Près de 4 à 10 % des personnes de plus de 65 ans seraient maltraitées de par le monde. Parmi les signalements de maltraitance dont sont victimes ces personnes vulnérables, seulement 2 % sont le fait des médecins généralistes. Notre objectif est d’évaluer les performances d’un test de dépistage de la maltraitance de la personne âgée et sa faisabilité en cabinet de médecine générale libérale. Notre étude a été scindée en trois parties. D’abord, nous avons fait une recherche bibliographique afin de choisir un test de dépistage de la maltraitance du sujet âgé et sa traduction valide en langue française. Ensuite, nous avons réalisé une étude quantitative prospective afin d’évaluer les performances du test choisi. Les réponses de patients de plus de 65 ans du CHU de Nice ont été analysées et comparées, en aveugle, aux réponses des assistantes sociales (gold standart). Enfin, nous avons effectué une étude prospective quantitative sur la faisabilité du test choisi. Pour ce faire, nous avons adressé un questionnaire aux médecins généralistes de la région des Alpes-Maritimes. Le test « Vulnerability to Abuse Screening Scale » (VASS) a été sélectionné et traduit en français. 200 patients ont été inclus entre mars et mai 2012 au CHU de Cimiez, à Nice. Nous avons retrouvé 104 patients à risque de maltraitance pour 40 cas relevés par les assistantes sociales. Ainsi, 40 % de cas de maltraitance ont été signalés par le gold standard, ce qui donne au test une sensibilité de 90,9%, une spécificité de 49,7 % et une valeur prédictive de 96,1 % pour un score seuil de 1. Dans le même temps, nous avons demandé à 31 médecins généralistes d’évaluer le test. Ces derniers estiment le test utile au cabinet mais de façon non homogène. Le test de dépistage VASS s’est montré utile pour repérer les patients âgés à risque, mais peu discriminant et inadapté pour les patients présentant des troubles cognitifs. Il semble cependant apporter une aide aux médecins généralistes dont les résultats laissent transparaître la nécessité d’une véritable formation à la maltraitance de la personne âgée, dû fait de sa complexité.

La prise en charge de la dépression chez le sujet âgé en médecine générale présente de nombreuses difficultés, tant au niveau du repérage et du diagnostic, qu'au niveau thérapeutique. Elle présente des formes cliniques polymorphes et souvent masquées qui nécessitent des temps de consultation longs. Le médecin généraliste fait souvent le diagnostic à l'aide de son empathie et n'utilise pas d'outils spécifiques. Les symptômes sont également banalisés par la personne âgée qui est souvent dans le déni. Concernant la prise en charge thérapeutique, les médecins généralistes vont généralement se heurter à la mauvaise adhésion du patient quoiqu'ils proposent. Ils utilisent les antidépresseurs qu'ils maitrisent bien et sont prudents devant le risque d'intolérances ou d'interactions médicamenteuses. La psychothérapie est encouragée mais difficile à mettre en place. Ils font rarement appel aux spécialistes devant leur indisponibilité et des relations interprofessionnelles souvent inexistantes. Par ailleurs, la prise en charge sociale et familiale des sujets âgés est importante car l'isolement des personnes âgées favorise la dépression et augmente la complexité de sa prise en charge. Les différents plans santés et recommandations nationales ont amélioré la pratique en médecine générale mais les praticiens font encore face à de nombreuses difficultés. Les solutions d'améliorations pourraient consister à favoriser le repérage de la dépression du patient âgé, poursuivre et améliorer la qualité des formations en médecine générale, et améliorer la communication entre la médecine générale et le réseau de soin psychiatrique.

Résultats et discussion : l'étude met en relief l'absence d'utilisation de tests ou échelles diagnostiques par les médecins, ainsi que le manque d'exploration du risque suicidaire chez la personne âgée. On retrouve différentes façons de conduire la prise en charge qui sont plutôt marquée par l'expérience clinique. La majorité des médecins ont une opinion positive sur les psychothérapies et les jugent efficaces. Mais ils soulignent plusieurs freins à leurs accès, tels le recours à un autre professionnel, la nécessité de déplacement et le cout. La majeure partie des psychothérapies chez les personnes âgées sont faites par les MG. Les jeunes MG se sentent démunis en habiletés psychothérapeutiques, ils ont plus tendance à prescrire des traitements anti-dépresseurs. Enfin la question de l'isolement et de la perte d'autonomie est centrale chez la personne âgée dépressive. Pour certains médecins généralistes, la restauration d'une certaine autonomie (vue, ouïe, dentition... adéquate) permettrait de limiter certains épisodes dépressifs caractérisés et ainsi d'éviter une prise médicamenteuse à risque iatrogène.
