Book excerpt: Depuis 1998, la prise en charge de la douleur est une priorité nationale et a permis le développement de Structures d'Etude et de Traitement de la Douleur Chronique. On étudiera les raisons qui incitent le médecin généraliste à faire appel à ces structures et le devenir des patients adressés. Nous avons effectué une étude multicentrique, rétrospective sur 9 mois, aux Centres Hospitaliers d'Avranches-Granville, Bayeux et Saint-Lô. 281 patients adressés pour la première fois aux consultations de lutte contre la douleur par un médecin généraliste, furent inclus dans l'étude. Les informations personnelles, la description de la douleur et le devenir du patient ont été analysés. 53,8% des patients sur les 522 adressés l'étaient par un médecin généraliste. Mais seul 25,3% des médecins alentours ont adressés des patients. Les médecins généralistes adressaient plus de femmes, entre 41 et 64 ans. Le délai moyen d'évolution de la douleur était de 6-7 ans et le délai d'attente moyen avant la consultation de 3 mois et 1 semaine. Les patients étaient pris en charge par la structure dans 95,7% des cas, justifiant les demandes de consultation. La TENS était proposé pour 65,1% des patients, une prise en charge physique pour 44,5% des patients et psychologique pour 33,8%. Les demandes d'avis par les généralistes aux consultations de lutte contre la douleur respectent les recommandations de l'HAS 2008. Seul un petit nombre de médecins travaillent avec la consultation. La formation et la communication entre intervenants de la consultation et les médecins traitants pourraient permettre de faciliter l'accès à une prise en charge globale et à des techniques spécifiques.

Ce travail a permis également de recueillir des données socio démographiques, de consommations de soins (consultations, médicaments, kinésithérapie), et concernant le statut professionnel et de protection sociale en cours. La fréquence du recours au médecin généraliste permet de rappeler leur place prépondérante dans la prise en charge des patients douloureux chroniques. Pour une plus grande puissance statistique, des recherches ultérieures seront à réaliser avec une méthodologie différente afin d'augmenter la taille de l'échantillon de patients.

Les objectifs secondaires étaient de faire un état des lieux de l'utilisation des MCA en pratique, et d'identifier d'éventuels freins et leviers à leur utilisation. METHODE : Étude qualitative avec analyse thématique et triangulation des données, à partir de onze entretiens semi-dirigés de médecins généralistes, réalisés en Haute-Loire, de juillet 2020 à novembre 2020. RESULTATS : Les onze médecins de l'étude reconnaissent que la prise en charge de la douleur chronique les met en échec. Ils ont une perception globale positive des MCA, qu'ils considèrent comme un outil complémentaire dans la prise en charge de la douleur chronique. Ils s'estiment ouverts et tolérants face à ces pratiques. Leur connaissance théorique à propos des MCA est partielle voire nulle. Il s'agit d'une connaissance empirique basée sur l'expérience des patients. Les MCA les plus fréquemment recommandées par les médecins sont, par ordre de fréquence : l'ostéopathie, le yoga, la psychothérapie, l'acupuncture, le coupeur de feu et la mésothérapie. Les principaux bénéfices attendus à l'utilisation des MCA sont : un temps d'écoute plus long, une approche globale, une aide à l'acceptation et à la compréhension de la douleur. Les principaux freins rencontrés sont : le risque d'abus de faiblesse, l'absence de clarté dans la législation, le manque d'explication scientifique, le manque de reconnaissances officielles et le manque de formation. CONCLUSION : Les MCA ont une place importante dans la prise en charge de la douleur. Elles doivent être utilisées de manière complémentaire avec la médecine conventionnelle. Pour favoriser cela, l'agence gouvernementale A-MCA a été créé en 2020, afin d'intégrer les MCA et les encadrer. Depuis 2021, il existe un enseignement sur les MCA, au cours de la formation universitaire du 2ème cycle des études médicales.

Le changement mis en évidence porte principalement sur la représentation et le vécu de la douleur par le patient, ainsi que sa posture par rapport à celle-ci. La validation de la souffrance apparaît comme un point de départ nécessaire au changement. La compréhension des mécanismes complexes et multifactoriels de la douleur chronique permet un changement de représentation du patient sur sa douleur. Quittant la recherche effrénée d'une cause, il peut redéfinir et orienter ses attentes initiales vers des objectifs concrets centrés sur ses ressources avec l'aide de l'équipe pluridisciplinaire. Devenir acteur dans la gestion de ses douleurs lui permet de retrouver du pouvoir et de l'autonomisation. Il coconstruit les solutions dans une relation de partenariat avec le médecin et l'équipe. Le patient a besoin de temps pour s'adapter, repenser sa vie, sa place dans la société et réinventer une vie avec la douleur qui ait du sens. C'est donc bien un chemin qu'est amené à emprunter le patient qui souffre de douleur chronique pour, pas à pas, changer ses représentations de la douleur, de son image, ses comportements et réinvestir sa vie en la réinventant. Ce chemin peut être initié et prendre appui sur l'accompagnement vers un changement proposé en consultation douleur, en partenariat avec le médecin traitant et l'équipe de soins primaire.
