Book excerpt: La perte de l’organe dentaire dans les secteurs postérieurs du maxillaire s’accompagne d’une résorption importante de la crête osseuse et d’une pneumatisation physiologique du sinus maxillaire. Le traitement des maxillaires résorbés par la pose d’implants endo-osseux nécessite alors l’utilisation de techniques chirurgicales. Celles ci visent à retrouver un volume d’os péri-implantaire suffisant par une élévation de la membrane sinusienne et le comblement de l’espace ainsi créé par une greffe d’os autogène ou par un biomatériaux de substitution osseuse. L’utilisation des biomatériaux de substitution osseuse, tels que les allogreffes et les xénogreffes, a permis une simplification de la procédure chirurgicale et une réduction de la morbidité, du temps opératoire et du coût. Les taux de succès cliniques et histologiques de ces matériaux sont comparables à ceux de l’os autogène, assurant la pérennité de l’implant à long terme. Notre étude montre que les effets de l’élévation du plancher sinusien sur la physiologie du sinus maxillaire, à l’aide d’une xénogreffe d’origine bovine (Bio Oss®) et d’une membrane résorbable (Bio Gide®), sont négligeables en présence de patients sans antécédents de sinusites chroniques. La muqueuse sinusienne est capable de s’adapter, particulièrement pour les cas où la clearance sinusienne n’est pas altérée.
