Ce type de suivi suppose des compétences professionnelles et engage des responsabilités, morale d'abord, ainsi que celles relevant d'une observation sans faille de la loi, du code de déontologie et des règlements, comme ceux de la Sécurité Sociale que l'on pourra trouver dans cette étude en annexes. Comment se déroule le suivi à domicile ? Des objectifs à la naissance de l'enfant, en passant par l'organisation de la première visite et des suivantes dans leurs différents volets, sans oublier la circulation de l'information ascendante, remontant au médecin obstétricien, ni celle descendante, que la sage-femme donne à la patiente dans un but double d'information et de prévention, les maîtres mots sont, entre autres : sécurité, sécurisation, prévention, information, travail en équipe et, au delà du contrat, économie publique. Les enquêtes menées auprès des patientes, des sages-femmes ainsi que des médecins montrent clairement la satisfaction commune, pour des motifs différents, à l'endroit du suivi à domicile des grossesses à risques. L'aspect de la courbe illustrant la population des sages-femmes libérales en France pourrait inquiéter. En effet, durant les dix années 1977-1987, les effectifs sont passés de 2500 à 1250 (chiffres arrondis). La dernière décennie voit une stabilisation se dessiner autour de 1250. Y a-til désaffection par manque d'information, de courage ç s'engager de la part de jeunes diplômés ? L'effectif des sages-femmes libérales est appelé, dans la conjoncture des années de fin de siècle, à une certaine renaissance dont l'auteur ne doute pas puisque 1695 sages-femmes libérales sont en activité en France au 1er janvier 1996 - chiffre à rapprocher des 1250 de l'année 1987. La dernière partie de l'étude montre les intérêts, dans des domaines variés, du suivi à domicile. Le travail de l'équipe, dans laquelle les obstétriciens référents pratiquant en maternité et les sages-femmes libérales composent un microcosme oeuvrant dans le sens d'une sécurité accrue de la santé de la mère et de l'enfant, loin d'occasionner des frais supplémentaires à la société, par les hospitalisations coûteuses qu'elle permet d'éviter lorsqu'elles ne sont pas impératives, contribue à réaliser à moindres frais un service judicieusement organisé.

Book excerpt: Introduction : l'accouchement à domicile avec une sage-femme est une pratique en cours de développement dans certains pays européens comme le Royaume Uni et la Hollande. Elle est peu développée et peu étudiée en France. L'objectif principal de notre étude était de décrire les caractéristiques sociodémographiques, les conditions de suivi et d'accouchement des cas d'accouchements programmés à domicile avec une sage-femme en Midi-Pyrénées pendant la période périnatale. Méthode : un questionnaire a été rempli entre 7 jours et 4 mois après l'accouchement pour chaque femme qui avait fait le projet d'accoucher avec une sage-femme à domicile en Midi-Pyrénées et dont le terme était prévu de mai 2018 à mai 2019. Les données ont fait l'objet d'une analyse descriptive. Résultats : caractéristiques sociodémographiques : 201 femmes ont été incluses. Il y avait 87 (43 %) nullipares et 114 (57 %) multipares. 131 (65 %) avaient un haut niveau d'étude, 177 (88 %) étaient d'origine française. Suivi de grossesse : en début de suivi, 146 (73 %) avaient un suivi de type A soit à bas risque obstétrical selon la définition de la Haute Autorité de Santé, 46 (23 %) un type A1( dans la majorité des cas en raison d'un âge > à 35 ans), 9 (4 %) un type A2 et 0 (0 %) un type B. Accouchement : 159 femmes ont débuté leur accouchement à domicile. Parmi les 42 ayant changé de projet avant l'accouchement 32 (76 %) ont dû le faire pour raison médicale. Transfert hospitalier pendant et jusqu'au 7ème jour après l'accouchement : sur les 159 femmes, 121 (76 %) n'ont pas été transférées, 28 (18 %) ont été transférées pendant l'accouchement et 10 (6 %) après. Sur les 159 femmes, 2 (1 %) nouveau-nés et 8 (5 %) femmes ont eu une issue défavorable (4 césariennes, 4 transfusions sanguines, 1 mort foetale per partum d'origine infectieuse, 1 mort néonatale d'origine inconnue). Conclusion : dans un contexte d'accouchement à domicile marginal en France, l'accompagnement du projet d'accoucher à domicile en Midi-Pyrénées a été bien encadré par les sages-femmes. Les femmes à risque de complication ont été détectées tout au long du suivi et mise en relation avec un service hospitalier. Les accouchements à domicile se sont bien déroulés dans la majorité des cas. En cas de transfert hospitalier, il y a eu dans la majorité des cas une issue favorable pour la femme et le nouveau-né.
