Book excerpt: Helderberg Lake Dam consists of a 285 foot long earth embankment with concrete cutoff wall and a 55 foot wide concrete channel spillway. The maximum height above the original stream bed is approximately 35 feet. Both the upstream and downstream slopes of the earth embankment are 1 vertical to 2 horizontal, and the crest width is 12 feet. The dam was reconstructed in 1944 after a failure of the original structure in 1938. The spillway crest was increased in length from 35 feet to 55 feet and lowered 2 feet. The original cutoff wall which is founded on bedrock was lengthened and increased in height by 2.25 feet. A 20 in. drain pipe was installed and incorporated into the tunnel that was in the original dam. The examination of documents and visual inspection of Helderberg Lake Dam and appurtenant structures did not reveal conditions which constitute a hazard to human life or property. The discharge capacity of the spillway is inadequate for all storms in excess of 50% of the PMF (Probable Maximum Flood).

Book excerpt: The dam is located on an unnamed tributary of the Bozen Kill, a tributary of the Normans Kill and the Lower Hudson River, approximately 2.3 miles west of the Village of Altamont. The dam provides storage for the supply of water to the Village of Altamont. The problem areas observed during the inspections and the recommended remedial actions are as follows: Cracking and movement of the right spillway chute wall was observed. Removal of backfill, installation of weeps and stiffening of the wall should control this problem; Seepage was observed resulting from deterioration of the construction joints and cracking of the spillway concrete. Vegetation was also noted growing in the joints of the spillway chute slabs. All construction joints require cleaning and recaulking to reduce seepage and inhibit infiltration through the spillway chute and core wall; Cracking of the exposed core wall, spillway buttresses, return walls and spillway chute slabs were observed. These areas require repair. Monitor the deterioration of the catwalk and repair as required, and Monitor the seepage observed at the base of the spillway chute and at the reservoir drain outlet. If seepage increases significantly, investigate and repair.

It has been determined that the dam would be overtopped for all storms exceeding approximately 23 percent of the Probable Maximum Flood (PMF). Therefore, the spillway is adjudged 'seriously inadequate', and the dam is assessed as unsafe, non-emergency. It is recommended that, within three months of owner notification, detailed hydrologic and hydraulic investigations of the structure should be undertaken to more accurately determine the site-specific characteristics of the watershed and their effects upon the overtopping potential of the dam. The results of this investigation and analyses will determine the appropriate remedial measures required to achieve a spillway capacity adequate to discharge the outflow from at least the 1/2 PMF. In the interim, a detailed emergency action plan must be developed and implemented during periods of unusually heavy precipitation. Also, around-the-clock surveillance must be provided during these periods.

During the 1/2 PMF event, the maximum water surface will be 4.4 feet below the top of dam. However, the dam will be overtopped by 2.0 feet during the full PMF; therefore the spillway is assessed as 'Inadequate'. The following problems were observed which require remedial action within one year of notification to the owner: Clean the deteriorated concrete and repair those areas where significant concrete is needed and where reinforcing bars are exposed; Repair the construction joints and cracking of the spillway; Remove the debris and sediment from the vicinity of the intakes and the downstream channel; Consider removal of penstock; Remove the tree and brush growth from the abutments upstream area and downstream channel. Provide a program of periodic cutting at these locations; Provide a program of periodic inspection and maintenance of the dam and appurtenances including periodic removal of sediment and debris; Develop an emergency action plan for notification of downstream residents and the proper governmental authorities.

The drainage area for this structure was limited to the reservoir itself. From normal water surface elevation to the top of the dam, the Probable Maximum Flood can safely be stored and discharged through the overflow pipe. The following problem areas were observed which require remedial action within 1 year of notification to the owner: Repair the areas of deteriorated concrete which are leaking (approx. 50 gpm) between the 24 inch and 48 inch valves within the intake chamber; Repair the deteriorated concrete and control the seepage within the diversion tunnel; Monitor the seepage within the intake chamber, particularly above the 12 inch valve, and repair as required; Repair the voids in the concrete spillway apron; Monitor the seepage from the horizontal joints of the spillway; Periodically monitor the concrete deterioration of the dam and appurtenances; Periodically remove the debris in the downstream channel; Remove the trees and brush on the slopes, crest and abutments of the embankments.

Using Corps of Engineers screening criteria, it has been determined that the dam would be overtopped for all storms exceeding approximately 56 percent of Probable Maximum Flood (PMF). The maximum spillway discharge capacity is 7.9 percent of the PMF peak outflow. The spillway is, therefore, adjudged as 'inadequate'. The following remedial measures should be performed within 12 months from notification: (1) Missing portions of the upstream stone masonry wall should be repaired. (2) Brush and shrubbery should be removed from the embankment, spillway-reservoir drain structure and downstream channel. Provide a program of periodic cutting and or mowing of the dam surfaces, spillway-reservoir drain structure and the downstream channel. (3) The collected debris at the spillway-reservoir drain structure and in the downstream channel should be removed and hauled away from the dam. (4) A program of periodic inspections and maintenance of the dam and appurtenances should be provided, including yearly operation and lubrication of the reservoir drain and its control facilities. This information should be documented for future reference. (5) An emergency action plan for the project should be developed.
