This discerning insight, which dates back more than 2000years to Aristotle, seems to have been ignored by most psycholo gists. For more than 40years theories of human action have assumed that cogni tion and action are merely two sides of the same coin. Approaches as different as S-O-R behaviorism,social learning theory, consistency theories,and expectancy value theories of motivation and decision making have one thing in common: they all assume that "thought (or any other type of cognition) can move any thing," that there is a direct path from cognition to behavior. In recent years, we have become more and more aware of the complexities in volved in the relationship between cognition and behavior. People do not always do what they intend to do. Aside from several nonpsychological factors capable of reducing cognition-behavior consistency, there seems to be a set of complex psychological mechanisms which intervene between action-related cognitions, such as beliefs, expectancies, values, and intentions,and the enactment of the be havior suggested by those cognitions. In our recent research we have focused on volitional mechanismus which presumably enhance cognition-behavior consistency by supporting the main tenance of activated intentions and prevent them from being pushed aside by competing action tendencies.

Book excerpt: Contexte : le manque d'activité physique (AP) de la population est un problème de santé publique. Les autorités de santé émettent des recommandations, notamment via le PNNS, mais peu de travaux étudient comment elles sont relayées par les médecins généralistes (MG), ni leur propre niveau d'AP. L'objectif principal de l'étude était de déterminer l'influence du niveau d'AP des MG sur leur prévention en terme de fréquence d'utilisation du conseil minimal d'AP. Les objectifs secondaires étaient la mise en évidence d'un impact du niveau de connaissance des recommandations en matière d'AP sur l'utilisation du conseil minimal ; ainsi que la mise en évidence de déterminants du niveau d'AP des MG. Méthode : Il s'agissait d'une enquête observationnelle analytique de pratique par questionnaire. Les MG ont été sélectionnés au hasard dans la région Nord-Pas-de- Calais. Ils ont été contactés par téléphone pour obtenir leur consentement et leur adresse mail, afin de leur envoyer le questionnaire. L'analyse principale a consisté en un test de corrélation multiple (modèle multivarié) comparant un score de fréquence d'utilisation du conseil minimal d'AP à plusieurs déterminants, notamment le score d'AP de Ricci et Gagnon et un score connaissance des recommandations sur l'AP. Résultats : Le score d'AP était corrélé significativement au score de fréquence selon un coefficient de corrélation à 0,13 (p=0,00067). La corrélation entre le niveau de connaissance d'AP et la fréquence d'utilisation du conseil minimal (coefficient à 0,33) n'était pas significative (p=0,111). La corrélation entre le niveau d'AP et les déterminants des MG : le sexe, l'âge, la zone d'installation (rurale, semi-rurale, urbaine), le statut de remplaçant ou le nombre de MG composant les cabinets, n'était pas significative. Conclusion : L'étude a montré l'existence d'un lien entre le niveau d'AP des MG et la fréquence d'utilisation du conseil minimal d'AP comme moyen de prévention. Il n'a pas été possible de conclure quant à l'existence d'un impact du niveau de connaissance d'AP sur la fréquence d'utilisation du conseil minimal d'AP. Dans le contexte actuel où l'augmentation du niveau d'AP de la population générale est un objectif de santé publique, le lien entre le niveau d'AP des MG et leur utilisation du conseil minimal d'AP constitue une piste importante pour l'amélioration des pratiques préventives. La formation à la prévention des médecins généralistes devra en tenir compte.
