Book excerpt: Cornwall Upper Reservoir Dam, located on an unnamed tributary of Moodna Creek, is 2 miles south of Cornwall-on-the-Hudson, New York. The dam and reservoir are used for water supply. The visual inspection revealed several deficiencies in this structure. The following items were noted: The left spillway training wall and the adjacent crest elevation are 0.9 feet below the average crest of the dam; Superintendent Ralph Smith reported seepage near a large tree (24-inch diameter) on the downstream face near the left abutment of the dam. Some seepage was observed in this area, but the source could not be determined; Four small local depressions along the crest of the dam; Trees are located in the discharge channel immediately downstream from the spillway; On the downstream slope, there are four or five tree trunks with diameters 4 to 8 inches. On the downstream slope there are a large number of trees below the road; The masonry portion of th left spillway wall has collapsed for approximately 3 feet long at the crest of the spillway weir; A large number of rodent holes are scattered on the downstream face of the dam; and There is a small eroded area on the downstream face of the dam near the left abutment.

The following items were noted: The cement joints of the spillway wing walls are badly deteriorated; A stone is missing from the right downstream side of the spillway near the top of the right wing wall; On the masonry portion of the dam, the cemented joints of the top few courses of stones are deteriorated; One of the stones is missing from the top of the downstream face; The masonry wall above the outlet works has deteriorated; Two small seeps were found in the masonry wall below the crest of the weir; three other small seeps were found on the downstream face of the dam. A small wet area was observed at the toe of the left section of the embankment; The stumps and root systems of several large trees are present in the downstream embankment. One of the stumps is partly uprooted; and There are fallen trees and other debris in the spillway discharge area.

Information and analysis are based on visual inspection of the dam by the performing organization. The examination of documents and the visual inspection of the Goshen Reservoir did not reveal conditions which constitute an immediate hazard to human life or property. However, the dam has some problem areas which require additional studies and remedial work. The most serious of these deficiencies are the two areas of seepage on the downstream slope of the dam. First is at the end of a grouted stone spillway channel and the second is at the toe in the center of the dam. It is recommended that within six months from notification, an engineering investigation is initiated to determine the sources of the seepage found. Using the Corps of Engineers' screening criteria for the initial review of spillway adequacy, it has been determined that the embankment would be overtopped by all storms in excess of 51% of the Probable Maximum Flood (PMF). Therefore, the spillway is adjudged as 'inadequate.' (Author).

It has been determined that the dam would be overtopped for all storms exceeding approximately 28 percent of the Probable Maximum Flood (PMF). The spillway is, therefore, adjudged as 'seriously inadequate' and the dam is assessed as unsafe, non-emergency. It is recommended that, within 3 months of notification of the owner, detailed hydrologic and hydraulic investigations of the structure should be undertaken to more accurately determine the site-specific characteristics of the watershed and their effects upon the overtopping potential of the dam. The results of these investigations and analyses will determine the appropriate remedial measures which will be required. In the interim, a detailed emergency action plan must be developed and implemented during periods of unusually heavy precipitation. Also, around-the-clock surveillance must be provided during these periods. Current inspection and maintenance procedures by the owner are inadequate. Monitoring of the reservoir levels should be expanded to include readings during peak flow periods.

It does not mean that there appears to be a serious deficiency in spillway capacity; and if a severe storm were to occur, overtopping and failure of the dam could take place significantly increasing the hazard to loss of life downstream of the dam.

The maximum spillway discharge capacity is 7.9 percent of the PMF peak outflow. The spillway is, therefore, adjudged as 'inadequate'. The following remedial measures should be performed within 12 months from notification: (1) Missing portions of the upstream stone masonry wall should be repaired. (2) Brush and shrubbery should be removed from the embankment, spillway-reservoir drain structure and downstream channel. Provide a program of periodic cutting and or mowing of the dam surfaces, spillway-reservoir drain structure and the downstream channel. (3) The collected debris at the spillway-reservoir drain structure and in the downstream channel should be removed and hauled away from the dam. (4) A program of periodic inspections and maintenance of the dam and appurtenances should be provided, including yearly operation and lubrication of the reservoir drain and its control facilities. This information should be documented for future reference. (5) An emergency action plan for the project should be developed.
