Book excerpt: Objectif : Le réentrainement à l'effort à montrer un bénéfice sur les performances de marche à court terme chez le patient hémiparétique chronique. On ne connaît pas l'évolution de ces paramètres de marche à plus long terme. L'objectif est d'analyser l'évolution de la marche hémiparétique à plus d' 1 an. Matériel Patients et Méthode : étude rétrospective. 27 sujets hémiparétiques chroniques dans les suites d'un AVC hémisphérique ancien (plus de 6 mois), autonomes à la marche, ont participé à un programme de réentrainement à l'effort. 17 patients ont été revus à plus d' 1 an. Ils ont été évalués à T=0, T=4 semaines et T> 1an par un test de marche de 6 minutes (TDM6), un test de marche sur 10 mètres à vitesse maximale (10 m), et un get up and go test (GUGT). Statistique : Tests non paramétrique type Friedman, Wilcoxon et Mann et Whitney. Résultats : On retrouve une amélioration statistiquement et cliniquement significative du TDM6, 10 m et GUGT à 4 semaines avec un effet rémanent à plus d' 1 an malgré une altération significative du TDM6 entre la fin du programme et à plus d' 1 an. Discussion: Cette étude montre l'effet très positif à court et long terme d'un programme de réentrainement à l'effort sur les paramètres de marche de l'hémiplégique chronique. L'enjeu est de maintenir une activité physique adaptée régulière et d'intensité suffisante à domicile afin de conserver le bénéfice maximal du stage. Un programme d'éducation thérapeutique et le réseau hôpital-ville permettraient-ils d'améliorer le maintien des capacités d'endurance en favorisant la poursuite d'une activité physique adaptée ?

As it is only the subject that has access to this mental or physical action, the goal is to help the subject with the verbalization, putting the implicit knowledge of this action in words. Yet, to achieve this verbalization, we need guidance, because we don't know how to give such a description ourselves. If we leave the interviewee without guidance, he risks remaining in the implicit, speaking in a general way, or in an incomplete or imprecise way, commenting instead of describing. The explicit and precise description we aim is the opposite of the implicit, meaning a fine, detailed and complete way of putting the action in words. Book excerpt: During the First European Congress of Anesthesiology, held in Vienna, Austria, in September, 1962, panel discussions on nineteen different subjects were held, each lasting approxi mately three hours. One, concerning Controversial Aspects of Resuscitation, was later edited by its chairman, PETER SAFAR, and published in 1963. At the request of the publisher, the discussion on hypnosis has been edited in a similar manner. The participants in the discussion on Hypnosis in Anes thesiology had agreed, prior to the meeting, on a list of questions to be debated, and Dr. STOKVIS' introductory statement on the nature of hypnosis was circulated among them in appropriate translations in order to give the debate a starting point. It had also been agreed that no formal papers should be read after this introduction, and that the participants should use at will the German, English or French language. Following each contribution it was the chairman's task to give a brief summary in the two other languages. Discussion was therefore somewhat slowed down and occasionally rendered difficult by misunderstandings or the omission of details. Drs. GUEGUEN, MOSCONI, and VOLGYESI, who were not able. to attend, had sent written contributions which the chairman presented briefly during the discussion. The present text has been worked out of the tape recording of the discussion and the chairman's correspondence with the panelists some time after the meeting. Whenever the original contributions were in French or German, the English translation has been the chairman's.
