Book excerpt: Un traitement endodontique nécessite la réalisation d'une désinfection canalaire optimale associant une mise en forme adéquate du canal et le passage d'une solution d'irrigation ayant pour but de parfaire l'élimination de l'intégralité des tissus du complexe pulpaire. Quelle que soit la technique qui était envisagée, les opérateurs réalisaient la mise en forme à l'aide d'instruments en acier en effectuant un mouvement de va-et-vient vertical. De nombreux accidents pouvaient survenir lors de cette procédure tels que la déviation de la trajectoire canalaire, des transports internes ou externes, une non reproductibilité du traitement, etc. Face à ces inconvénients, en 1985, Jim Roane introduit la notion de « forces équilibrées». Ce concept repose sur le principe que ces limes en aciers doivent être animées d'un mouvement alterné horaire / antihoraire asymétrique et non de mouvements verticaux. Quelques années plus tard, le lancement des instruments Nickel-Titane amène un renouveau dans le domaine de l'endodontie avec de nombreux avantages par rapport aux instruments en acier inoxydable. Aujourd'hui, il existe deux instruments qui, avec un moteur spécifique, répondent au concept du mouvement réciproque: WaveOne® et Reciproc ®. Ces instruments permettent à l'opérateur de mettre en forme un canal à l'aide d'un seul instrument tout en respectant les objectifs mécaniques et biologiques d'un traitement endodontique.

Ainsi, de nombreux systèmes mono ou pluri instrumentaux de PEC mécanisé ont vu le jour qui auraient pour objectif de faciliter la MEF et d'en améliorer la qualité. Le premier objectif de ce travail était de faire un point de clarification terminologique et technique sur le PEC, les recherches autour de cette thématique ayant rapidement révélé une ambiguïté de terminologie autour des premières étapes du traitement. Le second objectif de ce travail était de réaliser une revue de littérature évaluant les apports du PEC sur quelques critères sélectionnés pour leur incidence clinique, à savoir la fracture instrumentale, le maintien de la trajectoire canalaire, l'extrusion apicale de débris et le temps opératoire.
