Book excerpt: Hunters Lake Dam is an earthfill dam 450 feet long and 27 feet high. The upstream slope is 3H:1V with concrete paving blocks acting as slope protection. The upstream slope in the lower portion is 3.5H:1V. The downstream slope is 2.5H:1V and grassed. The embankment is homogeneous with a rock toe. Upstream of the dam is a concrete control tower which contains the principal spillway. Flow enters the concrete spillway tower and drops down the tower and into a 48 inch reinforced concrete pipe to an impact basin at the toe of the dam. In addition, the concrete spillway tower has a 4 foot by 5 foot sluice gate to act as a reservoir drain. The emergency spillway is located in a natural saddle at the opposite end of the reservoir. The emergency spillway has a 300 foot long concrete slab. (Author).

Book excerpt: Maple Lake Dam is an earthfill embankment 14.4 feet high and 283 feet long, excluding spillway. The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

Book excerpt: The assessment of the Hamilton Lake Dam is based upon visual observations made at the time of inspection (June 28, 1979), review of available records and data, review of available data, hydrology and hydrologic and past operational performance. The inspection and review of data of Hamilton Lake Dam did not reveal any problems which require emergency action. The dam appears to be stable, well maintained, safely operated and in good condition. Hamilton Lake Dam is a high hazard-intermediate size dam. The spillway design flood is the PMF (Probable Maximum Flood). The spillway and reservoir are capable of controlling the PMF. Based upon the criteria established by the Corps of Engineers, the spillway is termed adequate. The following recommendations and remedial measures should be instituted immediately: (1) Continue annual safety inspections and operation and maintenance inspections; and (2) A warning system should be instituted to warn downstream residents of large spillway discharges or failure of the dam.

Book excerpt: Forest Lake Dam is located on Forest Lake Creek in Forest Lake Township, Susquehanna County, Pennsylvania. The original purpose of the impounded water was for ice harvesting and recreation. Additionally, water releases from the impoundment activated a saw mill. Presently, the lake is being used for recreation. The overall appearance of the dam is poor. Location of observed deficiencies are shown on the General Plan. The profile and typical sections of the dam are presented and are based on field survey made on the day of inspection. The survey datum for this inspection is elevation 1527 feet above mean sea level which is stamped on the USGS Bench Mark (No. TT15K, 1931) on the large sandstone slab, upstream of the service spillway. On the inspection date (12/10/1980), the lake was being drawn down for the winter. The lake level was approximately at elevation 1523.6, or 2.8 feet below its normal level. Deficiencies observed during the field inspection are described. Visible features are depicted in photographs.

Book excerpt: Lake Walter Dam is located on Spring Run, approximately one mile upstream of its confluence with Meshoppen Creek in Springville Township, Susquehanna County, Pennsylvania. The condition of Lake Walter Dam is considered to be good. The only significant condition noted which requires further attention is the lack of erosion protection in the emergency spillway discharge channel. The swampy condition at the toe of the dam is not considered to be significant at this time; however, the condition should be monitored to document if seepage is developing.

Phase I Inspection Report written by and published by . This book was released on 1981 with total page 71 pages. Available in PDF, EPUB and Kindle. Book excerpt: The assessment of Pioneer Lake Dam is based upon visual observations made at the time of inspection, review of available records and data, hydraulic and hydrologic computations and past operational performance. The dam and appurtenant structures appear to be in fair condition. No major erosion was observed on the embankment crest. An erosion area was observed at the outlet for the 12 in. diameter cast iron pipe. Erosion at the outlet of the 12 in. diameter pipe exists on the downstream slope of the dam near the toe. Seepage 100 ft. right of the spillway channel was measured to be approximately 50 gallons per minute. A 30 in. corrugated metal pipe exists beneath the roadway near the outlet for the emergency spillway. It was observed during the inspection that the pipe is collapsing near the center of the pipe. Trees were observed growing along the upstream edge of the crest, and dense vegetation was observed along the entire length of the downstream slope. The Pioneer Lake Dam is a high hazard-small size dam. The recommended spillway design flood (SDF) for a dam of this size and classification is in the range of the 1/2 PMF to PMF. Due to the downstream potential for loss of life and property damage, the spillway design flood has been selected as the PMF. The spillway and reservoir are capable of controlling approximately 11% of the PMF without overtopping the embankment. Based on criteria established by the Corps of Engineers, the spillway is termed seriously inadequate. Pioneer Lake Dam is classified as unsafe, non-emergency.

Phase I Inspection Report written by D'APPOLONIA CONSULTING ENGINEERS INC PITTSBURGH PA. and published by . This book was released on 1981 with total page 69 pages. Available in PDF, EPUB and Kindle. Book excerpt: Lake Sophia Dam consists of an earth embankment approximately 340 feet long with a maximum height of 29 feet from the downstream toe and a crest width of 9 feet. The upstream side of the dam is protected by a small amount of riprap and the downstream face is covered with grass. The flood discharge facilities for the dam consist of a concrete open-channel spillway located near the left abutment. The outlet facilities consist of a 12-inch-diamenter corrugated metal pipe extending through the embankment along the original streambed. Based on the evaluation of existing conditions, the condition of Lake Sophia Dam is considered to be unsafe/nonemergency due to seriously inadequate spillway capacity. The flood discharge capacity was evaluated according to to the recommended procedure and was found to pass approximately 15 percent of the PMF without overtopping the dam. Results of the breach analysis indicate that downstream damage would be significantly increased due to a dam failure.

