Book excerpt: Introduction : Véritable épidémie silencieuse, si le diabète n’est pas diagnostiqué à temps, il se développe sournoisement et provoque des dégâts irréversibles. Se basant sur les recommandations citant un risque infectieux augmenté chez le diabétique, les dentistes prescrivent des antibiotiques avant leurs procédures chirurgicales. Objectifs : Evaluer l’application des recommandations sur l’antibioprophylaxie en chirurgie orale chez le diabétique par les dentistes des Pyrénées-Atlantiques, et mettre en évidence d’éventuelles surconsommations. Matériel et méthodes : Un questionnaire a été diffusé par mail. Il aborde les principaux concepts d’une antibioprophylaxie et du diabète à des mises en situation clinique. Deux rappels ont été envoyés à la non-réponse d’un nombre suffisant de questionnaires, remplis et recueillis entre Février et Avril 2012. Les données ont été exploitées à l’aide d’Excel® 2010 et de statistiques descriptives. Résultats : les dentistes appliquaient peu les recommandations et considéraient le diabétique comme à risque d'infection, indifféremment du fait qu'il fût équilibré ou non. Ils prescrivaient quasi-systématiquement une antibioprophylaxie avant leurs procédures invasives chez l'équilibré. Pourtant, chaque procédure n'en est pas tributaire. Leurs stratégies de décision s'appuyaient essentiellement sur le type d'acte et l'équilibre diabétique avant les recommandations 2011 de l'Afssaps. Mais aucun critère de seuil, à partir duquel le risque infectieux est significativement augmenté, n'a été défini. La grande variabilité des traitements illustre le manque de consensus. Conclusion: Des recommandations existent mais leur impact semble être limité. Le manque qualitatif de preuves suggère que les études bien conçues soient effectuées afin de quantifier précisément l'incidence et leur risque de complications infectieuses chez le diabétique. Un emploi inapproprié peut contribuer au problème croissant d'une résistance bactérienne et augmenter les coûts de santé globale.

Chez le patient diabétique, l'hyperglycémie chronique altère le pouvoir tampon de la salive, diminue le pH de la cavité buccale et fournit aux bactéries l'énergie nécessaire à leur croissance ; lorsque l'hyperglycémie systémique induit une susceptibilité aux infections et un retard de cicatrisation. Un diabète non équilibré (HbA1c > 7 %) est donc un facteur de risque de la maladie parodontale. La parodontite, sixième complication du diabète, est une pathologie infectieuse d'origine bactérienne, multifactorielle, à composante inflammatoire, qui se développe chez l'hôte permissif et aboutit à la destruction des tissus de soutien de la dent. Son évolution chronique, cyclique, par poussées, sur plusieurs années, aboutit à la perte des dents. Nous avons étudié la cicatrisation parodontale après chirurgie par lambeau de Widman modifié au sein de 3 groupes de patients : 20 patients sains, 20 patients diabétiques de type II équilibrés et 20 patients diabétiques de type II non équilibrés. Ils ont été suivis sur une période de 18 mois postopératoires. Les données extraites des dossiers médicaux permettent d'étudier la cicatrisation parodontale à travers l'évolution de la profondeur de poche au sein de chaque groupe.

When the Hannover Medical School established West Germany's fIrst Department of Trauma Surgery in 1970, we committed ourselves to this goal and instituted the Hannover Trauma Seminars as a regional forum for continuing physician education. Made up almost entirely of our colleagues at the Trauma Surgery Clinic, the basic goal of the seminars is to review new discoveries and techniques in the fIeld of traumatology and assess their practical relevance to the physician who treats trauma victims. In addition to research, the experience ofthe Hannover School of Trauma Surgery form an important basis for seminar activities. The fIrst Trauma Seminar was held on February 2, 1972. Initially, copies ofthe proceed ings were simply distributed to all interested participants. But as attendance grew and demand for the proceedings increased, it became necessary to seek a broader form of publication. Henceforth, the Hannover Trauma Seminars will be published as part of the Topics in Traumatology series.
