Book excerpt: Memorial Lake Dam was constructed in 1945 as a recreational facility for the Indiantown Gap Military Reservation. The 760 foot long rolled earthfill embankment has a maximum fill height of 37 feet above the original streambed. Reference is made to Appendix F, Plates III and IV, for a general plan and typical section. A cutoff trench was excavated along the centerline of the dam and a concrete cutoff wall was placed in this trench. A 110 foot long ogee spillway was excavated in the left abutment. It has a weir crest elevation of 430.0, which is 8 feet below the top of dam. The ogee discharges the water to an eight foot deep stilling basin. A control tower is located 12.5 feet upstream from the centerline dam. Flow through the 48-inch conduit is controlled inside the tower by a 48-inch sluice gate or by a gated weir also located inside the tower and near normal pool elevation.(Author).

Failure of the embankment due to overtopping by one-half of the PMF would not significantly increase the risk to loss of life downstream. The spillway, therefore, is considered to be inadequate, but not seriously inadequate. Recommendations are presented for immediate action by the owner.

The size classification of the facility is small and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility ranges between the 1/2 PMF (Probable Maximum Flood) and the PMF. Due to the high potential for damage to downstream structures and possibly loss of life, the SDF is considered to be the PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store about 77 percent of the PMF prior to embankment overtopping. Consequently the spillway is assessed as being inadequate, but not seriously inadequate. Deficiencies noted by the inspection team included the following: (1) large trees rooted within the downstream embankment face; (2) a damaged and inadequate spillway discharge channel; and (3) a bent gate stem and partially obstructed outlet associated with the blowoff conduit.

Book excerpt: Maple Lake Dam is an earthfill embankment 14.4 feet high and 283 feet long, excluding spillway. The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

Book excerpt: Page's Lake Dam is owned and operated by the Lakeside Outing Club and is classified as a 'Significant' hazard - 'Intermediate' size dam. The dam was found to be in good overall condition on 27 October 1980. Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway will not pass the 1/2 Probable Maximum Flood (1/2 PMF) without overtopping the dam. A spillway design flood (SDF) in the range of the 1/2 PMF to the Probable Maximum Flood (PMF) is required for Page's Lake Dam. The 1/2 PMF was chosen as the SDF because the dam is on the low end of the 'Intermediate' size category in terms of storage. During the 1/2 PMF, the dam is overtopped by a maximum depth of 3.34 feet for a total duration of 11.83 hours. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately initiate an engineering study to further evaluate the spillway capacity and develop recommendations for remedial measures to reduce the overtopping potential of the dam.

Book excerpt: The assessment of Pioneer Lake Dam is based upon visual observations made at the time of inspection, review of available records and data, hydraulic and hydrologic computations and past operational performance. The dam and appurtenant structures appear to be in fair condition. No major erosion was observed on the embankment crest. An erosion area was observed at the outlet for the 12 in. diameter cast iron pipe. Erosion at the outlet of the 12 in. diameter pipe exists on the downstream slope of the dam near the toe. Seepage 100 ft. right of the spillway channel was measured to be approximately 50 gallons per minute. A 30 in. corrugated metal pipe exists beneath the roadway near the outlet for the emergency spillway. It was observed during the inspection that the pipe is collapsing near the center of the pipe. Trees were observed growing along the upstream edge of the crest, and dense vegetation was observed along the entire length of the downstream slope. The Pioneer Lake Dam is a high hazard-small size dam. The recommended spillway design flood (SDF) for a dam of this size and classification is in the range of the 1/2 PMF to PMF. Due to the downstream potential for loss of life and property damage, the spillway design flood has been selected as the PMF. The spillway and reservoir are capable of controlling approximately 11% of the PMF without overtopping the embankment. Based on criteria established by the Corps of Engineers, the spillway is termed seriously inadequate. Pioneer Lake Dam is classified as unsafe, non-emergency.

The Rexmont No. 1 Dam, also owned by the City of Lebanon, is located about 1,000 feet further downstream. A state highway is located by between the two dams. The dam is a homogeneous earthfill structure with a maximum fill height of 42 feet and an embankment length of 690 feet. A 14.5 foot wide spillway (field measured) is located in the left abutment and has a weir elevation 4 feet below the top of the dam embankment. An intake control tower, located at the upstream toe of the dam, has three intake sluice gates and is connected to a downstream valve house by a 16-inch and a 20-inch pipe. The flow in these pipes is controlled by valves in the valve house. The embankment had slope failures in 1910, 1912, 1925. The downstream slope was stabilized in 1933 by adding fill and a berm. Book excerpt: The visual inspection, operational history, and hydrologic/hydraulic analysis indicate that the facility is in fair condition. Deficiencies noted by the inspection team included heavy overgrowth across the embankment crest and slopes, rodent burrows along the downstream embankment face, a deteriorated emergency spillway, and minor seepage beneath the emergency spillway and around the outlet conduit. These deficiencies are, for the most part, attributable to a general lack of adequate maintenance since the facility was phased out of operation in 1973. The size classification of the facility is small and its hazrad classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility ranges between the 1/2 PMF (Probably Maximum Flood) and the PMF. Due to the high potential for damage to downstream structures and loss of life, the SDF is considered to be the PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only 16 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency.

Book excerpt: Based on visual inspection, available records, calculations and past operational performance, and according to criteria established for these studies, Summit Lake Dam is rated as unsafe, nonemergency, because the spillway capacity is seriously inadequate. The existing spillway can pass 26 percent of the Probable Maximum Flood (PMF) without overtopping of the dam. The failure of the dam would cause an increased hazard to loss of life downstream. As a whole, the dam is judged to be in fair condition. If the top of the dam were raised 0.1 foot to its design elevation, the spillway could pass 27 percent of the PMF. The spillway capacity would still be rated as seriously inadequate. There is no evidence of stability problems with the embankment. The masonry gravity section of the embankment has no significant deviations from the OCE guideline for stability, since the toe pressure is well below the allowable.
