Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway will not pass the 100-year flood without overtopping the dam. A spillway design flood (SDF) in the range of the 100-year flood to the 1/2 Probable Maximum Flood (1/2 PMF) is required for Beaver Pond Dam. Because the dam is on the low end of the 'Small' size category in terms of storage capacity and height, the 100-year flood was chosen as the SDF. During the 100-year flood, the dam is overtopping by a maximum depth of 3.27 feet for a total duration of 8.67 hours. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately develop recommendations for remedial measures to reduce the overtopping potential of the dam.

Ball Lake is 2450 feet upstream and will retain considerable storage while the smaller unnamed pond was considered to have no effect on Hathaway Pond Dam. Two homes, a trailer, and township road are located 1800 feet downstream from Hathaway Pond Dam. These could suffer economic damage in the event of a dam failure.

Book excerpt: Based on visual inspection, available records, calculations and past operational performance, and according to criteria established for these studies, Winola Mill Pond Dam is judged to be unsafe, nonemergency, because the spillway capacity is rated as seriously inadequate. The existing walls of the main spillway will overtop at about 6 percent of the Probable Maximum Flood(PMF). Overtopping of the walls will cause erosion of the dry masonry section of the dam. It is estimated that the 15 percent PMF would cause sufficient erosion to cause the dam to fail. The initial surge from the failure of the dam would increase the hazard to loss of life downstream. As a whole, the dam is judged to be in fair condition. Cracks have developed in the concrete liner wall on the upstream face of the dry masonry. Excessive leakage and a hole in the upstream earthfill had previously developed and the repairs made to the structure do not eliminate the possibility of these conditions reoccurring. Although a preliminary stability analysis indicates no apparent need for immediate concern, the theoretical stability of Winola Mill Pond Dam is unknown because the foundation conditions are unknown.

Book excerpt: Based on the evaluation of the existing conditions, the condition of Cooks Pond Dam is considered to be unsafe/nonemergency due to the seriously inadequate spillway capacity and structural deficiencies. The condition of the embankment is considered to be poor. The dam is old and in a general state of disrepair. The crest of the dam is irregular and the center of the dam appears to have settled. The stone wall along the downstream toe is irregular and bulging. The toe of the wall is swampy, caused by a general underseepage. In view of these observations, the overall stability of the dam is considered to be questionable, requiring further investigation and implementation of measures to improve the stability of the dam. The spillway capacity was evaluated according to the recommended procedure and was found to pass less than 10 percent of the Probable Maximum Flood (PMF) without overtopping the embankment. This capacity is less than the required spillway capacity of one-half PMF relative to the size and hazard classification of the dam. Because the spillway capacity is less than 50 percent of the PMF and it is estimated that failure of the dam due to overtopping would significantly increase the downstream hazard of loss of life compared to that which would exist just before failure, the spillway is considered to be seriously inadequate, and consequently the condition of the dam is considered to be unsafe/nonemergency.

The spillway, located at the right abutment, consists of a cut stone lined broad crested weir and chute. The chute has an overall width of 10 feet and a length of 75 feet; the spillway crest is located 3.5 feet below the low spot of the crest of the dam. The outlet works for the dam consist of an 8 inch cast-iron pipe with the upstream end submerged approximately 30 feet out from the center line of the dam. The dam was found to be in fair overall condition at the time of inspection. The spillway will pass approximately 37 percent of the Probable Maximum Flood (PMF) before overtopping will occur.

The size classification of the facility is small and the hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) ranges between the 1/2 PMF (Probable Maximum Flood) and the PMF. Since the facility is classified near the lower bounds of the small category, the SDF is considered to be the 1/2 PMF. Results of hydrologic and hydraulic analyses indicate the facility will pass and/or store approximately 40 percent of the PMF prior to embankment overtopping at the low area in the embankment crest (elevation 1177.4). Breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided by the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency. Recommendations for owner action are listed.

Based on the size of the dam and degree of downstream hazard, the selected SDF is the 100 year flood. The hydrologic and hydraulic computations indicate that the combination of reservoir storage and outlet works discharge capacity will not pass the SDF (100 year flood) prior to overtopping the embankment. In accordance with the criteria outlined and evaluated in Section 5.5 of this report, the discharge capacity for Duck Pond Dam is considered to be inadequate.

A spillway design flood (SDF) in the range of the 100-year flood to the 1/2 Probable Maximum Flood (1/2 PMF) is required for Steinhauer Dam. Because the dam is on the low end of the 'Small' size category in terms of storage capacity and height, the 100-year flood was chosen as the SDF. The spillway will not pass the SDF without overtopping the dam. The spillway is therefore considered inadequate.

Hydraulic/hydrologic evaluations revealed that the spillway capacity is less than the peak inflow to the impoundment during the 100-year flood. A spillway design flood (SDF) in the range of the 50- year flood to the 100-year flood is required for Warner Dam. The 100-year flood was chosen as the SDF. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately develop remedial measures to provide adequate spillway capacity. The inspection revealed certain items of remedial work which should be performed by the owner without delay. These include: (1) Provide adequate spillway capacity; (2) Repair the spalling on the downstream face of the concrete corewall; (3) Ensure that the outlet works are operable, or develop an emergency drawdown plan; and (4) Cut all trees on the embankment at ground level. All trees with a trunk diameter greater than 3 inches should have their root systems removed. All resultant areas of erosion and cavities should be filled, graded, compacted and seeded. In addition, the following operational measures are recommended to be undertaken by the owner: (1) Develop a detailed emergency operation and warning system; (2) During periods of unusually heavy rainfall, provide around-the-clock surveillance of the dam; and (3) When warning of a storm of major proportions is given by the National Weather Service, activate the emergency operation and warning system.
