Twenty years ago, the immunological analysis of lymphoid proliferations was in its infancy. The techniques available, such as sheep red blood cell rosetting and immune adherence to frozen sections, now appear unbelievably crude when compared with our ability to accurately phenotype lymphocytes in suspension, in frozen section and, more recently, in formalin-fixed, paraffin-embedded tissue biopsies. Four international work shops have also standardized the nomenclature for the wide range of lineage restricted and lineage-related monoclonal antibodies available, and have provided a basis for the sophisticated phenotypic analysis of lymphoid neoplasms in even the smallest routine laboratories. Our concepts relating to the pathogenesis of a number of human lymphomas have also changed substantially, and this has been aided by the development of systems for the classification of human lymphoma which are firmly based in our knowledge of the differentiation and biological behaviour of normal lymphoreticular cells. In this monograph, we present contributions from many authors examining both leukaemia and lymphoma from an immunological perspec tive. It is our hope that some of these contributions will be of practical value in the laboratory investigation and diagnosis of lymphoreticular disease. Other contributions record our conceptual understanding of the histogenesis and pathogenesis of human lymphoma.

A Pattern Approach to Lymph Node Diagnosis demonstrates that a systematic approach to lymph node examination can be achieved through recognition of morphological patterns produced by different disease processes. It presents a combination of knowledge-based assessment and pattern recognition for diagnosis covering the major primary neoplastic and non neoplastic diseases and metastatic tumors in lymph nodes. This volume demonstrates that lymph node compartments can be recognized histologically especially with the aid of immunohistological markers and how this knowledge can be employed effectively to localize and identify pathological changes in the different compartments in order to facilitate histological diagnosis. It also defines histological features that, because of their pathological occurrence in lymph nodes, are useful pointers to specific diagnoses or disease processes. The volume is organized in accordance with the primary pattern of presentation of each diagnostic entity. Differential diagnosis is discussed and each diagnostic entity is accompanied by color illustrations that highlight the diagnostic features. Immunohistochemistry, clinical aspects, relevant cytogenetics and molecular information of each entity is provided by authors who are experts in lymphoproliferative diseases. An algorithmic approach to diagnosis is adopted at the end of each section by listing a set of questions that help to consider diagnostic entities that can present with the morphological features observed. The discovery of the virus two decades ago resulted from clinical and basic science collaborative studies on Burkitt lymphoma. Subse quently, nasopharyngeal carcinoma and infectious mononucleosis have been linked etiologically with the virus. During the first decade of research following the discovery of the virus, the mechanisms for the induction of BL, NPC, and IM were sought. At that time one prevailing view was that individual oncogenic strains of EBV were responsible for the different disorders. Paralleling the development of immunology in the 1970's was the accrual of knowledge about immunological events occurring during IM. These studies suggest that immune defense mechanisms deter mine the outcome of this viral infection rather than different viral strains. During the early 1970's, Starzl and Penn and Gatti and Good had noted an increased frequency of malignancy in renal allograft recipients and children with primary immune deficiency disorders, respectively. These observations provoked investigators to restudy the role of immune surveillance against malignancy. At that time immune surveillance was thought to occur against tumor-specific antigens; thereby neoplasms were eliminated.
