Book excerpt: Les objectifs étaient de décrire le profil clinique et épidémiologique des patients de plus de 75 ayant eu recours aux urgences pour altération de l’état général (AEG) afin d’observer si l’hypothèse d’un lien entre fragilité et AEG était pertinente puis de mesurer l’impact du motif de recours « AEG » sur l’accès à l’évaluation gériatrique recommandée pour tous les usagers âgés « fragiles ». Notre étude transversale sur 4 mois au SAU de l’hôpital Saint-Antoine à Paris a comparé rétrospectivement 2 échantillons de patients de plus de 75 ans : 179 malades inscrits au SAU pour le motif AEG et 180 témoins ayant consulté pour d’autres motifs. Les deux groupes présentaient les mêmes caractéristiques sociodémographiques et leur typologie était comparable à celle retrouvée dans la littérature pour les usagers des urgences de plus de 75 ans. Nous avons cependant retrouvé un lien entre le motif de consultation AEG et la fragilité ; les patients AEG présentaient bien plus fréquemment des marqueurs fortement corrélés à la fragilité : troubles cognitifs (p=0,05), troubles de la marche (p

Ils constituent même la principale voie d'accès à une hospitalisation pour les personnes âgées. Ces dernières constituent une population fragile, à la fois sur le plan médical mais également social. Notre travail s'intéressait aux recours aux urgences des personnes âgées de 75 ans et plus pour un maintien à domicile difficile. Méthodes : il s'agissait d'une étude épidémiologique, observationnelle, transversale et monocentrique, avec un recueil rétro-prospectif des données, réalisée aux urgences du centre hospitalier de Pontarlier durant l'été 2023. Un hétéro-questionnaire était à remplir dans ce service par le personnel médical pour tous les patients de 75 ans et plus consultant aux urgences. Résultats : cent trois questionnaires ont été analysés, permettant de mettre en évidence que les consultations aux urgences des personnes âgées de 75 ans et plus, pour un motif social, ne sont pas majoritaires. Notre étude a permis d'identifier ces patients présentant un maintien à domicile difficile : cela concerne essentiellement des femmes âgées de 90 ans et plus, isolées socialement. Conclusion : ce travail n'a pas prouvé la prépondérance des consultations aux urgences pour un motif social des personnes âgées, comparativement aux causes médicales. Il est important d'identifier, en médecine ambulatoire, les patients avec des fragilités, afin d'éviter les recours aux urgences pour un maintien à domicile difficile.

En 2003, la conférence de consensus de la SFMU rappelle leur prise en charge. La fragilité doit être dépistée par le score ISAR. L'objectif principal est la réalisation d'un état des lieux des personnes âgées consultant aux urgences du centre hospitalier de Maubeuge. Méthode : étude prospective, observationnelle, monocentrique, du 1er au 31 mars 2017. Tous les patients de 75 ans et plus étaient inclus. Les données étaient recueillies par analyse du questionnaire rempli par le médecin de garde et étude du dossier médical informatisé. Résultats : 122 patients étaient inclus. La moyenne d'âge était de 88,5 IC 95% [84,5- 86,5] et 77% étaient des femmes. La plupart vivaient à domicile (87%). Le score ISAR était supérieur ou égal à deux chez 64,8% avec une moyenne de 3,05 IC 95% [2,76-3,35]. 11 avis de l'équipe mobile de gériatrie (EMG) ont été donnés, dont 9 pour un score ISAR supérieur ou égal à 2. Le score ISAR n'influençait pas la durée de prise en charge (p = 0,53), tout comme l'âge (p = 0,15). La durée moyenne de prise en charge était de 4h43 IC 95% [4h18-5h08]. Le devenir était significativement différent selon l'évaluation par l'EMG (p = 0,01). Les deux diagnostics principaux retrouvés étaient les lésions traumatiques et le maintien à domicile impossible. Conclusion : les personnes âgées nécessitent un temps dédié de consultation aux urgences. L'intervention de l'EMG est un soutien pour une évaluation globale pluridisciplinaire.

De nouveaux enjeux se dessinent pour les professionnels de santé avec pour objectif, le maintien de la plus grande autonomie possible chez les personnes âgées. Nous avons conduit une étude pilote au sein du CHU de Caen, randomisée, prospective, au premier trimestre 2012 pour évaluer l’impact de la mise en place d’une consultation avancée de gériatrie pour les patients de plus de 75 ans consultant aux urgences et présentant un score ISAR (score d’évaluation de la fragilité) supérieur ou égal à deux. Trois objectifs étaient recherchés : étudier les seconds recours aux urgences à un mois, rechercher des indicateurs de fragilité dans la population cible, évaluer les bénéfices en médecine générale. Au terme de notre travail, il apparait que la consultation avancée de gériatrie semble diminuer significativement le taux de second recours à trente jours (20,4% vs 6,3% ; p=0,02). Par ailleurs, les patients consultant pour des pathologies ostéoarticulaires présentent le plus fort taux de retour (27,8%). La consultation avancée de gériatrie constitue en outre un outil intéressant pour les médecins généralistes afin d’optimiser leur prise en charge de ces patients. Il ressort également que Le délai entre la consultation aux urgences et l’évaluation gériatrique devrait être modulé en fonction du motif de recours initial. La pérennisation et l’élargissement de ce dispositif pourrait améliorer la gestion des patients âgés. Une étude à plus grande échelle serait alors nécessaire pour confirmer ces perspectives et en évaluer l’impact économique et sanitaire.
