Book excerpt: The Department of Health has until recently been focusing on speed of response as a measure of performance of the ambulance service, rather than on clinical outcomes for patients. The ambulance service achieves high levels of public satisfaction but there are wide variations in ambulance trusts' efficiency and the NAO concludes that the system has not delivered the best value for money to date. The 8-minute response target, intended for the most seriously ill patients, is one of the most demanding in the world. However, its application has skewed ambulance trusts' approach to performance measurement and management. The target is also applied to a much wider group of patients than intended. The report identifies various inefficiencies in the system, with cost per incident varying between £176 and £251. There is scope across the urgent and emergency care system to make more of different ways of responding to patients, such as clinical advice to callers over the phone and taking patients to minor injuries units rather than A&E departments. If all 11 trusts adopted the best practice currently being used in at least one trust, the NHS could save £165 million a year. The elements of the emergency care system are not yet fully integrated and this leads to delays in turnaround times at hospital A&Es. Over one-fifth of patient handovers take longer than the recommended 15 minutes. The new clinical quality indicators introduced from April offer the potential for a fuller measurement of performance based on outcomes.

Book excerpt: In Britain, between 1980 and 1998, the number of people classified as obese tripled to 21per cent of women and 17 per cent of men. It is estimated that this costs the economy, as a whole, �2 billion and the NHS �0.5 billion in treatment. However the response of the NHS is patchy, with no national guidelines and only 28% of health authorities taking action to address the problem. There is little activity related to the management of obesity outside of general practice but only a small proportion of GPs follow a protocol. This report recommends that there should be strategies to reduce obesity and that the Department of Health should build on the plan in the National Service Framework on coronary heart disease and work with partners and professional bodies to clarify responsibilities. It should also work with the National Institute of Clinical excellence to disseminate information. The Department of Health should also lead a cross government strategy to promote the benefits of physical activity and there should be local targets to encourage cycling and walking. It should also work with the food industry to improve the balance of diet. Much of this work needs to be aimed at school children to promote a healthy lifestyle throughout life and guidance to schools on commercial sponsorship should be strengthened so that there is no conflict with messages on healthy eating.

Book excerpt: Since the Department's 2007 Maternity Matters strategy, there has been improvement in maternity services. However, there is wide variation between trusts in performance. The Department did not fully consider the implications of delivering its ambitions and has failed to demonstrate that it satisfactorily considered the achievability and affordability of implementing the strategy. Nor has it monitored national progress against it. In 2011, one in 133 babies was stillborn or died within several days of birth. The mortality rate has fallen over time, but comparisons with the other UK nations suggest scope for further improvement. Trusts paid £482 million for maternity clinical negligence cover in 2012-13, equating to around a fifth of spending on maternity services. The level of consultant presence has substantially improved but over half of maternity units (including all of the largest units) do not meet recommended levels. The NHS is also not meeting a widely recognised benchmark of one midwife to 29.5 births. The government has commissioned more places to study midwifery, but it is unclear whether these will be enough. Meeting the benchmark would require around 2,300 additional midwives nationally. In terms of choice of place of birth, 79 per cent of women are currently within a 30-minute drive of both an obstetric and midwifery-led unit, compared with 59 per cent in 2007. However, choice is restricted where units have to close because of a lack of physical capacity or midwives. Over a quarter of units closed for half a day or more between April and September 2012

The Department for Education and Skills established Ufi, which is the government backed e-learning organisation which runs and coordinates the learndirect service, in 1998 to develop people's skills and work with employers and to increase employees' capabilities. It now provides a half million learners a year with the opportunity to improve their skills, from a choice of 2,400 learndirect centres, with 1,600 main centres and 800 link centres. In total 1.7 million people have taken 4 million learndirect courses. Ufi and the learndirect service have received £930 million of education funding. The NAO has a number of recommendations in five key areas for the Learndirect service by: reducing costs; maximizing benefits of the infrastructure and the tools it has created; making sure that services are sustainable; expanding work with employers; improving consistency of learner assessment and persuading more learners to continue learning.

Book excerpt: The Department of Health introduced a target to reduce MRSA across all NHS trusts by 50 per cent by 2008 and C. difficile by 30 per cent by 2010-11. MRSA had been reduced by 57 per cent by the end of March 2008 and C. difficile by 41 per cent. While a quarter of trusts have reduced MRSA by more than 80 per cent, in 12 per cent of trusts there has been an increase in MRSA infections. Twenty-nine per cent of trusts have reduced C. difficile by more than 50 per cent, but in 19 per cent of hospital trusts the numbers of C. difficile infections have increased. Since the introduction of the targets, the Department has spent some £120 million tackling healthcare associated infections. There have also been unquantifiable administrative costs and local expenditure on the drive to reduce infection rates. These initiatives have led to savings on treatment of between £141 million and £263 million, as well as reducing discomfort, disability and, for some, death that might have been caused by these avoidable infections. There has also been a perceptible change in trust leadership on tackling infections. The impact has not, however, been the same in all trusts or for other infections. Following the Department's intervention to improve recording on death certificates where MRSA or C. difficile was the underlying cause or a contributory factor, in 2007 around 9,000 people were reported as having died in such circumstances. There is still no national information on deaths from other healthcare associated infections such as urinary tract infections and pneumonia and blood stream infections due to other causes may be increasing.