Book excerpt: The visual inspection and review of available design and construction data indicate that Longford Lake Dam is in good condition. In accordance with the guidance provided, the spillway design flood (SDF) ranges between 1/2 the PMF and the full PMF. Based on the size and extent of downstream hazard for the dam, the SDF selected for this facility was the PMF. The hydrologic and hydraulic computations indicate that the combination of reservoir storage and spillway discharge capacity will pass 88% of the PMF prior to overtopping the embankment. Therefore, in accordance with the recommended criteria for Phase I inspections, the spillway for Longford Lake Dam is considered to be inadequate.

Book excerpt: Recreation Dam is an L-shaped earth dam with a measured maximum height of 14.7 feet and a total length of approximately 1,600 feet. The facility is provided with a masonry spillway at the left abutment along its northern section and a diversion canal along its easterly extension. The latter serves to divert acid mine drainage from upstream mined areas around the facility. Based on visual observations, past history, and a hydrologic and hydraulic investigation, the dam and its appurtenances are considered in poor condition. Based on a visual evaluation and past performance the spillway system is considered structurally unsafe but of non-emergency status as the reservoir is completely drawn down. In addition, evaluation of the flashboard system indicated that failure of the flashboards under normal operating procedures could in itself cause serious downstream consequences.

Book excerpt: Lake Mount Union Dam is a concrete buttress dam. Lake Mount Union Dam was built by the Ambursen Construction Company and is thus called an Ambursen type dam. Ambursen dams are articulated, reinforced concrete buttress dams with expansion joints between the decks and the buttresses. The deck consists of reinforced concrete water bearing slabs, separated by buttress tongues and supported by reinforced concrete haunches which are constructed monolithically with the buttresses. Lake Mount Union Dam has seventeen vertical concrete buttresses constructed on eighteen foot centers. These buttresses are founded on shale and quartzite. The buttresses are of variable thickness with the bottom of each buttress 21 inches thick and the top of the buttress 14 inches thick. The concrete slab on the upstream of each buttress is also of variable thickness. The bottom of the slab is 41.5 inches thick with the top of the slab 24 inches thick. The concrete slab is sloped at an angle of 1H:1V. At the toe of the upstream slope of the dam a cutoff trench was excavated. This cutoff trench was backfilled with concrete. Steel reinforcing was placed in all buttresses and throughout the concrete slab. Between all expansion joints, asphalt putty was placed.

Book excerpt: Based on visual inspection, available records, calculations and past operational performance, Lake Henry Dam is judged to be in good condition. The existing spillway can pass 60 percent of the Probable Maximum Flood (PMF) without overtopping of the dam. The spillway capacity is rated as inadequate. If the embankments were raised 0.7 foot to their design elevation, the dam could pass the PMF with 0.05 foot of freeboard. The spillway capacity would then be rated as adequate. A low area between the two embankments acts as an auxiliary spillway. There is no stability analysis for the embankments. There is no evidence of significant problems threatening the embankments. The spillway weir is judged to be stable.

Available in PDF, EPUB and Kindle. Book excerpt: Hidden Lake Dam is an earthfill dam, 550 feet long and 20 feet high. The crest width of the dam is 15 feet. The visible portion of the upstream slope was measured to be 1H:1V and the downstream slope was measured to be 1.5H:1V. No riprap protection was observed on the upstream slope. The spillway is located at the right abutment and consists of an overflow section 35 feet in length. The Hidden Lake Dam appears to be in poor condition. A slide was noted on the downstream slope approximately 240 feet from the left abutment. The slide appeared to be inactive. A wet area was noted to the left of the discharge end of the 15 ft. corrugated metal pipe drain line. Flow from the seepage area was determined to be less than 1 gallon per minute. The drain line for the dam consists of a 15 ft. corrugated metal pipe with the upstream portion of the pipe blocked. No adequate facilities exist to regulate the reservoir level. Based on the downstream potential for loss of life, the spillway design flood has been selected as the PMF. The spillway and reservoir are capable of controlling approximately 1% of the PMF without overtopping the embankment (low spot). Based on criteria established by the Corps of Engineers, the spillway is termed seriously inadequate. If Hidden Lake Dam would fail due to overtopping, the hazard potential for loss of life and property damage immediately downstream of the dam would be significantly increased from that which would exist just prior to the overtopping.

Book excerpt: Hathaway Pond Dam is located in Ararat Township, Susquehanna County, Pennsylvania. The reservoir was originally created to provide water for power purposes. The reservoir slopes are moderate with a good cover of vegetation. There are three ponds upstream from Hathaway Pond Dam on two tributary streams. The western stream contains Romobe Lake (PennDER I.D. No. 58-10) which is 1750 feet upstream from Hathaway Pond Dam. The eastern tributary contains Ball Lake and an unnamed smaller pond in series. Ball Lake is 2450 feet upstream and will retain considerable storage while the smaller unnamed pond was considered to have no effect on Hathaway Pond Dam. Two homes, a trailer, and township road are located 1800 feet downstream from Hathaway Pond Dam. These could suffer economic damage in the event of a dam failure.
