Book excerpt: The last five years have witnessed an increasing interest in the subject of transmyocardial laser revascularization (TMR) as illustrated by the number of abstracts presented at the meetings of the American Heart Association and the American College of Cardiology (Figure). The ideas and concepts associated with this particular method of myocardial revascularization have changed dramatically over even this short period of time. The original premise of "de-evolving" mammalian hearts to recreate a reptilian-like myocardial circulation by making multiple channels through the myocardium has been almost (but perhaps not quite) completely dismissed. Now, the most popular notion is that there is an angiogenic response to myocardial channel making. It is this development of new blood vessels that is thought to be responsible for the apparent improvements in symptoms and blood flow. Along the way, the idea that a channel could stay open and allow blood to flow directly from the ventricular chamber has found little support. Rather than directly explore all of these issues and merely duplicate previously published articles, our aim was to take a novel approach: that is, to step back from these arguments and provide perspective from the vantage point of distance. In the case of trans myocardial revascularization, distance comes both in terms of history and in terms of methodology and knowledge from other fields of research. Historically, innovative methods of myocardial revascularization are by no means uncommon. The first two chapters deal with this historical' perspective.

Book excerpt: The percutaneous method of transluminal myocardial revascularization has roused enthusiasm in the field of cardiology because it is minimally invasive and relatively inexpensive. In his progressive new book, George Abela, a widely recognized pioneer in cardiology and lasers, has brought together the expertise of international leaders on the subject in this premier reference, Myocardial Revascularization: Novel Percutaneous Approaches. This resource provides interventional cardiologists, technical personnel and nurses in cardiac catheterization labs with the most current scholarly and clinical information available, including: * A historical overview of myocardial revascularization (including the Vineberg procedure and the more recent TMR/PMR procedures) * Biological responses and mechanisms of action in myocardial revascularization using mechanical, laser, ultrasound, and cryoenergy * Current technology for the percutaneous method of the procedure * Descriptions of other novel approaches including percutaneous in situ coronary venous arterialization * Discussion of results from preliminary and advanced TMR/PMR in canine and human procedures * Examination of various catheter navigation systems and potential adjunct gene therapies * Myocyte transplantation to replenish the depleted myocardium * Insights into the economics of these novel percutaneous techniques

Book excerpt: An interdisciplinary panel of pioneers and opinion leaders review the basic, preclinical, clinical, and developmental pathways to new treatment strategies, such as therapeutic angiogenesis and myogenesis. The authors take advantage of new biological understanding, novel therapeutic targets, multiple available and well-studied therapeutic strategies, and the necessary imaging techniques to measure outcomes. Their in-depth discussions cover the identification of new therapeutic targets and pathways, the investigation of transcriptional factors, master switch molecules, cell-based approaches, chemokines, a better understanding of the effects of aging, endothelial dysfunction, and hypercholesterolemia in response to angiogenic stimuli. Highlights include examination of drug delivery problems, outcomes measure, stem therapy, high-risk interventions, development pathways, and future possibilities.

Book excerpt: With a vast background of personal experience and a review of more than 300 references, Dr. Segesser has written a book to close the gap between the theory and practice of coronary artery revascularization. In particular, he shows the possibilities and limitations of internal mammary artery grafting in this surgical procedure. The chapters guide you to a thorough understanding of the subject, from anatomy and pathology, to experimental studies and the history of first surgical attempts, to clinical application and results. The discussion of long-term follow-up, complications and re-operations point to areas of success as well as subjects for future research. The details provided here about operative strategies and their outcomes give the cardiovascular surgical team important feedback about present practices which will help lead to improved clinical results.

Book excerpt: Numerous new and important findings relating to the reconstruction of disturbed circulatory conditions in the coronary area are reported in this book. Fundamental aspects of therapeutic and prophylactic possibilities are covered in detail. Recommendations are then made to practitioners and clinicians for their everyday work. Critical standards for the indications and risks of the recommended procedures are given. Particular emphasis is laid on the possibilities for revascularization by means of dissolution of blood thrombi. The book aims to present top-level guidance to the pros and cons of reconstructive measures in coronary disorders.

Available in PDF, EPUB and Kindle. Book excerpt: Despite remarkable advances in the understanding and management of impaired left ventricular function (LVF) and related coronary disease, the prevalence of heart failure in the U.S. and the resulting death rates have almost tripled in the past 3 decades. New understanding of the relationships between the myocardium and LVF demonstrate a direct correlation between myocardial viability and improved patient survival. Because of this, myocardial viability is now a major investigative area in contemporary cardiology, one that holds significant clinical and prognostic relevance. Authored by physicians of international renown, the book brings together various disciplines affecting myocardial viability, with five main sections providing an introduction and comprehensive review of: basic concepts and mechanisms; vascular biology and cellular physiology; advances in functional imaging; and perfusion, metabolism and cell membrane integrity. New concepts, such as stunning and hibernation, are clarified, and subsequent novel diagnostic and therapeutic strategies are described. New and sophisticated examination techniques are also presented, as well as advances in instrumentation and imaging techniques, which may result in improved use of resources and enhanced efficiency of health care delivery. This monograph will serve as a reference source for those interested in the field of myocardial viability, and hopefully improve understanding between investigators from various disciplines. Clinical cardiologists, physicians, and nurses in the field, as well as radiologists, vascular surgeons, reperfusionists, cellular biologists and physiologists, and students will all find material of interest in this book.

Available in PDF, EPUB and Kindle. Book excerpt: Chronic total occlusions (CTO) are common, and found in approximately one third of patients with significant coronary artery disease who undergo angiography. Over the past 20 years, CTO lesions have represented the most difficult anatomy for treatment — with lower success rates and higher complication rates. Chronic Total Occlusions provides interventionalists insight into the world of CTOs with introductory chapters that describe the pathology and indications of CTOs along with a review of clinical trials. Imaging modalities including CT angiography, magnetic navigation wire, and IVUS guided recanalization of CTO are also introduced, together with information on new wires technology and devices for CTOs. With numerous illustrations of these devices, technologies, and strategies to improve the CTO success rate, this clinical guide, headed up by Ron Waksman, will prove to be the ideal companion for interventional cardiologists and cardiac surgeons who are required to perform angioplasty and coronary stenting.

It provides in-depth coverage of the phenomenon of CMVO which heavily affects prognosis by increasing the risk of death and heart failure at follow-up. A first of its kind reference dedicated solely to this topic, it is appropriate for a wide audience, from researchers, to those who aid in the management, prevention and treatment of CMVO. Provides in-depth coverage of coronary microvascular obstruction (CMVO), spanning research, management, prevention and treatment Includes the most up-to-date information on CMVO as presented from top experts around the word Provides access to a companion website with extra material, including tables, additional references and instructional videos Gives extensive coverage on how to measure CMVO, including in-depth indexes that can be used to detect and quantify the phenomenon
