Book excerpt: When asked why he robbed banks, an astute and success ful criminal is said to have replied "Because that's where the money is kept." Why study patients with multiple primary cancers? The answer follows the same practical approach. Because the intensive investigation of such patients is very likely to yield data useful to both the clinical and research on cologist. Studies of this problem provide an immediate return for the clinician responsible for the care of cancer patients. With improved forms of therapy, these individuals are enjoying longer periods of survival. One important factor in maintaining increased survival is the early detection and treatment of new primary tumors which may develop. Analyses of multiple primary malignancies serve as a guide to the probable anatomic location of a subsequent primary and help define characteristics of the individual at high risk for multiple primary cancer. But just as treatment may improve the life of the cancer patient, it may also increase the risk of a subsequent malignancy. Studies of multiple primaries provide an efficient means for quantifying potentially harmful effects of current therapeutic modalities.

All polyps should be promptly treated by resection or fulguration as soon as possible after their discovery. 4. The physician must assume the responsibility for impressing the patient with the necessity for frequent and regular follow-up examinations. These examinations must be done at no longer than 1-year intervals for at least 5 years after treatment of the initial lesion and preferably at yearly intervals thereafter. 5. A minimal follow-up examination must include a proctoscopic examination as well as a careful roentgenologic examination of the colon. Summary The literature concerning multiple colonic carcinomas has been reviewed. Data have been presented on 261 cases in which multiple carcinomas of the colon were seen at the Mayo Clinic from January 1, 1944, through December 31, 1953. 0 This represents a known rate of occurrence of 4.3 /0. A marked tendency to multiplicity has been observed in carcinoma of the colon associated with multiple polyposis or with chronic ulcerative colitis. Some evidence has been obtained of hereditary factors in predisposition to malignant disease of the colon. When the diagnosis of a malignant lesion of the colon or rectum has been made, the entire large bowel must be considered a potential source of malignant disease. An integral part of the management of carcinoma of the large bowel must be constant vigilance to ensure early diagnosis and adequate treatment of both simul taneous and interval lesions. Book excerpt: The purpose of this text is to help develop a better understanding of the nature of cancer, to expand prevention strategies, and to improve diagnostic and prognostic skills centred around multiple primary cancers. Clinical studies focusing on the aetiology of multiple primary cancers, statistical data presented in tables, and discussions on various organ sites and the possible occurrence of multiple primary cancers are addressed throughout the text. The first part reviews various methodological and statistical considerations in the study, management and prevention of primary, secondary and multiple primary cancers. An entire chapter contains tables of incidence rates of various multiple primary cancers that prove helpful in subsequent chapters. Part Two discusses predisposing genetic conditions and various forms of environmental factors that are associated with multiple primary cancers. Finally, the last section review selected types of cancers and focuses on possible clinical implications for occurrences of specific multiple primary cancers.

Book excerpt: In 2006, when my colleague Andrea Renda proposed multiple primary malignancies (MPM) as the subject of the Biennial Report to the 2008 Congress of the Italian - ciety of Surgery, I, together with the Steering Committee, quickly agreed. Recent progress in our understanding of the etiopathology of these neoplasms has led to - novative and significant progress on the clinical level. Importantly, the incidence of the onset of two or more tumors in the same patient suggests a more than casual - lationship. Furthermore, the occurrence of MPM derives from several different me- anisms—viral, iatrogenic, immunologic, environmental, and hereditary—such that any form of treatment must take into account the etiology of these tumors. After an epidemiological introduction, this monograph analyzes various aspects of multitumoral syndromes based on the experience of the Department of Surgical Sciences, along with that of other clinical departments of the University Federico II of Naples. In the discussion of inherited tumors, reference is made to the series of patients treated at the Department of Surgery at the University of Siena. The many topics that comprise this volume range from carcinogenesis to diagnostic strategies, and from epidemiology to innovations in imaging and endoscopic techniques. Among the clinical aspects, particular emphasis is given to sporadic and hereditary syndromes, as these patients are frequently treated by general surgery departments.

The pathophysiological, epidemiological and clinical aspects of hypercalcemia of malignancy are presented in this issue, with a focus on the recently discovered humoral factor responsible for the development of hypercalcemia. With a better understanding of the pathophysiology of this condition and the development of new potent drugs, capable of inhibiting bone resorption, especially bisphosphonates, the clinician will be more successful in correcting hypercalcemia in the great majority of patients.

