Cette privation est source de souffrance chez les patients fragiles psychologiquement. Cette thèse a pour but d'évaluer s'il est nécessaire de se priver de chocolat pour rester en bonne santé, à l'aide d'une revue systématique de la littérature concernant les effets du chocolat sur le système cardiovasculaire. Méthode : Une recherche a été effectuée dans les bases de données, SUDOC, PubMed, Cochrane, et Google Scholar, jusqu'en janvier 2019, pour mettre en évidence les études observationnelles, et les essais cliniques évaluant les effets du chocolat sur le système cardiovasculaire. Les articles non pertinents ou incomplets ont été exclus selon un protocole pré établi. Les articles sélectionnés ont été évalués, puis résumés, et critiqués . Résultats : au total 1502 articles ont été référencés. 1484 articles ont été exclus. Les 18 études sélectionnées rapportaient toutes un lien bénéfique entre consommation de cacao, et santé cardiovasculaire. Trois études mettent en évidence une diminution de la rigidité artérielle, et une autre, trouve une diminution de la prévalence de la calcification des artères coronaires. Trois études mettent en évidence une diminution de la pression artérielle. Trois études mettent en évidence une amélioration du bilan lipidique. Une étude met en évidence une diminution de l'indice de Masse Corporelle (IMC). Une étude met en évidence une diminution de la résistance à l'insuline, et du taux d'insuline plasmatique. Les effets sur ces différents facteurs de risques cardiovasculaires sont confirmés par une méta analyse globale. Ces résultats sont en partie expliqués par une augmentation d'oxyde nitrique NO lors de la consommation de chocolat. Trois études mettent en évidence une diminution de la survenue d'évènements cardiovasculaires, et une étude trouve une diminution des cas d'insuffisance cardiaque. Discussion : aucune étude ne présente de niveau élevé de preuve, cependant elles sont toutes concordantes entre elles, et montrent toutes des effets positifs sur le système cardiovasculaire. Il est difficile d'établir un lien de causalité indiscutable, mais il semble que la consommation modérée de chocolat soit bénéfique pour la santé cardiovasculaire, et qu'elle pourrait être un facteur protecteur des accidents vasculaires. La consommation optimale semble correspondre à une plaquette de chocolat noir par semaine.