Book excerpt: Although officially 'dismantled', the National Programme for IT in the NHS continues in the form of separate component programmes which are still racking up big costs. The original contracts with CSC totalled £3.1 billion for the setting up of the Lorenzo care records system in trusts in the North, Midlands and East. Despite the contractor's weak performance, the Department of Health is itself in a weak position in its attempts to renegotiate the contracts. It couldn't meet the contractual obligation to make enough trusts available to take the system. We still don't know what the full cost of the National Programme will be. The Department's latest estimate of £9.8 billion leaves out the future costs of Lorenzo or the potential large future costs arising from the Department's termination of Fujitsu's contract for care records systems in the South of England. Parliament needs to be kept informed not only of what additional costs are being incurred, but also of exactly what has been delivered so far. The Department estimates £3.7 billion of benefits to March 2012, just half of the costs incurred. There is still a long way to go before government departments can honestly say that they have learned and properly applied the lessons from previous contracting. Given the Department's track record with the National Programme, it is very hard to believe that the paperless NHS towards which the Department is working has much chance of being achieved by the target date of 2018

This NAO report examines the extent to which the OFT is operating as an effective competition enforcement authority, focusing on its approach to undertaking enforcement activities and casework, and how it measures and reports the results of its competition work. Findings include that although the UK's competition regime is still relatively young compared to those of many other major economies, the OFT has established a growing international reputation for leading the intellectual debate on substantive issues in competition analysis. Book excerpt: The National Programme for Information Technology in the NHS (the Programme) is a ten year programme to use information technology (IT) to reform the way the NHS in England uses information, and hence to improve services and the quality of patient care. The core of the Programme will be the NHS Care Records Service, but other elements include x-rays accessible by computer, electronic transmission of prescriptions, and booking of first outpatient appointments. The Programme was launched in 2002, and is now run by an agency, NHS Connecting for Health. This report examines: the progress made in delivering the systems against the original plans and costs (part 1); steps taken by the Department of Health, the agency and the NHS to deliver the Programme (part 2); how the IT systems have been procured (part 3); how the NHS is preparing to use the systems (part 4). The NAO estimates the gross cost of the Programme will be £12.4 billion to 2013-14. Although the pilot NHS Care Records Service will not be in place until late 2006, almost two years late, and other milestones have been deferred, the NAO reports substantial progress with the Programme. Management systems are in place, contracts were placed quickly and achieved large reductions in prices from bidders, and contract terms include important safeguards to secure value for money. Deployments of operational systems have begun, and several additional tasks have been delivered that were outside the original brief. Three key areas are identified which present significant challenges to the successful implementation of the Programme: ensuring IT suppliers continue to deliver systems that meet the needs of the NHS, to agreed timescales without further slippage; ensuring NHS organizations play a full part in implementing the systems; winning the support of NHS staff and the public in making the best use of the systems to improve services. The NAO report makes a number of recommendations for future management of the Programme.

Book excerpt: Primary Care Trusts (PCTs) are responsible at the local NHS level for the statutory "duty of care", largely through implementing clinical governance. The concept of clinical governance aims to improve continuously the overall standard of clinical care; reduce variations in outcomes of, and access to, services; and ensure that local decisions are based on the most up to date evidence of what is known to be effective. The key principles of clinical governance are: a coherent approach to quality improvement, clear lines of accountability for clinical quality systems and effective processes for identifying and managing risk and addressing poor performance. Clinical governance, implemented effectively, can provide PCT Chief Executives with assurance that healthcare, whether provided directly or commissioned from other providers, is both safe and of good quality. This report finds that the organisational structures and processes for clinical governance have largely been put in place at PCT level. But progress in implementing the different components of clinical governance varies both within and between PCTs. More needs to be done to provide assurance about the performance of General Practitioners and the systems which protect the safety of patients. Key features of those PCTs that can demonstrate consistent improvements in quality include effective clinical leadership, maintaining the capacity to deliver services, ensuring the quality of the patient experience and improving services based on lessons from complaints and patient safety incidents. The higher performing PCTs are characterised by: availability and accessibility of information to support evidence-based medicine; all staff appraised against an agreed work and development programme; service users involved in service development; clear action plans developed in response to clinical risks; and underperformance by clinical staff addressed by clear management procedures.

About half of such incidents could have been avoided, if lessons from previous incidents had been learned. This NAO report examines the progress being made in the NHS to improve the patient safety culture, to encourage incident reporting and to learn lessons for the future. The report finds that most trusts have developed a predominantly open and fair reporting culture at the local level, driven largely by the Department of Health's clinical governance initiative and more effective risk management systems. However, a 'blame culture' still exists in some trusts, and there have been delays in establishing an effective national reporting system. There is scope for improving strategies for sharing good practice and for monitoring that lessons are learned.