Available in PDF, EPUB and Kindle. Book excerpt: We welcome the publication of this volume, which discusses the diagnosis of bone tumours with particular reference to children and adolescents. As founder members of the International Skeletal Society we are delighted to learn that the book had its inception at one of the Society's meetings. It reflects, moreover, the combined presentation of radiological and pathological diagnostic information which has been such a feature of the meetings of the International Skeletal Society. We commend it to all readers with an interest in tumours of the skeleton. Hubert A. Sissons Ronald O. Murray Preface The diagnosis of primary bone tumors is often difficult. There are several reasons for this. As primary bone tumors are rare in childhood, practitioners in a number of pediatric subspecialties are not familiar with them. The clinical symptoms and signs are often elusive, the biochemical investigations usually normal and the radio graphic features often uncharacteristic. Even the pathologist, who is the final step in arriving at the proper diagnosis and who has all the available clinical, biochemical and radiographic data, may encounter difficulties. A good tissue sample is the basis for microscopic investigation. However, bone tumors often show an extreme variety of structures which confuse even experienced bone pathologists. Therefore, histo pathologic analysis must take into account all available clinical, biochemical and radiographic data. The close cooperation of the pathologist with clinicians and, especially, radiologists is of the utmost importance.

Book excerpt: A new, fully updated edition of the world’s most famous book on liver diseases—with updating of all areas and inclusion of new specific topics, by internationally renowned specialists This brand new edition of the classic book on hepatology provides a concise, clearly presented and well-structured review across the whole spectrum of hepatobiliary diseases by some of the world’s leading hepatologists and hepatobiliary specialists. Where many other hepatology textbooks provide detailed accounts of basic science and clinical management, Sherlock's Diseases of the Liver and Biliary System, 13th Edition takes a different approach. Concentrating on the clinical decisions to be taken and the relevant supporting data, it is written and edited to maintain Sheila Sherlock's unique approach, in particular the clarity and layout of the text, and the explanatory figures and tables. The book is thus concise, highly accessible, and generously illustrated with over 700 attractive color figures. There is a pithy approach to each disease based both on evidence and on the authors’ experience, the hallmark of this book. Based on these elements, the 12th edition was awarded first prize in the 2012 British Medical Association Book Awards in the Internal Medicine category. Sherlock's Diseases of the Liver and Biliary System begins by introducing the anatomy and function of the liver to readers, continuing then with in-depth coverage of liver biopsy techniques and interpretation, and fibrogenesis and its assessment. There are then chapters on all aspects of liver and biliary disease including acute liver failure, cirrhosis, portal hypertension, hepatic encephalopathy, ascites, hepatitis B and C, alcohol and the liver, non-alcoholic fatty liver disease, drug related liver reactions, cholestatic, autoimmune and genetic liver diseases, benign and malignant tumours and not least liver transplantation. There are also chapters on the liver in pregnancy, in the neonate, infancy and childhood, in systemic diseases and in infections. This new edition also features four new individual chapters focusing on coagulation, non-invasive assessment of fibrosis and cirrhosis; vascular diseases of the liver and portal vein thrombosis, and nutrition in liver disease. Digital downloads of the figures from this edition are offered on a companion website. Internationally recognized and loved, world-renowned hepatology book, first published in 1955 Takes a one-of-a-kind, clinical approach maintaining Sheila Sherlock’s clarity and legacy of presentation Full colour throughout with 700 illustrative figures Wide faculty of international contributors Sherlock's Diseases of the Liver and Biliary System, 13th Edition is an ideal primer in hepatology for students and trainees in hepatology and gastroenterology, and a valuable resource for all specialist gastroenterologists and hepatologists, paediatricians, pathologists, radiologists, general physicians and specialist nurses.

Book excerpt: A second primary cancer (SPC) is a new primary cancer developing in a person with a history of cancer at a new site or tissue and subsequent to the initial cancer. Advances in cancer treatment have led to marked improvements in cancer cure rates over the past 30 years, and the control of risk factors has further increased longevity. Still, cancer survivors are at increased risk not only for recurrence of the primary cancer but also for the development of second primary malignancies, the latter being especially true for survivors of childhood cancer. SPCs are now more common, and research into them has intensified. This monograph provides a synthesis of the current research on SPC, culminating in pertinent summary charts, covering the whole spectrum of first and second primary cancers and the association between them. Readers will gain a general understanding of the epidemiology and of the excess risk of SPCs following an implicated first malignancy. The known or suspected etiologic factors for SPCs are identified, and the field is further narrowed to those factors that are modifiable in practice. Effective preventive measures that might reduce the burden of SPC, both for individuals and across at-risk populations, are presented.Providing practical guidance in terms of possibilities and priorities, this monograph will be a valuable tool for oncologists, general practitioners and health administrators.
