Available in PDF, EPUB and Kindle. Book excerpt: Carcinomas of the larynx and hypopharynx present unique challenges when it comes to their assessment and management. Management involves the essential dilemma of optimizing cure rates while also ensuring that potential disability due to compromised airway, voice and swallowing is minimized. Treatment decisions involve careful disease assessment, judicious multispecialty decision-making, and subsequent meticulous implementation of the selected treatment modalities. The critical nature of each of these steps has led to dramatic advances in assessment techniques (radiology, endoscopy and photography, stroboscopy, narrow band imaging, contact endoscopy) and also in treatment methods (radiation therapy, combined concurrent chemo-radiation, combined sequential chemo-radiation, trans-oral CO2 laser and robotic surgery, partial laryngectomy, total laryngectomy, extended total laryngectomy). As such, treatment decisions have become increasingly complex with the twin objectives of maximizing cure as well as function preservation and are based on tumor stage, tumor location, systemic co-morbidities, nutritional status, patient priorities, and social and economic factors. This multidisciplinary book has been written by a team of editors with considerable experience and expertise in these critical areas and a deep understanding of teamwork and the strengths and limitations of individual technologies and treatments. It presents a homogeneous and unbiased view that is applicable to all specialties involved in the care of larynx and hypopharynx cancers.

This book was released on 2010 with total page 728 pages. Available in PDF, EPUB and Kindle. Book excerpt: This report considers the biological and behavioral mechanisms that may underlie the pathogenicity of tobacco smoke. Many Surgeon General's reports have considered research findings on mechanisms in assessing the biological plausibility of associations observed in epidemiologic studies. Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.

This book was released on 2004 with total page 354 pages. Available in PDF, EPUB and Kindle. Book excerpt: A working group of sixteen experts from seven countries re-evaluated the evidence of the carcinogenicity of betel-quid and areca-nut chewing and some areca-nut related nitrosamines. Betel-quid and areca-nut chewing are widely practised in many parts of Asia and in Asian-migrant communities elsewhere in the world. There are hundreds of millions of users worldwide. They evaluated betel quid with tobacco as carcinogenic to humans (Group 1) on the basis of sufficient evidence of an increased risk of cancer of the oral cavity, pharynx and oesophagus. The working group reviewed epidemiological studies of human cancer, mainly studies from India, Pakistan and Taiwan (China). Studies on betel quid with tobacco and areca nut with tobacco in experimental animals now also provide sufficient evidence of carcinogenicity. The working group also evaluated betel quid without tobacco as carcinogenic to humans (Group 1), on the basis of sufficient evidence of an increased risk of oral cancer. Studies on betel quid without tobacco and areca nut without tobacco in experimental animals now also provide sufficient evidence of carcinogenicity. Areca nut, a common ingredient of betel quid and many different chewing preparations, including those available commercially, has been observed to cause oral submucous fibrosis

Book excerpt: Cancers of the larynx, while survival outcomes increase, result in massive treatment damage from radiation and surgery. Patients often lose ability to speak and to eat. Preserving the larynx is a fine balance of cancer eradication, life extension, and quality of life. This issueof Otolaryngologic Clinics led by Dr Babak Sadoughi should be of interest to Otolaryngologists, Oncologists, Radiologists, and Speech Therapists. The issue approach reaches all the most important aspects of diagnosing and treating the patient with laryngeal cancer with a focus on preserving the voice in early laryngeal cancer and preserving and restoring function in advanced laryngeal cancer. Topics include: Functional Anatomy and Oncological Barriers of the Larynx; Evaluation of the Dysphonic Patient; Role of Advanced Laryngeal Imaging in Glottic Cancer; Laryngeal Function after Radiation Therapy; Management of Dysphonia after Radiation Therapy; Contemporary Surgical Management of Early Glottic Cancer; Voice Prognosis after Transoral Laser Microsurgery of the Larynx; Voice Rehabilitation after Transoral Laser Microsurgery of the Larynx; Quality of Life after Conservation Surgery for Laryngeal Cancer; Salvage Conservation Surgery of the Larynx; Airway Preservation in Ablative Laryngeal Surgery; Voice Restoration after Total Laryngectomy. Available in PDF, EPUB and Kindle. Book excerpt: MRI is assuming a dominant role in imaging of the larynx. Its superior soft tissue contrast resolution makes it ideal for differentiating invasion of tumors of the larynx from normal or more sharply circumscribed configuration of most of the benign lesions. Over ten years ago CT made a major impact on laryngeal examination because it was the first time that Radiologists were beginning to look at submucosal disease. All of the previous examinations duplicated the infor mation that was available to the clinician via direct and in-direct laryngo scopy. With the advent of rigid and flexible endoscopes, clinical examination became sufficiently precise that there was little need to perform studies such as laryngography which merely showed surface anatomy. The status of deep structures by these techniques was implied based on function. Fortunately laryngography is now behind us together with all of the gagging and contrast reactions which we would all like to forget. CT is still an excellent method of examining the larynx but it is unfortunately limited to the axial plane. With presently available CT techniques motion deteriorates any reformatting in sagittal or coronal projections. The latter two planes are extremely helpful in delineating the vertical extent of submucosal spreads. MRI has proven extremely valuable by producing all three basic projections, plus superior soft tissue contrast. Although motion artifacts still degrade the images in some patients, newer pulsing sequences that permit faster scanning are elimi nating most of these problems.

Available in PDF, EPUB and Kindle. Book excerpt: Laryngeal cancer is a complex medical condition, and its treatments, both surgical and nonsurgical, typically affect a variety of life functions. These complex consequences of laryngeal cancer necessitate that many different medical specialties work together as a cohesive, collaborative care team to support patients and their caregivers/families. Despite the interdisciplinary treatment that laryngeal cancer requires, many resources offer the perspective of only one specific profession. Laryngeal Cancer: An Interdisciplinary Resource for Practitioners caters to the team approach that is needed to provide patients with evidence-based treatment and maximize outcomes. To fill this need for an interdisciplinary text, Drs. Jennifer Campion Friberg and Lisa Vinney bring together a wide range of medical professionals to represent their unique perspectives on the treatment of patients with laryngeal cancer. Each chapter addresses a different critical aspect of the interdisciplinary care and how they impact the other aspects, written by experts in each field. Sample Chapters Include: Medical and Surgical Diagnosis Airway and Respiratory Challenges Nutrition and Swallowing Challenges Psychosocial Care of the Patient Bonus! A supplemental website is available for healthcare providers to share with their laryngeal cancer patients and their caregivers/families. This resource provides critical information about all aspects of their care and treatment, presented in a simple, digestible way for the layperson to understand. When the whole team of health care professionals involved in laryngeal cancer management understands the big picture of care, they can tailor individual treatment priorities to the needs of each unique patient and maximize quality of life. Because of this, Laryngeal Cancer: An Interdisciplinary Resource for Practitioners is designed to be a comprehensive guide for any professional involved in the many facets of interdisciplinary care for patients with laryngeal cancer.
