Book excerpt: The objective of this study is to use data from the National Survey of Children with Special Health Care Needs (NS-CSHCN) to test whether Medicaid physician fees are correlated with access to health services and adequacy of insurance coverage among CSHCN. We use a difference-in-differences method, comparing the effects of Medicaid physician fees on outcomes of publicly-insured children in states that raised fees vs. in states that did not. We also consider a triple difference specification using privately-insured children as the comparison group. Our findings indicate that raising the Medicaid primary care fee level close to at least 90 percent of the Medicare level reduces the likelihood that publicly-insured CSHCN lack a usual source of care in a doctor's office by about 15 percent. Fee increases are also associated with improved access to specialty doctor care, and large improvements in caregivers' satisfaction with the adequacy of health insurance coverage, among publicly-insured CSHCN. Results for some other access measures, such as global measures of having difficulties and delays accessing services, were mixed.

Book excerpt: The barriers most commonly identified as being exacerbated for CSHCN on Medicaid are the use of multiple medical providers, transportation, and more frequent visits than other children. Fifty-five percent of the physicians also indicated that it would not be acceptable to manipulate the Medicaid system to better serve CSHCN, 21.7% said it would be acceptable, and 20.8% said it would depend. Sixty-seven point nine percent of physicians indicated that they had done something to ensure the well being of CSHCN not covered or allowed by Medicaid. Of these, 51.5% had given free prescription samples to patients. These findings illustrate barriers in the direct provision of health care to CSHCN, such as missed appointments, as well as burdens stemming from Medicaid bureaucracy. The implications of these findings for Medicaid policy include addressing reimbursement, administrative burden, and prescription medication issues to ensure physicians' continued participation in the Medicaid system and the medical well being of CSHCN.

The existing literature suggests that the low level of Medicaid fee payments to physicians reduces their willingness to see Medicaid patients, thus creating an access-to-care problem for these patients. For the identical service, current Medicaid reimbursement rates are only about 65 percent of those covered by Medicare. Increasing the relative payments of Medicaid would increase its perceived value, as it would provide better access to health care for Medicaid beneficiaries. Using variation in the timing of the changes in Medicaid payment across states, I find that increasing Medicaid generosity is associated with both an increase in take-up and a reduction in uninsured rate. These results provide a partial answer to the puzzling question of why many low-income children who are eligible for Medicaid remain uninsured. Now that states are increasingly providing health care services to children enrolled in Medicaid through managed care systems, policymakers face the difficult task of building systems of care that meet the unique needs of CSHCN. The National Governors' Association convened a policy forum that included policymakers and program managers from five states where services for CSHCN are being provided or planned within a Medicaid managed care framework -- California, Florida, Massachusetts, Michigan, and Oregon. The forum participants discussed implementation issues and strategies for building a system of care that includes access to both primary care and specialty services. This Issue Brief summarizes these critical policy discussions. It is the last in a series of publications highlighting the needs of children as part of the Governors' Campaign for Children.
