Specifically, MICCAI offers a forum for the discussion of the state of art in computer-assisted interventions, medical robotics, and image processing among experts from multi-disciplinary professions, including but not limited to clinical doctors, computer scientists, and mechanical and biomedical engineers. The expectations of society are very high; the advancement of medicine will depend on computer and device technology in coming decades, as they did in the last decades. We received 321 manuscripts, of which 41 were chosen for oral presentation and 143 for poster presentation. Each paper has been included in these proceedings in eight-page full paper format, without any differentiation between oral and poster papers. Adherence to this full paper format, along with the increased number of manuscripts, surpassing all our expectations, has led us to issue two proceedings volumes for the first time in MICCAI’s history. Keeping to a single volume by assigning fewer pages to each paper was certainly an option for us considering our budget constraints. However, we decided to increase the volume to offer authors maximum opportunity to argue the state of art in their work and to initiate constructive discussions among the MICCAI audience.

The pioneering works on this CAS topic date from the 1980's, with applications in orthopaedics and biomechanical models of bones. More recently, however, biomechanical models of soft tissues have been proposed since most of the human body is made of soft organs that can be deformed by the surgical gesture. Such models are much more complicated to handle since the tissues can be subject to large deformations (non-linear geometrical framework) as well as complex stress/strain relationships (non-linear mechanical framework). Part 1 of the volume presents biomechanical models that have been developed in a CAS context and used during surgery. This is particularly new since most of the soft tissues models already proposed concern Computer Assisted Planning, with a pre-operative use of the models. Then, the volume addresses the two key issues raised for an intra-operative use of soft tissues models, namely (Part 2) “how to estimate the in vivo mechanical behavior of the tissues?” (i.e. what are the values of the mechanical parameters that can deliver realistic patient-specific behavior?) and (Part 3) “how to build a modeling platform that provides generic real-time (or at least interactive-time) numerical simulations?”

Book excerpt: A constitutive inverse modeling framework is presented, relying on conventional indentation testing along with real-time three dimensional ultrasound imaging of the internal tissue deformation. The internal organ deformation field is estimated with a novel, mechanically regularized nonrigid image registration algorithm. A physically- based visco-elastic constitutive model of the liver response is developed and its material parameters are estimated within the proposed inverse modeling framework. Three perfused porcine livers were characterized using tests representative of surgical manipulation, including cyclic loading tests spanning applied strain rates between 0.01 s -1 and 1.0 s -1 and stress relaxation tests. The proposed model and the identified material parameters offer good fit to the experimental response and show good predictive capability for alternative loading histories. The proposed material testing methods are independent of imaging modality and constitutive law, suggesting potential applications for other tissues and scales (i.e. nanoindentation, confocal microscopy, etc.).
