Book excerpt: Background: The mechanics of implant screw loosening or fracture are well understood in the field of engineering. Due to the several factors involved, they have not been as widely explored in dentistry. The current knowledge about the role of the implant external hexagon is incomplete. Two of the factors that seem to influence in this complication are the type of connection and the angulation of the implant.Aim: The aim of this preliminary in vitro study was to investigate the effect of abutment screw loosening of an external hexagon implant system when a cyclic loading was applied.Material and Methods: Twelve Mozo Grau Osseous u00ae (External Hexagonal Connection System) (3, 75 x 11, 5 mm) implants were divided into groups A and B. Group A included six 0u02da(4 mm) implant abutments and group B included six 15u02da(4 mm) implant abutments. Each implant assembly was tightened using an implant motor (IChiroPro Bien Airu00ae) to 30 Newton centimeters (Ncm) and placed on a custom made implant fixing jig. Cyclic loading was applied (200 N of force at a frequency of 2 Hz for 300, 000 cycles) (12 months of simulated function). Reverse torque was recorded before and after loading. After cyclic loading, the reverse torque value (RTV) data were analyzed using Wilcoxon signed-rank test (p

Book excerpt: Background: Screw loosening has been reported as one of the most frequent complications in implant supported single crowns. Nevertheless, the factors that influence in this mechanism have not been clearly established. Rigorous efforts to reduce the recurrence of abutment screw loosening in single-tooth implant restorations have been developed including changes in implant design, screws and abutments.Aim: The aim of this preliminary in vitro study was to analyze the screw loosening of two types of machined prefabricated abutments for internal morse taper connection implant system when a cyclic loading was applied.Material and Methods: 12 Mozo Grau Internal Implant Connection Systemu00ae (3, 75 x 11, 5 mm) implants were divided into groups A and B. Book excerpt: The aim of this study was to evaluate the microgap between the abutment and implant as well as the circularity of implant platforms and associating conformational errors with bacterial micro leakage in tapered connection implant systems. u2028Four brnads of implants with tapered abutment connection were tested. Bacterial leakage was assessed using 0.3 u00b5L of Escherichia coli suspension inoculated into the abutment screw chamber of the implants, which were then torqued and incubated at 37oC for 14 days. The same samples were than cut lengthwise, and the microgap measured at 3 points on each side using scanning electron microscopy (SEM). Microtomography was used to assess implant platform circularity to validate the SEM findings qualitatively. All implant Systems presented bacterial leakage, with no significant difference between groups. Neodent system presented the highest mean microgap values, followed by Nobel Biocare, Ankylos and Conexu00e3o, with no significant difference between groups. All systems showed conformational errors of circularity on microtomography images.u2028The tapered connection systems evaluated herein were not able to halt bacterial leakage, nor were free from conformational errors.

Book excerpt: The use of dental implants for treating edentulous patients with implant-supported fixed complete denture (ISFCD) has shown to improve the quality of life with great success over a long period of time. While the use titanium bar with acrylic denture teeth attached to the framework with denture acrylic is most common, zirconia is utilized due to its high strength, its color and translucency are close to those of teeth, and high biocompatibility that can reduce the risk of inflammation. However zirconia is 10 times harder than titanium based on the Knoop hardness scale and the long-term damage to the junction of titanium parts and zirconia abutment has not been widely investigated. The purpose of this study was to investigate the surface changes and the screw loosening after cyclic loading in vitro for Zirconia and Titanium and to compare the surface changes and the torque value before and after loading. Total of 24 multiple-unit titanium abutments were used. Twelve straight 3 mm height abutments were used for the two anterior implants. Twelve 30 degree angle correction abutments were used for the posterior two posterior implants. The abutments were placed on the testing model and torqued to 15 Ncm and scanned prior to placement under a digital light microscope and images were acquired for all the abutments to study the surface characteristic and to confirm the absence of any damage to the surface prior to placement. Masticatory forces were simulated using a testing machine (MTS- Machine Testing Systems). Each framework was subjected to a vertical 400 N of load for 500,000 cycles at a frequency of 2 HZ (2 Cycles/second). Abutments were scanned after the loading was complete with the SEM to detect any damages to the surface and none of the abutments showed any signs of damage or wear. The torque values to loosen the screws were analyzed by an analysis of variance (ANOVA) (MIXED procedure, SAS (r) Proprietary Software 9.3, SAS Institute, Inc., Cary, NC, USA) using the maximum likelihood estimation method to eliminate the need for normality and eq. The factors were the material type(Zr or Ti), the screw location (anterior or posterior) and the screw type (prosthetic or abutment screw), with all possible interactions included in the statistical model. Results showed statistical difference on the reverse torque value for the following variables that the material type (zirconia or titanium) (p= .007), the screw type (prosthetic vs. abutment) (p

Book excerpt: This comprehensive guide to short implants will take the reader through their research and development, explain the clinical indications, evaluate the outcomes achieved with various implants, and explore restorative and laboratory considerations. Short implants have steadily gained greater market share in the last decade as practitioners sought alternatives to traditional length implants in order to avoid grafting procedures. Current manufacturers offer a variety of implant lengths and widths, allowing surgeons and restorative dentists the ability to select the best implant for each clinical circumstance. Cutting edge information is provided on the research and clinical results achieved utilizing a range of implants, specifically those developed by Nobel Biocare, Straumann, Jack Hahn, and Bicon. Readers will also find an extensive description of the role of ultra-short implants involving reconstruction in both cleft patients and cancer patients who have lost portions of their mandible and/or maxilla. This book is a must-have for those interested in learning how the use of short and ultra-short implants offers both surgeons and restorative dentists an opportunity to stand out from those that use only the traditional length implants.

Book excerpt: Dental implant materials are advancing in the fusion of various scientific fields. Surface modification technologies for implants have been applied to titanium at the micro-level for about four decades. Now, implant surfaces are being topographically and chemically modified at both the micro- and nano-level. The modification techniques are altering other metals and ceramics, making these materials more biocompatible. Materials for abutments in dental implant systems appear to depend on implant–abutment connection structures. Biomechanical factors, such as friction and preload, influence the development of the abutment materials. Additionally, the surfaces of the abutment materials are important in the soft-tissue attachment, which is being actively investigated. As dental implants have to be functional in human bodies for a long time, numerous materials are being clinically tested as implant-supported restorations. The Special Issue, “Dental Implant Materials 2019”, introduces the creative works of scientists on the current advancements in the field of materials for implant dentistry.
