Information ranges from explanations of how specific drugs work, through the precautions to take in the presence of certain disorders, to a list of factors that influence the effectiveness of opioids. Concerning the use of opioids, readers are reminded that psychological dependence does not occur in cancer patients and that the only correct dose of morphine is the one that relieves the pain. Part two provides a guide to opioid availability. A discussion of the reasons why opioids continue to be underprescribed or difficult to obtain is followed by an explanation of the Single Convention on Narcotic Drugs.

In just over a decade the public and official response to the disease has resulted in the development of a whole network of organizations devoted to the study, containment, and practical treatment of AIDS. In this important and original analysis of AIDS policy, Virginia Berridge examines the speed and nature of the official (and unofficial) response to this new and critical historical event. The policy reaction in Britain passed through three stages. From 1981-1986 the outbreak of a new contagious disease led to public alarm and social stigmatization, with a lack of scientific certainty about the nature of the disorder. AIDS was a new and open policy area - there were no established departmental, local, or health authority mechanisms for dealing with the problem. This was a period of policy development from below, with relatively little official action and many voluntary initiatives behind the scenes. This phase was succeeded in 1986-1987 by a brief stage of quasi-wartime emergency, in which national politicians and senior civil servants intervened, and a high-level political response emerged. The response was a liberal one of `safe sex' and harm minimization rather than draconian notification or isolation of carriers. The author demonstrates that despite the `Thatcher revolution'in government in the 1980s, crisis could still stimulate a consensual response. The current period of `normalization' of the disease sees panic levels subsiding as the rate of growth slows and the fear of the unknown recedes. Official institutions have been established and formal procedures adopted and reviewed; paid professionals have replaced the earlier volunteers. The 1990s have seen change in the liberal consensus towards a harsher response and the partial repoliticization of AIDS. In this fascinating and scholarly account, Virginia Berridge analyses a remarkable period in contemporary British history, and exposes the reaction of the British political and medical elites, and of the British public to one of the most challenging issues of this century.

Book excerpt: Contexte Les cancers du sein et du col utérin sont des problèmes de santé publique en France pour lesquels le dépistage est recommandé. Ils ont été intégrés aux indicateurs évalués lors de la mise en place en France de la Rémunération sur Objectifs de Santé Publique (ROSP) au 1er janvier 2012. En effet, comme dans de nombreux pays depuis environ 10 ans, la France a diversifié la rémunération de ses médecins en intégrant une part de paiement à la performance. Méthode : L'étude s'est intéressée à l'influence de la ROSP sur les taux de dépistages des cancers du sein et du col utérin au sein de la patientèle des Maîtres de Stage Universitaires (MSU). Il s'agit d'une étude descriptive rétrospective par un questionnaire à choix multiples réalisé via le logiciel LimeSurvey® et proposé aux MSU par mail. Ce questionnaire couvrait les données sociodémographiques des médecins ainsi que leurs taux de dépistages des cancers gynécologiques au 1er janvier 2012 et au moment du questionnaire (entre le 18/11/2013 et le 13/02/2014). L'analyse des résultats a été réalisée avec le service d'épidémiologie et de santé publique du CHRU de Lille. Résultats : 58 médecins MSU ont répondu au questionnaire (sur 135 sollicités). 75,8% des répondants avaient adhéré à la ROSP. Les résultats ne mettent pas en évidence d'évolution significative des taux de réalisation des Frottis Cervico-Utérins (FCU) (1,451 % : IC95 % [-0,396 ; 3,299]) et des mammographies (0,474 % : IC95 % [-1,572 ; 2,519]) sur la période étudiée. Ils ne retrouvent pas non plus de corrélation entre l'évolution des taux de mammographies et de FCU et les facteurs sociodémographiques suivants : durée d'installation, milieu d'activité, âge et sexe du médecin. Enfin, il n'est apparu aucune différence significative des taux de mammographies et des FCU en fonction du milieu d'activité ou du sexe du médecin. En revanche, une corrélation significative est retrouvée entre le jeune âge (et l'installation récente) des médecins et de plus hauts taux de mammographie et de FCU réalisés en 2013. Conclusion : L'absence d'évolution des taux de mammographie et de FCU est concordante avec les données nationales disponibles. Cependant, la corrélation entre l'âge jeune des médecins et des taux de dépistages important n'a pas été retrouvée dans la littérature. Au total, la ROSP n'a pas été suivie de modification des taux de dépistages au sein de la population des médecins généralistes MSU du Nord Pas Calais.
