Ils sont responsables de stress, de mal être au travail voir de burn out ou de reconversion. L'hypnose grâce à ses différentes techniques de communication apparaît comme une solution pour aider à la gestion de cette violence. Nous avons réalisé une étude qualitative dans le but d'étudier l'intérêt de l'hypnose dans la gestion des faits de violence au sein des services d'accueil des urgences. Douze entretiens semi dirigés ont été réalisés avec des soignants travaillant aux urgences (médecin, infirmier et aide-soignant) et ayant suivi une formation à l'hypnose. Cette étude a permis de mettre en évidence l'utilisation fréquente voire systématique de l'hypnose dans les situations de violence. Elle est considérée par les soignants comme un véritable outil face aux violences verbales mais aussi physiques grâce notamment aux techniques de communication, de respiration et de relaxation. Il a été montré que l'hypnose apporte également un bénéfice personnel notamment dans la gestion du vécu de la violence et dans l'aide à la prise de recul. L'hypnose médicale est un outil utilisé par les soignants face aux situations de violence au sein des services d'accueil des urgences. Elle permet de désamorcer les situations violentes, de mieux les appréhender et de mieux les vivre. Elle permet également une nouvelle approche du patient et de la relation soignant-soigné. L'ensemble de ces données est en faveur du développement la pratique de l'hypnose au sein des services d'urgence et de la formation des personnels soignants.

Comment les médecins urgentistes s'approprient-ils cette pratique ? Quels sont ses apports, et les freins à son utilisation aux urgences ? Étude qualitative par entretiens semi-dirigés réalisés auprès de médecins urgentistes formés à l'hypnose médicale et exerçant dans des services d'urgences de la région Rhône-Alpes. Parmi les quinze médecins rencontrés entre juillet et août 2016, certains pratiquent régulièrement l'hypnose, mais la plupart ne l'utilise que rarement. Les indications concernent la prise en charge de la douleur aiguë et de l'anxiété. Les apports soulignés portent sur l'efficacité de l'hypnose comme outil complémentaire aux soins classiques, la diminution du temps de prise en charge grâce à l'épargne des morphiniques et sédatifs intraveineux, et l'amélioration de la relation médecin-patient. Les limites tiennent d'une part à l'environnement des urgences (sollicitations, interruptions, pression des patients en attente, regard du reste de l'équipe), et d'autre part au manque de confiance en soi, en la méthode, à la sensation de manque d'expérience. Certains de ces freins ressentis peuvent en réalité être transformés en atouts. La mise en garde contre ces fausses croyances lors de la formation et l'initiation à l'hypnose pour l'ensemble des équipes semblent être des axes d'amélioration pour l'utilisation de cette pratique. Dans notre étude, l'intérêt de l'hypnose aux urgences est reconnu, mais de nombreuses difficultés font obstacle à son application pratique. La diffusion de la formation à l'hypnose, la prise de conscience des freins potentiels, et la multiplication des études cliniques dans ce domaine permettront un meilleur emploi de cette pratique
