Book excerpt: Preoperative Magnetic Resonance Imaging allows now a better analysis of the relationship between intracranial tumors and the surrounding brain tissue. Even very small deepseated tumors can be identified during the operation by intraoperative ultrasound. These technological advances have had a great influence on indications, planning strategies and microsurgical techniques. The author shows the functional anatomy of the brain with the eye and the mind of a microsurgeon and demonstrates well known topographical relationships under the aspect of an utmost gentle operative approach.

Book excerpt: Modern diagnostic imaging and operative approaches have witnessed significant improvements in our times. Computerassisted methods are in use in all microsurgical fields. Neuronavigation, novel stereotactic methods, endoscopic procedures, magnetic resonance imaging, ultrasound and the progress in pre- and intraoperative epilepsy diagnostics must be mentioned in particular in this connection. However, the insights of neuroanatomy and neurophysiology have not become obsolete thereby, on the contrary: such knowledge is imperative and a prerequisite for all neurosurgeons, nowadays more than ever before. Otherwise, excellent modern approaches are liable to fall into discredit if microanatomical aspects are neglected. The goal of this book is two-fold: first, to guide the resident towards a fruitful application of anatomical basics in visualizing and operative techniques. Second, to draw attention to as many anatomical norm variants as possible to forestall complications during surgery. Standard methods, such as the pterional approach, often confront the surgeon with a range of anatomical variants.

Book excerpt: This practical, step-wise text covers the surgical approaches, resection strategies and reconstruction techniques used for each type of presenting tumor of the spine. Demonstrating the variety of anterior, posterior and intradural approaches and stabilization techniques, and spanning from pathologies of the craniocervical region to sacral and intradural pathologies, each chapter is generously illustrated with figures, radiographs and intraoperative photos. The chapters themselves follow a consistent and user-friendly format: the anatomy and biomechanics of a specific region, patient evaluation, essential oncologic principles, the decision-making process, and technical steps of surgery. A representative case illustration is provided at the conclusion of each chapter, exemplifying pertinent concepts described. Additionally, video segments accompany selected chapters, providing real-time illustration of surgical techniques. Technical and in-depth, yet highly accessible, Spinal Tumor Surgery: A Case-Based Approach is an essential resource for orthopedic spine surgeons, neurosurgeons, and surgical oncologists operating on tumors of the spine.

Available in PDF, EPUB and Kindle. Book excerpt: In this comprehensive and original monograph, Professor Rene Louis presents in minute detail in one volume the gross anatomy, nerve supply, biomechanics, and microcirculation of the spine. He also presents the surgical approaches to the vertebral bodies and their contents. Professor Louis is a great anatomist and this book has been prepared from his personal observations, both anatomical and surgical. His studies have been meticulously conducted and contain much original research, for instance his work on the motion of the neural elements within the lumbar vertebral canal. The illustrations are neady all original and very often a photograph of the neural or vascular elements is presented alongside a drawing of a given important anatomical area. For all these reasons, this inspiring treatise makes a valuable contri bution to our knowledge of the spine and forms a basis for an under standing of the intricacies of surgical anatomy and approaches. It will be especially valuable to the spinal surgeon, but the medical student, the orthopedic resident (or registrar), and the anatomist will also find it extremely useful. Leon L. Wiltse, M.D.

Book excerpt: As an addition to the European postgraduate training system for young neurosurgeons we began to publish in 1974 this series devoted to Advances and Technical Standards in Neurosurgery which was later sponsored by the European Association of Neurosurgical Societies. The fact that the English language is well on the way to becoming the international medium at European scientific conferences is a great asset in terms of mutual understanding. Therefore we have decided to publish all contributions in English, regardless of the native language of the authors. All contributions are submitted to the entire editorial board before publication of any volume. Our series is not intended to compete with the publications of original scientific papers in other neurosurgical journals. Our intention is, rather, to present fields of neurosurgery and related areas in which important recent advances have been made. The contributions are written by specialists in the given fields and constitute the first part of each volume. In the second part of each volume, we publish detailed descriptions of standard operative procedures, furnished by experienced clinicians; in these articles the authors describe the techniques they employ and explain the advantages, difficulties and risks involved in the various procedures. This part is intended primarily to assist young neurosurgeons in their post graduate training. However, we are convinced that it will also be useful to experienced, fully trained neurosurgeons.

Book excerpt: The 17th volume of the "Advances in Neurosurgery" contains a selection of the scientific reports of the 39th annual meeting of the German Society for Neurosurgery, which was held in Cologne from May 8-11, 1988. The first section deals in particular with the long-term results of severe head injuries, as well as with problems of the acute traumatic hematomas and brain edema. The second section covers microsurgical experiences. Attention is focused on the anatomy and operative technique for lesions in and around the jugular foramen and the craniospinal transition. The third section describes the special new results of brain death determination. Beyond this, numerous contributions on clinical and research results were presented in a poster exhibition which was systematically studied in order to give younger neurosurgeons the opportunity for extensive discussion. The volume contains new information which will bring specialists up-to-date on the latest clinical and research developments in the field.

Book excerpt: Advances in Neurosurgery presents the experience and research results of modern neurosurgeons confronted with urgent diagnostic and therapeutic problems. The first topic here is spontaneous intracerebral hematomas, whose localization determines the form of treatment and usually its outcome. Newer therapeutic approaches such as stereotactic removal, endoscopy and fibrinolysis supplement the results of standard operative methods. Secondly, the hydrocephalus malresorptivus is covered, presenting experience with the diagnostic value of intrathecal infusion tests, CT-cisternography, evoked potentials, and MRI CSF-flow studies, as well as aspects of neurosurgical intensive care such as barbiturate treatment, respiratory problems in head injury patients, auto-regulation mechanisms, and hospital infection. The last section is devoted to peripheral nerve surgery, reports on experience with microsurgery and various forms of anastomosis in both traumatic and tumorous nerve lesions.
