Book excerpt: Objectifs: essayer de comprendre comment et par quels moyens l'Évaluation de Pratiques Professionnelles (EPP) a influencé la pratique médicale quotidienne. Matériel et méthode: Enquête qualitative par le biais d'entretiens semi directifs auprès de médecins généralistes libéraux de Savoie. Résultats: Le statut de médecin (médecin habilité, HAS, URML, maître de stage), l'implication dans la Formation Médicale Continue (FMC), les groupes de pairs et la connaissance du médecin habilité favorisent l'EPP. L'échange entre confrères est perçu comme un point positif. Par contre, l'EPP trop « chronophage », la « peur de l'inconnu » et du jugement sont vécus comme pénalisants. L'EPP a permis aux médecins de se mettre à jour sur les recommandations médicales, de modifier leur logiciel informatique en fonction de leurs besoins. Certains sont plus rigoureux, aussi bien dans leur interrogatoire que pour le suivi des patients. La plupart des médecins ont apporté des changements au niveau de leur prescription (tenue des ordonnances). Le dossier médical a été modifié, tant au niveau administratif qu'au niveau médical. Un médecin propose d'évaluer les connaissances avant d'évaluer les pratiques. Un autre propose un choix de sujet plus adapté, « en fonction des faiblesses de la profession ». La plupart ont jugé l'EPP positivement. Conclusion: La méthode qualitative a apporté une grande richesse de réponses. Le seuil de contentement et les motivations restent très différents d'un médecin à l'autre. Ainsi, le Développement Professionnel Continu (DPC) va peut-être permettre à chaque médecin de pouvoir mettre en œuvre son propre plan d'évaluation, en développant des outils d'aide à l'analyse des pratiques de chacun.

Book excerpt: Les Recommandations de Bonne Pratique (RBP) ont été créées dans le but d'améliorer la qualité et la sécurité des soins avec comme conséquence indirecte une standardisation des pratiques médicales. Pour autant, force est de constater une grande variabilité des pratiques des médecins généralistes face à une même situation clinique. Le but de notre travail est de connaître le positionnement des médecins face à ces RBP et s'ils étaient ou non désireux de les suivre, et dans la négative, pourquoi. Nous avons voulu savoir si certaines caractéristiques propres aux médecins avaient une influence sur la volonté de suivre ou non les RBP (en particulier l'âge et le sexe). Nous avons mené une étude qualitative auprès de 22 médecins généralistes lorrains. Une première série d'entretiens semi-dirigés, se basant sur un cas clinique de cystite aiguë simple de la femme, nous a permis de comprendre l'intérêt porté aux RBP. Puis une deuxième série de 6 entretiens d'ordre biographique a mis en évidence quelques déterminants psychosociologiques de l'influence et du changement d'attitude. Cette étude a montré que même si le genre et la formation sont des facteurs essentiels déterminant le suivi ou non des RBP, d'autres facteurs comme l'expérience, l'éducation, la personnalité entrent en jeu, et nous nous sommes alors intéressés aux concepts d'obéissance et de conformité. A travers ce travail, nous avons également vu que la féminisation de la profession est à l'origine d'une restructuration de l'organisation du temps de travail chez les médecins libéraux. Le phénomène de « judiciarisation » de la médecine a également des conséquences sur les pratiques des médecins. D'une manière générale, la plupart des médecins est satisfait de l'existence des recommandations mais des points sont pourtant à améliorer comme la présentation et le mode de diffusion. L'apparition tout récemment de nouveaux modes de rémunérations avec des objectifs et un système de prime, va également modifier de manière considérable le suivi des recommandations. Notre étude sociologique nous montre alors que l'exercice de la médecine reste un art à dimension « humaine », et tenant compte de la personnalité des médecins mais aussi de la situation du patient. Il est probable que persisteront des pratiques différentes face à des situations qui semblent identiques. Chacun fait avec son expérience, sa sensibilité et sescompétences. C'est ce qui fait l'intérêt et la richesse de cette profession.

It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.

In recent years, we have become more and more aware of the complexities in volved in the relationship between cognition and behavior. People do not always do what they intend to do. Aside from several nonpsychological factors capable of reducing cognition-behavior consistency, there seems to be a set of complex psychological mechanisms which intervene between action-related cognitions, such as beliefs, expectancies, values, and intentions,and the enactment of the be havior suggested by those cognitions. In our recent research we have focused on volitional mechanismus which presumably enhance cognition-behavior consistency by supporting the main tenance of activated intentions and prevent them from being pushed aside by competing action tendencies.
