Book excerpt: L'accident vasculaire cérébral (AVC) est l'une des causes principales de mortalité et de morbidité en France. L'hyperglycémie à la phase aiguë de l'AVC aggraverait son pronostic. Objectif : Déterminer s'il existe une relation entre la glycémie lors de la survenue d'un AVC ischémique constitué et l'état des patients une semaine et un mois après. Méthodes : Nous avons mesuré la valeur de la glycémie veineuse chez 121 patients consécutifs admis dans le service de neurologie de l'hôpital Bretonneau, à Tours, dans les 24 heures après la survenue d'un AVC ischémique constitué. Nous avons pris en compte les autres facteurs susceptibles d'intervenir sur l'état des patients : l'âge, le sexe, le délai d'hospitalisation, le dosage de l'hémoglobine glyquée et les handicaps préexistants. L'état des patients a été évalué à l'aide du score de Rankin modifié. Résultats : Notre étude met en évidence une association significative entre la glycémie à l'arrivée aux Urgences et les conséquences de l'AIC à une semaine. Les conséquences à un mois ne sont pas significativement associées à cette glycémie, probablement en raison d'un effectif trop faible. Ces résultats sont obtenus après ajustement sur les autres facteurs de risque, l'état étant classé en deux catégories. L'effet de la glycémie est le même chez les diabétiques et les non diabétiques. Nos résultats semblent indiquer que le glucose serait toxique pour le cerveau ischémié. Notre étude ne permet pas de conclure quant à l'origine de l'hyperglycémie. Aujourd'hui, les patients hyperglycémiques arrivant aux urgences avec un AIC ne sont pas traités pour rétablir la glycémie. Deux essais cliniques sont en cours. Ils permettront de mieux comprendre le rôle de la glycémie dans le pronostic des AVC et aboutiront peut-être à de nouveaux traitements médicamenteux lors de la phase aiguë des AVC.

Ils ont tous eu à l'entrée une glycémie suivie d'au moins quatre dosages pour la première année et cinq dosages pour la deuxième année avant la fin de la première semaine et ont tous bénéficié d'une scanographie cérébrale dans les quinze jours. Chez les malades non diabétiques, présentant une hyperglycémie supérieure à 2 g/l, la mortalité a été significativement plus élevée que chez les malades ayant une glycémie inférieure à 2 g/l. Cela ne fut pas retrouvé valable pour les malades diabétiques présentant une hyperglycémie supérieure à 1g/l. L'hyperglycémie a été retrouvée corrélée, chaque année, chez le patient diabétique comme chez le patient non diabétique, avec les troubles de conscience initiaux, l'existence sur la scanographie d'une hémorragie cérébrale ou d'un ramollissement oedémateux. L'hyperglycémie en phase initiale d'un accident vasculaire cérébral est donc liée à l'intensité de la lésion cérébrale, présente une valeur diagnostique quant au mécanisme, et représente un critère de mauvais pronostic à un mois sur le plan vital, surtout pour le non diabétique, aussi sensible que les troubles de conscience, la présence de vomissements, l'existence d'une hémiplégie totale et proportionnelle et que la présence d'un effet de masse à la scanographie.

Méthodes : les patients traités dans les 6 heures par thrombectomie mécanique pour une occlusion symptomatique (NIHSS≥10) de l'artère cérébrale moyenne ont été inclus (registre AVC PACA). L'association entre la glycémie d'admission et l'indépendance fonctionnelle (mRS≤2) a été évaluée par analyse multivariée. Une analyse en sous-groupe a été menée en fonction de la pénombre définie par Clinical ASPECTS Mistmach version 2 (CAM2) et de la recanalisation artérielle (RA) définie par un mTICI 2b/3. Discussion : sur les 216 patients inclus, 104 présentaient un mRS≤2 à 3 mois. Après ajustement aux variables associées au pronostic fonctionnel, la glycémie d'admission était indépendamment associée à un mRS≤2(par 100 mg/dl,OR: 0,10[0.03-0.37]). En sous-groupe, la glycémie d'admission était délétère sur l'autonomie dans les groupes CAM2+(OR:0,14[0,04-0,49]) et RA+(OR:0,21[0,06-0,69]). Le groupe combiné CAM2+/RA+ présentait une association significative entre la glycémie initiale et un mRS≤2(OR:0,20[0,05-0,73]), spécifiquement pour les 50/135 patients dont la recanalisation fût précoce (≤270 min). Conclusion : cette étude montre pour la première fois que l'effet délétère de l'hyperglycémie sur le pronostic fonctionnel d'un infarctus cérébral traité par thrombectomie mécanique n'est pas homo-gène à tous et dépend du sauvetage de la pénombre après recanalisation.
