Available in PDF, EPUB and Kindle. Book excerpt: Despite changes in treatment modalities, pain and anxiety are both natural and highly anticipated patient outcomes related to a surgical procedure. Patients experience anxiety related to the uncertainty of any surgical procedure. Increased levels of pain and anxiety can lead to negative effects both physiologically and psychologically. Narcotics and sedatives are most commonly administered to alleviate both of the conditions. When given in large amounts and coupled with anesthetic drugs, adverse effects such as over-sedation can occur. This is an adverse effect that can be problematic and costly for not only the patient, but the nurse and facility as well. Respiratory depression and over sedation go hand-in-hand. When over sedation occurs, a patients respiratory effort can be greatly impacted. Over-sedation and other adverse effects can prolong not only the recovery period, but ultimately delay patient discharge. By adding a non-pharmacologic intervention such as music therapy to the treatment plan, the nurse will be able to reduce pain and anxiety, thus reducing the amount of narcotics and sedatives required. This can be used as a nursing driven intervention to be implemented in the pre-operative phase and follow the patient through the post-anesthesia phase. With the appropriate implementation plan, stakeholder acceptance, and resources, music therapy can be a beneficial influence on physiological parameters and psychological state. Music therapy can provide relaxation, lessen muscle tension, and provide a healing influence for patients. Music therapy can have a great impact on not only patient satisfaction, but anesthesia outcomes as well. Literature has shown that music therapy used as a complementary therapy has been proven as an inexpensive way to reduce perioperative pain and anxiety.

In patients having ophthalmic surgery, music therapy is effective at reducing patient anxiety compared to the administration of anti-anxiety medications such as Versed. Long wait times, fear of the unknown or pain, fear of disfigurement, and loss of control all contribute to feelings of anxiety. Upon entering into the preoperative area, the patient's body will enter in to the flight or fight phase. Music therapy is implemented while the patient is in the preoperative, intraoperative, and postoperative areas in an effort to reduce anxiety and help the patient's body return back to its baseline state. By increasing the utilization of this intervention by nurses in the surgical setting, the overall surgical experience can be improved for patients.

Book excerpt: The use of music as an intervention in the perioperative setting may be a beneficial tool in decreasing anxiety and pain associated with the surgery. The patient facing a surgical procedure may have anxiety related to the surgery itself, the physical challenges post-surgery, financial concerns, and the prospect of postoperative pain. The implementation of music in the perioperative setting is a cost-effective, non-invasive, non-pharmacologic intervention without adverse effects. The current preoperative procedure does not include the provision of a music intervention. This proposal utilizes the use of portable music players and headphones for patients in the perioperative setting. The staff will receive instruction on the intervention, questions and concerns will be addressed prior to induction of the program. The music intervention will be employed preoperatively, can be used intra-operatively pending the type of surgical procedure, and post-operatively and its effectiveness will be measured by using the Visual Analog Scale (VAS). The use of music has been shown in previous research as beneficial in decreasing anxiety and pain, leading to lessened use of pharmacologic interventions. Both staff and patients will be queried regarding their opinions on the intervention and its benefits or limitations. The collected data will be analyzed and the appropriateness, cost-effectiveness, and impact on patient satisfaction will be determined by upper management, surgery co-management, and the anesthesiology group. Further dissemination of the evidence will be made to additional departments, appropriate organizations, and associations.

It can be performed with no preoperative testing, no intravenous insertion, and no monitoring. Like a dental procedure, the patient simply gets up and goes home after the procedure. Presented in an easy-to-read, bullet-point format, Wide Awake Hand Surgery guides surgeons through all aspects of WALANT. The book covers a wide variety of topics including minimal pain injection of local anesthesia, nerve and tendon decompression, wrist surgery, repair of lacerated tendons, tendon transfers, finger fractures, lacerated nerves, metacarpal fractures, arthritis surgery and complex reconstructions in hand surgery. The book includes more than 150 step-by-step surgical and instructional videos as well as numerous color clinical photographs. Color drawings clearly guide the surgeon to the correct anatomic locations for anesthetic injections, and the book includes an atlas of tumescent local anesthesia distribution anatomy. Featuring a complimentary eBook, this valuable resource offers chapters written by worldwide experts, making it the definitive guide to wide awake hand surgery.

Available in PDF, EPUB and Kindle. Book excerpt: The introduction of total joint arthroplasty throughout the world has contributed manifold benefits to patients who suffer from joint diseases. Concurrently, however, there has been an increase in revision surgery. Many orthopedic surgeons agree that durability of prostheses is an eternal problem. In particular, periprosthetic osteolysis recently has been identified as one of the serious problems affecting prosthetic dura bility. To improve durability, osteolysis and many other problems must be investi gated and solved both experimentally and clinically with respect to such aspects as prosthetic material, design, and biological and biomechanical behavior. This book comprises 37 papers that were presented by orthopedic surgeons and biomedical engineers at the 28th Annual Meeting of the Japanese Society for Replace ment Arthroplasty, held in March 1998 in Kanazawa, Japan. The volume is thus a compilation of the latest knowledge about the pathogenesis and reduction of osteolysis and wear, newly developed total hip prostheses, and other current topics of total knee arthroplasty. We earnestly hope that this book will be of benefit to clinicians and researchers, and that it will contribute to the creation of more durable total joint prostheses in the future. The book's chapter structure coincides with the clinical path patients tread during their treatment, from preoperative evaluation to post-hospital care. Epidemiological aspects and aging processes are illustrated, providing keys to understanding the quick expansion of geriatric surgery and defining the clinical profile of older surgical patients in a cybernetic perspective. Preoperative evaluation and preparation for surgery, including medication reconciliation and pre-habilitation, are developed in the light of supporting decision-making about surgery in an evidence-based and patient-focused way. Intra- and postoperative management are discussed, aiming to tailor anesthetic, surgical and nursing approaches to specific patients' needs, in order to prevent both general and age-related complications. This volume also addresses issues relevant to geriatric surgery, from different organizational models to clinical risk management and systems engineering applied to hospital organization.

Book excerpt: The fourth edition of this highly successful textbook discusses the practical applications of the various methods of regional anaesthesia, including ultrasound, as well as giving an account of their theoretical aspects. The well-established author team use their experience to provide a practical and stimulating book which reflects everyday clinical activity, supplemented by high definition ultrasound images and schematic diagrams. The book will also discuss the place of regional anaesthesia in current medical practice, particularly with regard to pain processes, ultrasound imaging, and patient outcomes. Regional anaesthesia is a rapidly expanding sub-speciality of anaesthesia: routine provision of long-lasting pain relief and the introduction of real-time ultrasound block has fuelled a demand for cadaver and ultrasound-based anatomy training courses. Thus, it is more important than ever to have a thorough grounding in this exciting subspecialty.
