Under the model, active-duty personnel would be assigned to civilian settings during peacetime. The study on which this monograph is based explored the feasibility of this model from a civilian perspective, focusing on civilian receptiveness to the proposed arrangement and identifying potential barriers and concerns. The study found that civilian medical organizations are generally receptive to the idea of such a model and that DoD could consider conducting a pilot study to assess the effectiveness of the model in improving military medical readiness.

Book excerpt: This investigation evaluated Continuing Medical Readiness Training (CMRT) skills among medical personnel assigned to the USAF Medical Center at Wright-Patterson AFB Ohio. Results of the study indicate that personnel tended to score higher in the area of disaster medical skills than in general field and wartime specific skill categories. In comparing population subgroups, it was found that officers scored higher than enlisted personnel across all major skill categories. Individuals whose primary duties involved direct patient care scored higher in disaster medical and wartime specific skill categories than those involved primarily in indirect patient care, while no significant difference was observed in the general field skill category. No significant difference between those with seven or less years of experience and those with more than seven years experience was found in the disaster medical and wartime specific skill categories. Those with more than seven years experience did however, score significantly higher in general field skills. Book excerpt: Medical Readiness Training Exercises (MEDRETES) are conducted by military medical units in a field environment, where US military personnel conduct medical evaluation, treatment, and health education for persons who are not health care beneficiaries of the US government. The primary mission of these exercises is the training of military personnel. The Army National Guard (ARNG) has actively pursued these training opportunities which provide training in environments which closely approximate what they might experience in times of conflict. over the past ten years the bulk of this training has occurred in the Southern Command (SOUTHCOM) area of responsibility. In 1987, the ARNG established a Field Medical Training Site in Pan a which provided the process and infrastructure for medical units to deploy to Central or South America on 2- 3 week annual training (AT) periods to participate in MEDRETES. The ARNG MEDRETE program in SOUTHCOM provides an excellent case study showing how reserve component forces, while in an AT status, can serve as a vital resource to a CINC's peacetime strategy, as well as when mobilized in time of war.

A second purpose was to describe the focus of medical readiness training required to meet knowledge and performance needs identified in the study. The sample consisted of 1,085 officer and enlisted personnel in six nursing specialty areas in the active and reserve components of the AMEDD. Readiness competency was measured by participants performance on competency based and written exercises that were developed for the study. A demographic questionnaire and semi-structured interviews were used to elicit participants' institutional training, operational assignment, and self development experiences related to their readiness competency as measured by the competency based and written exercises. It was concluded that study participants need training in clinical skills and functions that they do not routinely perform when working in civilian or military medical facilities during peacetime, but that they perform frequently or perform as life saving measures when working in a deployed or field status. It was recommended that a readiness training program focused on these skills and functions be developed for nursing personnel in the active and reserve components of the AMEDD.

Available in PDF, EPUB and Kindle. Book excerpt: Efforts in medical readiness, peacetime health services and quality assurance were examined. Previous studies of the US military medical organization and the current structures of selected Allied military medical organizations were reviewed. As a result of the growth of interest in improving wartime and peacetime effectiveness and efficiency, several organizational improvements have been made to eliminate unnecessary triplication of effort in the US military medical system. Medical readiness has been improved in the areas of intelligence, research and development, and logistics. While progress in readiness has occurred in plans, operations and training, there is need for improvement. Peacetime health services have also experienced the emphasis on joint operations. Regionalization, has fallen short of original expectations and needs revitalization. The Joint Interservice Resource Study Group process offers additional opportunities to review medical functions and services. Quality assurance has benefited from the joint efforts of the Tri-Service Committee on Quality Assurance and the Interservice Training Review Organization. Common credentialing criteria, however, will require standardization of the professional training base which can only occur after joint functional reviews of specialty training. The major recommendation made by the study was that Department of Defense direct a system-wide analysis of medical functions, requirements, programs and resources to determine the best military medical organization for both wartime and peacetime.
