The author's intent is to share her experience with others who may be struggling with a cancer diagnosis, and hopefully, give them strategies to improve the quality of their lives. AUTHOR BIOGRAPHY Carol A. Miele retired from nursing after a 45 year nursing career. Growing up in Old Forge, Pennsylvania, she graduated from Community Medical Center School of Nursing in 1965, received a BS Degree in Nursing Education at Wilkes College in 1977 and a Masters of Public Health at East Stroudsburg University in 2000. The author enjoyed many roles in nursing: Medical-Surgical, Psychiatric, Operating Room, Emergency Room, Recovery Room and Intensive Care Unit clinical nurse. Also. She was an Occupational Health Nurse, Nursing Instructor, Assistant Director of Nurses, and Quality/Risk Manager in the hospital, home care, hospice and residential environments. Diagnosed with Stage 4 Breast Cancer and metastasis to bone in October 2010, she went into remission in June 2011 following months of Chemotherapy. Enjoying her stable condition, she lives with her husband and dog Flora in Indian Mt. Lakes, PA. They have two married daughters, Marisa and Kristen.

We aim to give a clear overview of the knowledge about CTCs, framed in the context of breast cancer, by analysing basic and clinical research carried out so far. In a broader sense, we will address what are the main clinical needs of this disease based on its molecular heterogeneity (subtypes) and lay out the knowledge and understanding that CTCs are giving about it and how they are contributing and can still improve the better monitoring and management of breast cancer patients. We will discuss the evidences of the use of CTCs as a tool to monitor cancer progression and therapy response, based on the prognostic and predictive value they have, as well as a tool to unravel mechanisms of resistance to therapy and to identify new biomarkers allowing to predict therapy success. Moreover, we will analyse the main aspects of ongoing clinical trials and how they can contribute to determine the clinical utility of CTCs as a breast cancer biomarker. We will also touch upon general knowledge or basic notions of the biology of the metastatic process in epithelial cancers, in order to understand the origin and biology of CTCs. In this sense, we will pay special attention to EMT (epithelial to mesenchymal transition), dormancy and minimal residual disease, three key aspects that determine the outcome of the disease. We will also cover general aspects on the isolation and characterization techniques applies to the study of CTCs, and also the possibilities that the study of CTCs, as a biomarker with biological function, is opening in terms of understanding the biology of metastatic cells and the identification of therapeutic targets based on the functional and molecular characterization of CTCs. Lastly, we will try to foresee the future of CTCs in terms of clinical application and implementation in the clinical routine.

MicroRNAs (miRNAs) are recently described small endogenous noncoding RNAs implicated in the posttranscriptional control of gene expression. These tiny molecules are involved in developmental, physiologic phenomenon as well as pathologic processes including cancers. In fact, miRNAs have emerged as critical regulators of cancer progression, invasion and metastasis. This is mainly because a single miRNA can affect several downstream genes and signaling pathways with oncogenic or tumor suppressor actions depending on the target genes affected. Due to this multimodal downstream signaling effects, these small endogenous molecules hold great promise in metastasis prevention and treatment. Modulating the activity of miRNAs can provide opportunities for novel cancer interventions. Targeting miRNAs could become a novel prognostic and therapeutic strategy to prevent the future development of metastasis. Thus, miRNAs could also serve as a potential targets for anti-metastatic therapy. The book explores how the expression of miRNAs in the primary tumor could be silenced using antagomirs (chemically modified anti-miRNA oligonucleotides), which could prevent the development of metastasis; whereas once metastasis develops then it could be treated with miRNA mimics for inducing its expression for the treatment. Therefore, development of miRNA-based prophylactic therapies could serve as precision and personalized medicine against future development of metastasis of breast and other cancers.
