hydrologic/hydraulic analysis establishes the spillway capacity as 10.7% of the Probable Maximum Flood (PMF). The dam will be overtopped by 10.0 feet and 4.8 feet during the PMF and 1/2 PMF respectively. The spillway is inadequate since failure of the dam during the 1/2 PMF event will not significantly increase the downstream hazard from that which would occur just prior to the dam failure.
