The study on which this monograph is based explored the feasibility of this model from a civilian perspective, focusing on civilian receptiveness to the proposed arrangement and identifying potential barriers and concerns. The study found that civilian medical organizations are generally receptive to the idea of such a model and that DoD could consider conducting a pilot study to assess the effectiveness of the model in improving military medical readiness.

Book excerpt: The U.S. Army Medical Department has a dual mission: to care for the war wounded during times of conflict and to operate medical treatment facilities (MTFs) that provide care to service members, their beneficiaries, and military retirees. Because the injuries that require treatment during wartime can be very different from the case mix seen in MTFs, the Army asked RAND Arroyo Center to identify ways to help providers prepare for wartime missions while they are stationed at home. Using a variety of data sources, RAND Arroyo Center quantified how providers were assigned during wartime relative to their home duties, how the types of procedures seen in theater compared with those performed at home-station MTFs, and the rate at which providers attended mandatory predeployment trauma training (PDTT). In addition, the research team interviewed previously deployed providers to gather their perspectives on how they prepared-clinically and for trauma specifically-for the deployment mission, what their roles were in theater and how their patient mix in theater differed from the types of cases they treated in MTFs, and what additional training or other preparation would have helped them for the deployment mission.

Book excerpt: Advances in trauma care have accelerated over the past decade, spurred by the significant burden of injury from the wars in Afghanistan and Iraq. Between 2005 and 2013, the case fatality rate for United States service members injured in Afghanistan decreased by nearly 50 percent, despite an increase in the severity of injury among U.S. troops during the same period of time. But as the war in Afghanistan ends, knowledge and advances in trauma care developed by the Department of Defense (DoD) over the past decade from experiences in Afghanistan and Iraq may be lost. This would have implications for the quality of trauma care both within the DoD and in the civilian setting, where adoption of military advances in trauma care has become increasingly common and necessary to improve the response to multiple civilian casualty events. Intentional steps to codify and harvest the lessons learned within the military's trauma system are needed to ensure a ready military medical force for future combat and to prevent death from survivable injuries in both military and civilian systems. This will require partnership across military and civilian sectors and a sustained commitment from trauma system leaders at all levels to assure that the necessary knowledge and tools are not lost. A National Trauma Care System defines the components of a learning health system necessary to enable continued improvement in trauma care in both the civilian and the military sectors. This report provides recommendations to ensure that lessons learned over the past decade from the military's experiences in Afghanistan and Iraq are sustained and built upon for future combat operations and translated into the U.S. civilian system.

Previous missions have included care of U.S. personnel, training of host nation medical personnel, disaster medicine during peacetime contingency operations, and, most frequently, medical humanitarian/civic action. U.S. efforts in this area have suffered from the lack of a coherent strategy for using military medicine in LIC. This paper begins with an analysis of the pressures which lead to involvement of military medicine in LIC. The defined ends to which military medicine is directed in LIC are presented followed by the ways in which it contributes to those ends and the means by which operations are conducted. Guidelines for successful military medicine operations in LIC are also detailed. The author proposes a range of new organizations and personnel specialties to strengthen military medicine's contribution to LIC operations and doctrine.

Previous studies of the US military medical organization and the current structures of selected Allied military medical organizations were reviewed. As a result of the growth of interest in improving wartime and peacetime effectiveness and efficiency, several organizational improvements have been made to eliminate unnecessary triplication of effort in the US military medical system. Medical readiness has been improved in the areas of intelligence, research and development, and logistics. While progress in readiness has occurred in plans, operations and training, there is need for improvement. Peacetime health services have also experienced the emphasis on joint operations. Regionalization, has fallen short of original expectations and needs revitalization. The Joint Interservice Resource Study Group process offers additional opportunities to review medical functions and services. Quality assurance has benefited from the joint efforts of the Tri-Service Committee on Quality Assurance and the Interservice Training Review Organization. Common credentialing criteria, however, will require standardization of the professional training base which can only occur after joint functional reviews of specialty training. The major recommendation made by the study was that Department of Defense direct a system-wide analysis of medical functions, requirements, programs and resources to determine the best military medical organization for both wartime and peacetime.
