Deficiencies noted by the inspection team included heavy overgrowth across the embankment crest and slopes, rodent burrows along the downstream embankment face, a deteriorated emergency spillway, and minor seepage beneath the emergency spillway and around the outlet conduit. These deficiencies are, for the most part, attributable to a general lack of adequate maintenance since the facility was phased out of operation in 1973. The size classification of the facility is small and its hazrad classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility ranges between the 1/2 PMF (Probably Maximum Flood) and the PMF. Due to the high potential for damage to downstream structures and loss of life, the SDF is considered to be the PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only 16 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency.

Book excerpt: Rexmont No. 2 Dam, formerly known as Lebanon No. 2 Dam, is owned by the City of Lebanon and was originally constructed as a water supply reservoir for the City. At present, the reservoir is used for recreation (fishing) and is only a standby water supply for the City. The Rexmont No. 1 Dam, also owned by the City of Lebanon, is located about 1,000 feet further downstream. A state highway is located by between the two dams. The dam is a homogeneous earthfill structure with a maximum fill height of 42 feet and an embankment length of 690 feet. A 14.5 foot wide spillway (field measured) is located in the left abutment and has a weir elevation 4 feet below the top of the dam embankment. An intake control tower, located at the upstream toe of the dam, has three intake sluice gates and is connected to a downstream valve house by a 16-inch and a 20-inch pipe. The flow in these pipes is controlled by valves in the valve house. The embankment had slope failures in 1910, 1912, 1925. The downstream slope was stabilized in 1933 by adding fill and a berm. Book excerpt: The assessment of Sugar Creek Dam is based upon visual observations made at the time of inspection, review of available records and data, hydraulic and hydrologic computations and past operational performance. The inspection and review of Sugar Creek Dam did not reveal any problems which would require emergency action. The dam appears to be in fair condition and in need of minor maintenance. The Sugar Creek Dam is a high hazard-small size dam. The spillway design flood (SDF) for a dam of this size and classification is 1/2 PMF to PMF. The PMF has been selected as the spillway design flood based on the downstream potential for loss of life. The spillway and reservoir are considered capable of controlling approximately 50% of the PMF. The 0.1 feet of overtopping of the embankment crest during the 1/2 PMF event for a duration of 1 hour was not considered sufficient to cause failure of the structure. Based on the judgement of the evaluating engineer, the location and duration of the overtopping as well as the existence of a concrete curb on the upstream and downstream edge of the embankment crest would sufficiently contain the overtopping to the spillway area minimizing damage to the embankment and spillway. The spillway is considered inadequate.

Book excerpt: The dam is located on Kings Creek, approximately 4 miles north of the Village of Estella, Elkland Township, Sullivan County, Pennsylvania. The Roscoe Burgess Dam can be located on the Overton U.S.G.S. 7.5 minute quadrangle. The onsite inspection of the Roscoe Burgess Dam consisted of: (1) Visual inspection of the retaining structure, abutments and toe. (2) Examination of the spillway facilities, exposed portion of any outlet works and other appurtenant works. (3) Observations affecting the runoff potential of the drainage basin. (4) Evaluation of the downstream area hazard potential. In general, the dam and appurtenant structures appear to be in fair condition. No major erosion areas were observed during the inspection. No seepage was observed along the downstream slope of the structure or along the toe area. Dense brush exists along the downstream slope of the structure and hampered attempts at close visual inspection of the downstream slope and toe area. The brush and small trees on the downstream slope and in the area of the toe of the dam should be removed under the direction of a registered professional engineer. Once the slope and toe area are cleared, the area should be reinspected to verify that no seepage existed at the structure. No drainline was observed at the structure. This is considered a deficiency in some means should be provided to drain the reservoir.

Book excerpt: Lake Jean Dam is a zoned earth embankment approximately 26 feet high and 780 long, including spillway. The emergency spillway is an uncontrolled, rectangular, concrete and rock cut chute channel located at the right abutment. The crest of the spillway is 20 feet long and is spanned by a concrete bridge which supports a portion of the bituminous roadway that covers the embankment crest. Drawdown capability is provided by means of a 30-inch diameter cast iron blowoff pipe controlled at the inlet by a 30-inch diameter sluice gate. The facility consists of a main dam (Lake Jean Dam) and two appurtenant saddle dams (east and west dikes). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store about 73 percent of the PMF prior to overtopping the east dike. Thus, based on the screening criteria contained in the recommended guidelines, the spillway is considered to be inadequate, but not seriously inadequate. Should the saddle dams be regraded to their design elevation, the spillway would then pass floods in excess of the PMF and would be considered adequate.

(5) That a formal surveillance and downstream warning system be developed for use during periods of high or prolonged rainfall. (6) That an operation and maintenance manual be prepared for guidance in the operation of the dam during normal and emergency conditions, and that a schedule be developed for the annual inspection of the dam and its appurtenant structures. The concrete on the upstream slope of the dam is in a deteriorated condition as is the concrete in the spillway section. The spillway retaining walls are visibly deteriorated and a section of the right spillway wingwall has cracked and fallen away from the structure. Maintenance of the dam and operating facilities is considered poor. The drainline valves have not been operated in the recent past. Pomeroy Memorial Reservoir Dam is a high hazard-small size dam. The recommended spillway design flood (SDF) for a dam of this size and classification is in the range of 1/2 PMF to PMF. Based on the downstream potential for loss of life, the spillway design flood has been selected as the PMF. The spillway and reservoir are capable of controlling approximately 9% of the PMF without overtopping the embankment low spot. If the low spot elevation were to be filled to an elevation consistent with the remaining portion of the embankment crest the spillway and reservoir would be capable of controlling approximately 20% of the PMF without overtopping the embankment. Based on criteria established by the Corps of Engineers, the spillway is termed inadequate.

Book excerpt: Based on visual inspection, available records, calculations and past operational performance, and according to criteria established for these studies, Summit Lake Dam is rated as unsafe, nonemergency, because the spillway capacity is seriously inadequate. The existing spillway can pass 26 percent of the Probable Maximum Flood (PMF) without overtopping of the dam. The failure of the dam would cause an increased hazard to loss of life downstream. As a whole, the dam is judged to be in fair condition. If the top of the dam were raised 0.1 foot to its design elevation, the spillway could pass 27 percent of the PMF. The spillway capacity would still be rated as seriously inadequate. There is no evidence of stability problems with the embankment. The masonry gravity section of the embankment has no significant deviations from the OCE guideline for stability, since the toe pressure is well below the allowable.

Book excerpt: The visual inspection and review of available design and construction data indicate that Ross Dam is in fair condition. The deteriorated condition of the spillway ways and the adjacent low areas of the embankment are the primary deficiencies which require maintenance work to assure the operational safety of this facility. In accordance with the recommended guidelines, the spillway design flood (SDF) for this facility is in the range of the 100 year flood to the 1/2 PMF. Based on the size of this dam and the degree of downstream hazard, the selected SDF is the 100 year flood. The hydrologic and hydraulic computations indicate that the combination of reservoir storage and spillway discharge capacity will pass the SDF (100 year flood) prior to overtopping the embankment. Therefore, in accordance with the criteria outlined and evaluated in Section 5.5 of this report, the spillway for Ross Dam is considered to be adequate.
