The Balanced Budget Act (BBA) of 1997 continued to require Federal approval for state Medicaid programs that mandate that these children enroll. This report: (1) presents data on the extent to which states are enrolling children with special needs, as defined in the BBA, in capitated MMC plans, & (2) assesses the scope & effectiveness of the safeguards that these states are implementing to ensure that children with special needs receive appropriate care within MMC.

Children with special health care needs (CSHCN) often have diagnosed conditions that require specialized treatment services; these services frequently have been reimbursed by Medicaid under fee-for-service payment arrangements. Now that states are increasingly providing health care services to children enrolled in Medicaid through managed care systems, policymakers face the difficult task of building systems of care that meet the unique needs of CSHCN. The National Governors' Association convened a policy forum that included policymakers and program managers from five states where services for CSHCN are being provided or planned within a Medicaid managed care framework -- California, Florida, Massachusetts, Michigan, and Oregon. The forum participants discussed implementation issues and strategies for building a system of care that includes access to both primary care and specialty services. This Issue Brief summarizes these critical policy discussions. It is the last in a series of publications highlighting the needs of children as part of the Governors' Campaign for Children.
