Book excerpt: Potake Lake Dam is 1 mile southwest of Sloatsburg, New York. The dam and reservoir are used as a water supply source for Sloatsburg, New York. Visual inspection revealed several deficiencies in this structure. The following items were noted: There are three depressions and an animal burrow on the crest of the dam; Trees and brush are growing on the upstream and downstream face of the dam; The spillway discharge channel has trees, boulders, and natural ground which restricts flow over the spillway weir; A 3-foot diameter culvert in the discharge channel has the downstream end higher than the upstream end; There are tire ruts 0.4 foot deep on the crest of the dam; and There is a small amount of spalling and deterioration of the low section of the concrete weir.

Phase I Inspection Report written by Granville Kester (Jr) and published by . This book was released on 1981 with total page 105 pages. Available in PDF, EPUB and Kindle. Book excerpt: Examination of available documents and a visual inspection of the dam and appurtenant structures did not reveal conditions which constitute an immediate hazard to human life or property. It has been determined that the dam would be overtopped for all storms exceeding approximately 23 percent of the Probable Maximum Flood (PMF). Therefore, the spillway is adjudged 'seriously inadequate', and the dam is assessed as unsafe, non-emergency. It is recommended that, within three months of owner notification, detailed hydrologic and hydraulic investigations of the structure should be undertaken to more accurately determine the site-specific characteristics of the watershed and their effects upon the overtopping potential of the dam. The results of this investigation and analyses will determine the appropriate remedial measures required to achieve a spillway capacity adequate to discharge the outflow from at least the 1/2 PMF. In the interim, a detailed emergency action plan must be developed and implemented during periods of unusually heavy precipitation. Also, around-the-clock surveillance must be provided during these periods.

Phase I Inspection Report written by Eugene O'Brien and published by . This book was released on 1980 with total page 79 pages. Available in PDF, EPUB and Kindle. Book excerpt: The spillway is about 24 feet wide, ungated and has a sill 2 feet wide and 1.6 feet below the top of the dam. A 16-inch diameter cast iron reservoir drain is located near the left end of the spillway. The examination of documents and the visual inspection of Lake Suzanne did not reveal conditions which constitute an immediate hazard to human life or property. However, the dam has some deficiencies which require further investigation and remedial action. Using the Corps of Engineers screening criteria for review of spillway adequacy, it has been determined that the dam would be overtopped for all storms exceeding approximately 4 percent of the PMF.

Phase I Inspection Report written by Eugene O'Brien and published by . This book was released on 1981 with total page 84 pages. Available in PDF, EPUB and Kindle. Book excerpt: The examination of documents and the visual inspection of Lake Mahopac Dam and its appurtenant structures did not reveal conditions which constitute an immediate hazard to human life and property. Using Corps of Engineers screening criteria, it has been determined that the dam would be overtopped for all storms exceeding approximately 56 percent of Probable Maximum Flood (PMF). The maximum spillway discharge capacity is 7.9 percent of the PMF peak outflow. The spillway is, therefore, adjudged as 'inadequate'. The following remedial measures should be performed within 12 months from notification: (1) Missing portions of the upstream stone masonry wall should be repaired. (2) Brush and shrubbery should be removed from the embankment, spillway-reservoir drain structure and downstream channel. Provide a program of periodic cutting and or mowing of the dam surfaces, spillway-reservoir drain structure and the downstream channel. (3) The collected debris at the spillway-reservoir drain structure and in the downstream channel should be removed and hauled away from the dam. (4) A program of periodic inspections and maintenance of the dam and appurtenances should be provided, including yearly operation and lubrication of the reservoir drain and its control facilities. This information should be documented for future reference. (5) An emergency action plan for the project should be developed.

Phase I Inspection Report written by Eugene O'Brien and published by . This book was released on 1980 with total page 200 pages. Available in PDF, EPUB and Kindle. Book excerpt: Lake Welch Dam, formerly known as Beaver Pond Dam is located on the east side of Lake Welch. The maximum height of dam is 32 feet. The dam is 788 feet long and consists of a concrete gravity section (563 feet) and an earth embankment (right of gravity section) with a central concrete core wall (225 feet). The concrete dam, supported on a rock foundation consists of 18 monoliths which are anchored to the rock by 25-ton post-tensioned rock anchors. Examination of the available documents and visual inspection of the Lake Welch Dam did not reveal conditions which constitute a hazard to human life or property. Using the Corps of Engineers screening criteria for review of spillway adequacy, it has been determined that concrete gravity section of the dam would be overtopped for all storms exceeding approximately 12 percent of the Probable Maximum Flood (PMF).
