Book excerpt: CONTEXTE : L'alcool est un problème de santé publique majeur en France (41 000 décès sont attribuables à l'alcool en 2015). Alors que la consommation globale d'alcool diminue depuis les années 60, celle des femmes augmente. La consommation d'alcool chez la femme est plus nocive que chez l'homme et du fait d'une stigmatisation plus importante, les femmes consultent peu pour ce motif. Les médecins généralistes étant les médecins de premier recours pour le repérage d'un trouble de l'usage d'alcool : Comment les femmes alcoolodépendantes souhaitent voir aborder leur problème par leur médecin généraliste ? OBJECTIF : L'objectif principal de ce travail est d'évaluer la perception des femmes sur le repérage du trouble de l'usage d'alcool par leur médecin généraliste. MÉTHODE : Il s'agit d'une étude qualitative basée sur l'analyse d'entretiens individuels réalisés entre le 1er mars et le 21 avril 2021 à partir d'un guide d'entretien semi-directif et évolutif, auprès de 18 femmes alcoolo-dépendantes hospitalisées pour sevrages ou suivies en consultation d'addictologie au CHU de Montpellier. RÉSULTATS : Selon les femmes interrogées dans cette étude, les médecins généralistes peuvent avoir une relation privilégiée avec leur patiente et semblent, en tant que médecin de premier recours, être les acteurs majeurs du repérage de la maladie d'alcool. Ce repérage est malgré tout rarement effectué puisque ce sont majoritairement les patientes qui abordent le sujet. Le repérage précoce de la maladie d'alcool est majoritairement perçu comme bénéfique mais jugé comme difficile et insuffisant notamment du fait de nombreux freins liés aux patientes mais aussi vis à vis des médecins. Les freins liés aux patientes (tels que la honte, le tabou, la consommation cachée, ...) sont particulières au trouble de l'usage d'alcool chez la femme. Bien que les patientes interrogées aient conscience du rôle de leur propre motivation dans la démarche de soin, elles ont de nombreuses attentes vis à vis de leur médecin généraliste dans sa manière d'aborder l'alcool. Ces attentes peuvent être des pistes d'amélioration au repérage (se sentir en confiance, avoir un médecin disponible, formé et à l'écoute, être informé/questionné). CONCLUSION : Le repérage précoce du trouble de l'usage d'alcool par les médecins généralistes est jugé comme insuffisant par la majorité des patientes interrogées. Elles décrivent de nombreuses attentes vis à vis du médecin généraliste dans sa manière d'aborder la maladie d'alcool, qui se rapprochent d'outils déjà existants comme le RPIB ou l'EM. Ces pistes d'amélioration du repérage sont dépendantes de la formation et de la disponibilité des médecins.

La médiane de la Freq RPIB chez les patients de plus de 65 ans était de 30 %, identique à celle des plus jeunes. Une formation préalable en addictologie était significativement associée à la Freq RPIB. Les obstacles principaux étaient le temps consacré, la rémunération et des connaissances insuffisantes concernant les particularités du mésusage d'alcool chez la personne âgée. Discussion : au-delà des formations, indispensables, il faudra se pencher sur un mode d'organisation incitatif pour augmenter la réalisation du RPIB en médecine générale.

Des outils destinés à améliorer le repérage et l'accompagnement sont mis à sa disposition. L'addictologie évolue : la prévention des risques et réduction des dommages regroupées sous le terme réduction des risques (RdR) se développe et une nouvelle classification apparaît. L'objectif de cette étude était d'évaluer l'évolution de l'accompagnement des patients. Les objectifs secondaires étaient d'explorer la place du Repérage Précoce et Intervention Brève (RPIB) et l'impact des changements récents en addictologie en pratique. Méthode : Une étude qualitative par entretiens semi-dirigés a été réalisée auprès de 12 médecins généralistes du Nord ayant ou non une compétence en addictologie. Les données ont été analysées par codage à l'aide du logiciel NVivo12®. La saturation des données a été atteinte. Résultats : Le repérage précoce n'était pas correctement intégré. Certains médecins utilisaient l'intervention brève et la trouvaient d'autant plus efficace qu'elle était répétée. Les questionnaires standardisés n'étaient pas utilisés et le changement de classification ne semblait pas avoir d'impact en pratique. Par contre, le changement de paradigme a bien été intégré, tous pratiquaient la réduction des risques et rejetaient les anciennes méthodes. Plusieurs conditions nécessaires à cet accompagnement et des pistes d'améliorations ont été évoquées. Conclusion : Les mesures mises en place pour promouvoir le RPIB ne semblent pas suffisantes, un travail de diffusion reste à faire. Améliorer l'accès à la prévention et l'accompagnement patients présentant un TUA est une des priorités du schéma régional de santé en place depuis 2018. Une évaluation de ce plan est prévue en 2023.

Book excerpt: Résumé Introduction : Les pathologies liées à l’alcool sont nombreuses et sont à l’origine d’un grand nombre d’admissions dans les services d’urgence. L’objectif de ce travail est d’évaluer la mise en œuvre du dispositif de « repérage précoce et intervention brève » (RPIB) en alcoologie dans un service d’urgence. Matériel et méthode : Etude transversale descriptive réalisée par entretien auprès du personnel des urgences du CHU d’Amiens formé au RPIB. Dix médecins et infirmiers ont été formés afin qu’ils puissent l’intégrer à leur pratique quotidienne. Nous les avons interrogés 4 mois après la formation initiale. Résultats : Aucun ne l’a intégré dans sa pratique. Les soignants n’ont eu aucun refus de la part des patients mais expriment une crainte concernant les réactions des patients en lien avec le déni et la honte qui peuvent être liés à ce sujet. Pourtant tous sont convaincus de la nécessité de cet enjeu de santé publique. Les freins évoqués sont le manque de temps et le fait qu’ils jugent les urgences comme inadaptée à cette pratique. Les attentes du personnel seraient l’intégration du questionnaire FACE au logiciel utilisé dans le service ou l’utilisation d’une borne informatisée en salle d’attente. Le manque de retour de la part des partenaires de santé et de suivi du patient sont également des éléments cités comme démotivants. Discussion : Le RPIB a fait ses preuves dans la réduction de consommation à risque après un passage aux urgences. Le personnel prône sa pratique mais se heurte à des contraintes de mise en œuvre. Les obstacles retrouvés sont comparables à ceux décrits dans la littérature. Il apparaît donc nécessaire de trouver des outils adaptés afin de faciliter sa réalisation face aux contraintes spécifiques des services d’urgence.

L’un participant à une campagne structurée de prévention organisée par une association locale « Agir en Promotion de la Santé » et l’autre informé par courrier simple. Méthode : Recueil du nombre de tests de dépistage réalisés dans chaque groupe. Résultats : Les 42 médecins du groupe encadré par l’association « APS » ont réalisé en moyenne 25 tests alors que les 95 autres n’en ont pratiqué que 5 en moyenne. Parmi ces derniers, près de 60% déclarent ne pas avoir reçu ou lu les documents qui leur avaient été adressés. Discussion : Afin de promouvoir au mieux les différentes stratégies de diffusion de ces techniques visant à réduire la consommation d’alcool des buveurs « à risque » ou « à problèmes », il faut privilégier les démarches de proximité, valorisantes, incitatives auprès des médecins généralistes. Conclusion : L’initiative de l’association « Agir en Promotion de la Santé » est encourageante pour aider à changer et améliorer les pratiques de prévention au sein des cabinets de médecine libérale.
