Book excerpt: Currently, the data sources available suggest that overall methamphetamine availability and use in Europe remains relatively low, in comparison with other stimulant drugs. However, when viewed collectively, the data suggest that threats in this area appear to be growing to a point at which they cannot be ignored. Wastewater analysis and other epidemiological data sources suggest some recent methamphetamine diffusion to new markets, with the drug now appearing in some southern European countries and becoming more prominent in parts of central Europe. There is also evidence of large-scale production in Europe for non-European countries, and significant production in some countries bordering the European Union. The increasing presence of Mexican groups involved in large-scale production of smokable crystal methamphetamine in Europe is of particular concern. This could result in increased availability of this drug in the European Union in future. Overall, current developments in the availability of precursor chemicals and in European synthetic drug-production capacity indicate that if demand for this drug increased in Europe, illicit producers would be able to respond quickly. While still relatively low, seizures of methamphetamine, both in terms of numbers and quantities, are increasing: with very large increases in the quantity of methamphetamine seized in Turkey together with reports of methamphetamine production in that country. Many of the indicators on drug use and some indicators of the drug market are ill-adapted to distinguish between amphetamine and methamphetamine. As a result, especially in view of the overall greater part played by amphetamine, they are likely to be insensitive to changes in methamphetamine use in Europe. Currently, the availability of other stimulants on the European drug market appears to be high. However, if this were to change, a threat exists that methamphetamine availability and use could grow rapidly. Given the prominent role played by this drug in the global market for stimulants and its association with both severe acute and chronic health problems, it must be considered one of the priority areas for drug monitoring. In the current environment, where new drug trends can emerge rapidly, contingency planning to improve Europe's preparedness for any future escalation in problems in this area would also be beneficial.

Historically, the use of methamphetamine has been confined largely to the Czech Republic and Slovakia; however, recent signs of the spread of methamphetamine linked to different European countries have sparked further investigation of this topic. The methodology used is based on the triangulation of data collected using a number of investigative approaches and from multiple sources. The findings summarised in this paper include insights into the chemistry of methamphetamine and the history of its use in Europe since the 1930s. Signs of increased involvement of organised crime groups in methamphetamine markets and possible scaling-up of production are noted. In some countries, there is evidence to suggest that methamphetamine use is increasing, while new injection trends have been detected among small groups of gay men in large cities (London, Paris). Worrying reports are emerging from south-east Europe that crystal methamphetamine smoking is a limited, but emerging, problem, with the possibility of spreading among vulnerable populations. Negative consequences for physical and mental health are associated with the use of methamphetamine, which may also involve high levels of sexual risk-taking. Evidence exists for effective health and social responses, based on cognitive behavioural and contingency management approaches. The study notes that the knowledge available in Europe on the use of methamphetamine and the associated problems remains incomplete, and highlights information and research gaps. The report concludes that, although methamphetamine use is not a major phenomenon in Europe, even at a relatively low prevalence, it is a drug that has the potential to cause significant harm.

As a result of the large flow of drugs through the country, combined with widespread socioeconomic insecurity due to decades of sanctions and domestic political factors, Iran reports levels of substance dependence that are very high by global standards. Iranian Drug Control Headquarters (DCHQ) reported that in 2019, an estimated 2.8 million Iranians (5.4 % of the adult population aged 15-65) were living with substance use disorders. This included an estimated 225 000 regular methamphetamine users, a reduction from the estimated 400 000 regular users in 2015. It should be noted that estimating community levels of dependent or regular drug use is methodologically and practically challenging so the precision of estimates in this area should be viewed with caution. Since mid-2019, Iranian public officials have reported that methamphetamine use has been increasing due to the influx of cheap Afghan methamphetamine and a simultaneous rise in the prices of other drugs. In the Iranian calendar year 1398 (March 2019 to March 2020), approximately 950 tonnes of drugs was seized in Iran, an increase of 20 % on the previous year. Some 80 % of the total quantity of drugs seized was opium, totalling 761 tonnes and representing a 22 % increase on the previous year. Although methamphetamine, known locally as 'shisheh' (meaning 'glass' in Persian), represented only 1.8 % of all seizures, 17 tonnes of the drug were seized, which was a considerable increase (208 %) on the previous year. There are now regular reports in the Iranian media featuring statements by authorities claiming that methamphetamine trafficking from Afghanistan is increasing, with traffickers appearing to use pre-existing opiate trafficking networks along the Balkan and Southern routes. In November 2020, the head of the Anti-Narcotics Police announced that between March and November 2020, Iran had seized 10 tonnes of methamphetamine, 9 tonnes of which were reported to have been trafficked from Afghanistan (Babaei, 2020). This report builds on the study published by the European Monitoring Centre for Drugs and Drug Addiction (EMCDDA) on the Afghan methamphetamine trade (EMCDDA, 2020) and addresses the threats posed by Iran's potential emergence as a transhipment point for Afghan shisheh, as suggested by a reported increase in methamphetamine seizures originating from Iran and bound for countries in Southeast Asia and Oceania. This report is based on interviews with Iranian drug treatment practitioners, law enforcement officers, United Nations (UN) officials and key informants involved in drug supply, in combination with an analysis of more than 70 Iranian newspaper articles and several Turkish, Kurdish and Australian news articles that outline the scale of methamphetamine trafficking from Iran.
