L'évaluation de la qualité des soins tient elle aussi de plus en plus compte de l'avis du patient. Les disciplines médicales répondant à une demande esthétique sont d'autant plus confrontées à ces évolutions. L'orthodontie en particulier fait face à un véritable changement de paradigme : l'approche traditionnelle, paternaliste, a longtemps consisté en l'établissement de normes dont découlent à la fois un diagnostic, des objectifs de traitement et ultimement une stratégie thérapeutique. Le patient n'a alors d'autre choix que d'adhérer à la proposition du praticien ou de renoncer au traitement. L'avènement de l'orthodontie fondée sur la preuve, le souci de l'esthétique et son retentissement psychosocial, le développement législatif des codes de santé placent aujourd'hui le patient au cœur d'un processus interactif qui mènera ultimement à une décision thérapeutique partagée, résultant de l'implication du praticien autant que de celle du patient ; on parle d'« orthodontie centrée sur le patient ». L'objectif de ce travail est de montrer les différents niveaux de prise en compte de l'avis du patient lorsqu'un traitement orthodontique est envisagé. La première partie s'intéresse à la justification du traitement orthodontique : qualité de vie, bénéfices potentiels des traitements orthodontiques et attentes des patients. La deuxième partie traite des objectifs du traitement orthodontique en soulignant l'évolution de leur établissement, notamment la place grandissante des études de perception esthétique. La troisième partie se concentre sur l'étape de la décision thérapeutique : choix entre différentes modalités selon leur perception par les patients et cadre législatif actuel.
