Book excerpt: This book analyzes the development of medical big data projects in Japan.Japan is experiencing unprecedented population aging, and labor productivity has decreased accordingly. Big data analysis of the Japanese medical real-world database (RWD) has the potential to tackle this issue. To allow readers to gain an understanding of Japanese medical big data analysis, the book discusses the original Japanese system that generates medical RWDs in the hospital medical records system, the nationwide standardized health checkup system, and the public medical insurance system in Japan. After introducing four major big data projects in the healthcare–medical field in Japan, the book explains the importance of creating information standards to maintain data quality and to analyze medical big data. It enables readers to analyze which standards are installed in which RWDs, how the standards are maintained, and which issues are prevalent in Japan. This book also describes the ethical processes involved in big data projects involving medical RWDs in Japan.

Book excerpt: Recent data show wide disparity between Japan and the United States in the effectiveness of their health care systems. Japan spends close to the lowest percentage of its gross domestic product on health care among OECD countries, the United States spends the highest, yet life expectancies in Japan are among the world’s longest. Clearly, a great deal can be learned from a comprehensive comparative analysis of health care issues in these two countries. In Health Care Issues in the United States and Japan, contributors explore the structural characteristics of the health care systems in both nations, the economic incentives underlying the systems, and how they operate in practice. Japan’s system, they show, is characterized by generous insurance schemes, a lack of gatekeepers, and fee-for-service mechanisms. The United States’ structure, on the other hand, is distinguished by for-profit hospitals, privatized health insurance, and managed care. But despite its relative success, an aging population and a general shift from infectious diseases to more chronic maladies are forcing the Japanese to consider a model more closely resembling that of the United States. In an age when rising health care costs and aging populations are motivating reforms throughout the world, this timely study will prove invaluable.

Available in PDF, EPUB and Kindle. Book excerpt: Health spending has risen rapidly in Japan. We find two-thirds of the spending increase over 1990–2011 resulted from ageing, and the rest from excess cost growth. The spending level will rise further: ageing alone will raise it by 31⁄2 percentage points of GDP over 2010–30, and excess cost growth at the rate observed over 1990–2011 will lead to an additional increase of 2–3 percentage points of GDP. This will require a sizable increase in government transfers. Japan can introduce micro- and macro-reforms to contain health spending, and financing options should be designed to enhance equity.

Book excerpt: Research Paper (undergraduate) from the year 2009 in the subject Health Science, grade: 1,6, University of Applied Sciences Bremen, language: English, abstract: The development of the Japanese society is characterized by a lot of similarities to Germany. This is why I have chosen the country to compare especially the Health Care System with the German one. Not only the economic achievements are comparable, but first of all the Demographic Change. In 2020 28% of the Japanese population will be over 65 years. In Germany it is prognosticated to be around 21% (Tab.1). Moreover the Japanese Health Care System is known as own of the cheapest of the industrialized countries. This leads to the question of adoption of some parts or ideas from the Japanese system to the German one. In my elaboration I want to describe a case of a family in Japan regarding the family and work situation, the Health Care System in general and additionally the system of taking care of the elderly and the children. My example family has got following parts: The mother is 42 years old, she works as a nurse in an outpatient department of a local government. In addition she has to take care of her parents almost every day. The father is 45 years old, works as an engineer and likes his hobby, which is driving motorbikes. The daughter of them is 7 and their son is 13, both go to school. The grandmother has got dementia, she is 76 years old and lives together with her husband, who is 76 and has got diabetes. They live in the neighborhood of their children and grandchildren. Case 1: Values, culture, roles of the family members. Case 2: Health Care System of Japan in comparison to the German one. What happens after a traffic accident of the father? Case 3: Elderly and Child Care in general and in the case of the accident and depression of the mother.

Book excerpt: Ageing population poses a set of complex policy and dilemmas for social security systems, intensifying the concerns about rising expenditures in health care and long-term care for elderly. In this context, ageing societies has many valuable lessons to learn by studying Japan's experience dealing with its hyper-aged society and particularly from its strategies to ensure the financial sustainability of the Long-Term Care Insurance (LTCI) system. Based on an exhaustive literature review, and the results from six original researches on long-term care expenditures in Japan (LTCE) conducted during a doctoral program, the book provides a comprehensive view in analyzing trends and factors associated with increasing expenditures in the Long-Term Care Insurance system in Japan. The book address relevant topics such as; the main socio-demographic changes experienced by the Japanese society during the last three decades, predictors of the LTCE, measuring efficiency in nursing homes, the impact of the LTCI 2005-reform to contain expenditures, cost-effectiveness of the in-home and community based services and institutional LTCE in the last year of life. The book end with a discussion on futures challenges and strategies oriented to contribute with the sustainability of LTCI system in Japan.
