Book excerpt: En la actualidad hablar de terapéutica de la enfermedad vascular cerebral no es sorprendente; sin embargo, estos avances son muy recientes y, a pesar de ello, la actitud hacia el manejo y el pronóstico de la enfermedad vascular cerebral es pesimista en muchos médicos. En el decenio de 1950 se realizaban los trabajos pioneros en cirugía carótida, siendo hasta 1988 cuando el Dr. Henry Barnett, investigador principal del estudio NASCET, demostró en un impecable estudio la utilidad de la endarterectomía carotídea en pacientes con estenosis sintomática mayor de 70%. En 1968 unos investigadores del Massachusets General Hospital descubrieron el mecanismo de acción de la Aspirina® y pocos años después el mismo Dr. Barnett dirigió el Estudio Canadiense de Aspirina® y demostró la eficacia de este fármaco en la prevención de la enfermedad vascular cerebral isquémica. A partir del decenio de 1970 siguieron numerosos estudios que permitieron conocer la dosis y el espectro de acción de la Aspirina® en diferentes enfermedades vasculares. Así, en la década de 1990 aparecieron estudios que demuestran la eficacia de otros agentes antiplaquetarios, como clopidogrel y la combinación Aspirina®–dipiridamol, aumentando las alternativas terapéuticas. En 1995 el Dr. Thomas Brott, de la Universidad de Cincinnati, dirigió el estudio rTPA–NINDS y demostró la eficacia de la trombólisis intravenosa en pacientes con infarto cerebral de menos de tres horas de evolución con un aceptable rango de seguridad y eficacia. Otros estudios de trombólisis intravenosa con rTPA realizados por investigadores europeos permitieron definir con más detalle las características de los pacientes ideales para esta terapéutica. Hace poco el Dr. Werner Hacke presentó los resultados del estudio ECASS3, en el que demostró la eficacia de la trombólisis intravenosa en un tiempo hasta de 4.5 horas de evolución. Se han incluido colaboraciones del Dr. Abraham Majluf, hematólogo del Instituto Mexicano del Seguro Social, con notable experiencia en trombosis y particularmente en aterosclerosis y una de las mayores experiencias en el uso y las complicaciones de los agentes antitrombóticos, y del Dr. Edgar Nathal, neurocirujano del Instituto Nacional de Neurología y Neurocirugía con gran experiencia en cirugía vascular. But one question soon led to another and it just growed, like Topsy. In fact, it is hard to characterize precisely what this should be called. It is in part a Review of the Literature, in part a critique and reworking of other publications, and in part a standard view of stroke epidemio logy in the more restricted sense of attack and mortality rates and distribution. Still the result would I hope provide a synthesis of the population features of stroke as they appear to me at this time - a highly individual interpretation of the "state of the art". I have studiously avoided any survey of the history of cerebrovascular disease, and citations are for those of most recent vintage appropriate to the situation. Literature in this field continues to burgeon; my references end with the Fall of 1967. When counting noses we must have numbers, so the reader will find a massive compilation of tables. They are however necessary, especially since so many of my statements seem to fly in the face of current orthodoxy, whether lay or medical. With the data, one may decide for himself their validity. Insofar as possible tables have been placed in the appendix. Unless an author is directly quoted by me, all interpretations of his data are my own and he should be held blameless.
