Book excerpt: The Medicare Prescription Drug, Improvement, and Modernization Act of 2003 (the Medicare Modernization Act, or MMA) substantially expanded the federal Medicare program by creating the prescription drug benefit known as Part D. In FY 2013, Medicare Part D covered 39 million people. The federal government spent $59 billion net of premiums on Part D in that year; after accounting for certain payments from states under the program, the net federal cost was $50 billion, which represented 10% of net federal spending for Medicare. A combination of broader trends in the prescription drug market and lower-than-expected enrollment in Part D has contributed to much lower spending for the program than projected when the MMA became law in 2003. This report examines the federal budgetary cost and competitive design of Medicare Part D and compares Medicare Part D and Medicaid Fee for Service. Figures and tables. GAO analyzed (1) current challenges in identifying and enrolling new dual-eligible beneficiaries in PDPs, (2) the Centers for Medicare & Medicaid Services' (CMS) efforts to address challenges, and (3) federal and state approaches to assigning dual-eligible beneficiaries to PDPs. GAO reviewed federal law, CMS regulations and guidance and interviewed CMS and PDP officials, among others. GAO also made site visits to six states to learn about the enrollment of dual-eligible beneficiaries from the state perspective.

Vaccinations for flu and pneumonia are covered by Medicare's Part B physician and outpatient service benefit. Since 2006, Medicare's Part D prescription drug benefit covers other routinely recommended vaccinations, such as the shingles vaccination. Part D typically contracts with pharmacies, but not physicians, for covered vaccinations. The Patient Protection and Affordable Care Act required GAO to study Medicare beneficiary access to routinely recommended vaccinations covered under Part D. GAO examined (1) the extent to which beneficiaries have received Part D-covered vaccinations, (2) factors affecting beneficiary access to vaccinations, and (3) any findings and recommendations of government agencies and other stakeholders to increase access to Part D-covered vaccinations. To do this, GAO (1) analyzed Centers for Disease Control and Prevention (CDC) national health survey data and Medicare data; (2) surveyed physicians, pharmacies, and state beneficiary assistance programs nationwide; and (3) convened a facilitated discussion with stakeholder groups.
