Book excerpt: Actuellement, des dispositifs de mesure en continu de la glycémie existent mais peu d’études ont évalué leur utilisation dans le cadre des troubles de la perception des hypoglycémies. L’objectif principal de notre étude était d’évaluer l’intérêt du holter glycémique chez 19 patients diabétiques de type 1, qui avaient perdu partiellement ou totalement la perception de leurs hypoglycémies. L’objectif secondaire était d’étudier la corrélation entre glycémie capillaire et glucose interstitiel et d’identifier des facteurs pouvant influencer cette corrélation. Après 3 mois de port de la Paradigm Real Time®, 60% des patients ont récupéré des symptômes d’hypoglycémie, avec une augmentation du seuil de ressenti des hypoglycémies de 0.24g/L en moyenne. La restauration des signes cliniques d’hypoglycémie est liée à l’ancienneté de la perte de la perception. Ces effets se maintiennent 3 mois après la fin de l’étude. De plus, nous avons retrouvé un coefficient de corrélation entre glucose interstitiel et glycémie capillaire de 0.83 et identifié plusieurs facteurs influençant cette corrélation : la période de port du capteur, l’hypoglycémie et le délai entre la pose du capteur et la mesure. Cependant, ces technologies restent contraignantes : 1/3 des sujets ont arrêté l’étude avant la fin alors qu’un réel bénéfice était en jeu. Des progrès techniques doivent donc encore être réalisés pour s’adapter au mieux à la vie quotidienne de nos patients.

Méthodes : Trente-sept patients diabétiques institutionnalisés ont porté, en aveugle, un dispositif FreeStyle Libre Pro® de mesure continue du glucose interstitiel durant 14 jours entre Janvier 2019 et Juin 2020. Les taux de glucose interstitiel et les glycémies capillaires ont été comparés en calculant la MARD (valeur moyenne des différences relatives absolues) globale pour 971 paires de glucose. Les paires de glucose ont été placées dans des grilles d’erreur de Clarke et de Parkes afin d’évaluer l’efficacité du système de mesure continue du glucose. Résultats L’âge moyen était de 86,8 ± 6,8 ans. La MARD globale était de 15 % et variait de 14,7 à 15,6 % pour des valeurs de glucose entre 70 et 180 mg/dL, >180 mg/dLet >250 mg/dL. Les pourcentages globaux des valeurs de glucose interstitiel situés dans des plages de ± 15%, ± 20% et ± 30% par rapport à la glycémie capillaire de référence étaient respectivement de 60,7 %, 74 % et 9,3 %. La proportion de valeurs situées dans les zones A et B de la grille d ’erreur de Clarke et de Parkes étaient respectivement de 98,2 % et 99,1 %. Conclusion : nos résultats d’efficacité du dispositif FreeStyle Libre Pro® sont similaires à ceux déjà observés dans des populations de patients diabétiques plus jeunes, en ambulatoire ou en hospitalisation. Nous démontrons ainsi pour la première fois que le dispositif de mesure continue du glucose FreeStyle Libre Pro® est efficace chez des sujets âgés diabétiques vivant en institution. Même si les résultats de notre étude nécessitent d’être confirmés dans des études de plus grande ampleur et notamment pour les valeurs basses de glycémies, ils sont rassurants quant à l’utilisation de ce dispositif au sein de cette population.

The report makes extensive use of the Iraq Household Socio-Economic Survey, the first nationwide income and expenditure survey since 1988. IHSES data is complemented income and expenditure data from a wide range of other measures of living standards, allowing us to analyze living standards in a holistic way. The analysis presented here was performed with two main goals first, to inform the Government s Poverty Reduction Strategy; and second, to serve as a baseline for future assessments of changes in living standards and the identification of critical issues for deeper examination. Iraqi living standards have two unusual characteristics. First, they have fallen over the past generation. Second, they feature surprisingly little inequality. These characteristics are both rooted in Iraq s recent history of authoritarian government, war, military occupation, insurgency, and civil strife leading to infrastructure destruction and population displacement. There have been few opportunities for individuals to prosper from professional or entrepreneurial activities. Decades of neglected investment have resulted in deterioration of social services and economic infrastructure. Consequently, individuals have lacked capabilities to prosper and an investment climate conducive to prosperity. School enrollment and life expectancy have declined. Extremely low returns to education reflect the combination of poor educational quality and lack of employment opportunities. In terms of economic infrastructure, access to reliable electricity and water, and even access to paved roads are low, are further reflections of decades of neglect. While the upper end of the distribution has been pulled down by a lack of opportunities, the lower end has been supported by direct government provision of food. The Public Distribution System (PDS) provides 85 percent of food needs. While PDS has been useful as a safety net for the poor and the vulnerable, the system is expensive, inefficient, and fiscally risky. Indeed, PDS food rations account for a far greater share of public spending than does education or health. Going forward, Iraq faces two main challenges. First, although Iraq does not have to develop from scratch, it faces a formidable challenge in re-development. Second, a shift by the Government is required from direct provision of basic subsistence toward investment in human capacities. The Government can provide an enabling environment through investments in economic infrastructure and services to business and citizens, thus allowing the population to make productive use of education and their own labor. Both challenges are now being taken up by the Poverty Reduction Strategy, which articulates a detailed set of required actions and outlines priorities for government spending.
