Book excerpt: Résumé: Introduction : Les conduites à risques des adolescents et les situations de mal-être sous-jacentes sont un véritable enjeu de santé publique dont la prévention et le dépistage relèvent de la médecine générale et nécessitent que l'adolescent puisse être reçu sans ses parents en consultation. L'objectif de cette étude est donc de faire le point sur les pratiques actuelles, comprendre les attentes et les difficultés rencontrées par les adolescents, les parents et les médecins généralistes à la mise en place de ce mode de consultation afin de proposer des moyens de le favoriser. Matériel et Méthode : Il s'agit de trois études réalisées dans le département des Hautes-Alpes. La première est une étude quantitative descriptive réalisée auprès d'adolescents de 15 à 18 ans scolarisés en lycée à l'aide de questionnaires. La seconde est une étude quantitative descriptive réalisée avec des médecins généralistes par questionnaire également, et la troisième et une étude qualitative par entretiens semi-dirigés téléphoniques auprès de parents d'adolescents de 15 à 18 ans. Résultats : 1102 lycéens de 15 à 18 ans, 63 médecins et 9 parents ont participé à ces études. Les résultats montrent que la consultation des adolescents sans leurs parents reste minoritaire, et que la problématique des conduites à risques n'est que peu abordée en consultation. A 18 ans, 1/4 des adolescents n'a encore jamais été reçu seul, alors que les médecins sont conscients de l'importance de ce mode de consultation et déclarent essayer de le favoriser, que les parents comprennent plutôt facilement l'intérêt de leur absence et que les adolescents eux-mêmes y sont favorables. Cette étude a également permis de montrer l'effet positif de ce mode de consultation sur la relation de confiance entre l'adolescent et son médecin et sur la fréquence de l'abord de problématiques spécifiques par le médecin. Conclusion : Il existe à la fois des pistes de réflexion et des moyens simples de favoriser ce mode de consultation au quotidien, et donc d'améliorer la prise en charge des adolescents.
