Book excerpt: The Pomeroy Memorial Reservoir Dam appears to be in poor condition and poorly maintained. The concrete on the upstream slope of the dam is in a deteriorated condition as is the concrete in the spillway section. The spillway retaining walls are visibly deteriorated and a section of the right spillway wingwall has cracked and fallen away from the structure. Maintenance of the dam and operating facilities is considered poor. The drainline valves have not been operated in the recent past. Pomeroy Memorial Reservoir Dam is a high hazard-small size dam. The recommended spillway design flood (SDF) for a dam of this size and classification is in the range of 1/2 PMF to PMF. Based on the downstream potential for loss of life, the spillway design flood has been selected as the PMF. The spillway and reservoir are capable of controlling approximately 9% of the PMF without overtopping the embankment low spot. If the low spot elevation were to be filled to an elevation consistent with the remaining portion of the embankment crest the spillway and reservoir would be capable of controlling approximately 20% of the PMF without overtopping the embankment. Based on criteria established by the Corps of Engineers, the spillway is termed inadequate.

Book excerpt: The assessment of Sugar Creek Dam is based upon visual observations made at the time of inspection, review of available records and data, hydraulic and hydrologic computations and past operational performance. The inspection and review of Sugar Creek Dam did not reveal any problems which would require emergency action. The dam appears to be in fair condition and in need of minor maintenance. The Sugar Creek Dam is a high hazard-small size dam. The spillway design flood (SDF) for a dam of this size and classification is 1/2 PMF to PMF. The PMF has been selected as the spillway design flood based on the downstream potential for loss of life. The spillway and reservoir are considered capable of controlling approximately 50% of the PMF. The 0.1 feet of overtopping of the embankment crest during the 1/2 PMF event for a duration of 1 hour was not considered sufficient to cause failure of the structure. Based on the judgement of the evaluating engineer, the location and duration of the overtopping as well as the existence of a concrete curb on the upstream and downstream edge of the embankment crest would sufficiently contain the overtopping to the spillway area minimizing damage to the embankment and spillway. The spillway is considered inadequate.

The inspection consisted of: Visual inspection of the retaining structure, abutments and toe; Examination of the spillway facilities, exposed portion of any outlet works and other appurtenant works; Observations affecting the runoff potential of the drainage basin; and Evaluation of the downstream area hazard potential. The dam appears to be in good condition and adequately maintained. From a brief survey conducted during the inspection, it was noted that the low spot on the crest of the dam exits at the top left edge of the spillway crest. It was observed during the inspection that a new concrete cap had been placed on the structure. The concrete appeared to be in good condition. The dam was constructed with dry rubble masonry with an upstream earthfill. No concrete exists on the downstream face of the structure or on the bench located along the downstream face of the structure. The rubble masonry appeared to be in good condition and well maintained.

Hydraulic/hydrologic evaluation, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, revealed that the spillway capacity is less than the peak inflow to the impoundment during the 100-year flood. A spillway design flood (SDF) in the range of the 100-year flood to the 1/2 Probable Maximum Flood (1/2 PMF) is required for Galvin Pond Dam. Because the dam is on the low end of the 'Small' size category in terms of storage capacity and height, the 100-year flood was chosen as the SDF. The spillway is therefore considered 'Inadequate.' Several items of remedial work should be immediately initiated by the owner. These include: (1) Develop remedial measures to ensure that the dam is not overtopped by the 100-year flood; (2) Repair the two 12-inch steel pipes, fill the embankment to original crest elevation and reseed with grass; (3) Cut the trees and brush on the dam; and (4) Provide means to draw down the reservoir during an emergency.

Book excerpt: Based on the evaluation of the existing conditions, the condition of Cooks Pond Dam is considered to be unsafe/nonemergency due to the seriously inadequate spillway capacity and structural deficiencies. The condition of the embankment is considered to be poor. The dam is old and in a general state of disrepair. The crest of the dam is irregular and the center of the dam appears to have settled. The stone wall along the downstream toe is irregular and bulging. The toe of the wall is swampy, caused by a general underseepage. In view of these observations, the overall stability of the dam is considered to be questionable, requiring further investigation and implementation of measures to improve the stability of the dam. The spillway capacity was evaluated according to the recommended procedure and was found to pass less than 10 percent of the Probable Maximum Flood (PMF) without overtopping the embankment. This capacity is less than the required spillway capacity of one-half PMF relative to the size and hazard classification of the dam. Because the spillway capacity is less than 50 percent of the PMF and it is estimated that failure of the dam due to overtopping would significantly increase the downstream hazard of loss of life compared to that which would exist just before failure, the spillway is considered to be seriously inadequate, and consequently the condition of the dam is considered to be unsafe/nonemergency.

The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

Book excerpt: The purpose of a Phase I investigation is to identify expeditiously those dams which may pose hazards to human life or property. The assessment of the general condition of the dam is based upon visual observations and review of available data. Detailed investigations and analyses involving topographic mapping, subsurface investigations, material testing, and detailed computational evaluations are beyond the scope of a Phase I investigation; however, the inspection is intended to identify any need for such studies which should be performed by the owner. In reviewing this report, it should be realized that the reported condition of the dam is based on observations of field conditions at the time of inspection along with data available to the inspection team. In cases where the reservoir was lowered or drained prior to inspection, such action, while improving the stability of the dam, removes the normal load on the structure and may obscure certain conditions which might otherwise be detectable if inspected under the normal operating environment of the structure.

Book excerpt: The visual inspection and review of available data indicate that Duck Pond Dam is in fair condition. The lack of embankment protection from spillway flows and the inadequate spillway capacity are the primary deficiencies which cause concern for the safety of this dam. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for this facility is in the range of the 100 year flood to 1/2 the Probable Maximum Flood (PMF). Based on the size of the dam and degree of downstream hazard, the selected SDF is the 100 year flood. The hydrologic and hydraulic computations indicate that the combination of reservoir storage and outlet works discharge capacity will not pass the SDF (100 year flood) prior to overtopping the embankment. In accordance with the criteria outlined and evaluated in Section 5.5 of this report, the discharge capacity for Duck Pond Dam is considered to be inadequate.

Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway capacity is less than the peak inflow to the impoundment during the 100-year flood. A spillway design flood (SDF) in the range of the 100-year flood to the 1/2 Probable Maximum Flood (1/2 PMF) is required for Steinhauer Dam. Because the dam is on the low end of the 'Small' size category in terms of storage capacity and height, the 100-year flood was chosen as the SDF. The spillway will not pass the SDF without overtopping the dam. The spillway is therefore considered inadequate.
