Based on the evaluation of available design information and visual observations of conditions as they existed on the dates of the field reconnaissances, the general condition of Laurel Hill Lake Dam is considered to be good. However, the cause and origin of a seepage zone located downstream of the dam embankment could not be conclusively established by visual observation and review of the construction drawings. Therefore, it is recommended that periodic monitoring of the seep be made by the dam owner. The presence of eroded footpaths, animal burrows, cracking and spalling on concrete surfaces, and shallow depression located on the dam crest, are considered minor deficiencies in need of maintenance. Guideline criteria recommend a PMF spillway design flood for intermediate size, high hazard dams. Spillway discharge capacity was found to be seriously inadequate based on the following data: (1) Maximum non-overtopping spillway discharge capacity is 34 percent PMF, (2) Failure of the dam resulting from 37 percent PMF overtopping significantly increases the downstream loss of life and damage potential compared to that which would exist prior to dam failure. Laurel Hill Lake Dam is categorized as unsafe, non-emergency in accordance with recommended criteria.

The size classification for the facility is intermediate and the hazard classification is high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the Probable Maximum Flood (PMF). Results of the hydrologic and hydraulic analysis indicate that the facility is capable of passing and/or storing only 36 percent of the PMF. The analysis also indicates that spillway discharges are controlled by the downstream valley capacity when the flow over the weir exceeds approximately 3 feet (10 feet below the embankment) after which the pool level and tailwater will rise at roughly the same rate. Thus, failure from overtopping is essentially precluded and the spillway is considered inadequate, but not seriously inadequate. The consequences of embankment inundation by tailwater and subsequent rapid drawdown are uncertain; although, it is noted that portions of the downstream slope are designated as pervious fill and rock toe on the design drawings. This construction is favorable with respect to rapid drawdown response; however, thorough inspection of the embankment should be performed during and after inundation.

Observations made during the visual inspection suggest that the embankment may be structurally unsafe as extensive areas of seepage were noted on the downstream embankment slope and immediately below the toe area to the right of the outlet conduit. Seepage conditions became evident shortly after reservoir filling and have persisted in spite of the installation of a remedial sand drain system near the toe. Failure of the embankment does not appear imminent and the condition is presently considered non-emergency. It is recommended that the owner immediately develop and implement a warning system to notify downstream inhabitants should hazardous conditions develop. Included in the plan should be provisions for around-the-clock surveillance of the facility during periods of unusually heavy precipitation.

Lake Rene Dam, Which was constructed in 1955, was the original dam at the site. Wolf Creek Dam and Galion Bay Dam were constructed in 1971 in conjunction with the enlargement of Lake Rene. The flood discharge facilities for Lake Rene consist of primary and emergency spillways associated with Wolf Creek and Galion Bay dams. The Wolf Creek Dam flood discharge facilities are comprised of a drop inlet primary spillway located at the center of the embankment and an emergency spillway on the left abutment (looking downstream). The primary spillway structures consist of a single-stage reinforced concrete riser, a 36-inch-diameter reinforced concrete conduit, and a plunge pool at the downstream end of the outlet pipe. The emergency spillway is an 130-foot-wide trapezoidal earth channel excavated into the left abutment.

Book excerpt: The PA-647 dam is one of the seven flood control structures in the Wheeling Creek watershed. The dam consists of an earth embankment approximately 500 feet long with a maximum height of 95 feet from the downstream toe and a crest width of 15 feet. The flood discharge facilities for the dam consist of a drop inlet type primary spillway located near the left abutment (looking downstream) and an emergency spillway on the right abutment. Based on the evaluation of the conditions as they existed on the dates of inspection and as revealed by visual observations, the condition of PA-647 dam is considered to be good. The spillway capacity is classified to be adequate according to the recommended criteria.

Based on the evaluation of available design information and visual observations of conditions as they existed on the dates of the field reconnaissances, the general condition of Lost Creek Dam is considered to be good. Wet zones, observed located at the downstream embankment toe, are believed attributable to surface runoff, and are not considered significant relative to dam stability at the present time. However, seepage emanating along the reservoir drain pipe outlet may represent a potential hazard to the dam. The cause and origin of this seepage could not be conclusively established by visual observation and review of construction documents. Therefore, periodic monitoring of the seep is recommended.
