Cependant, et en raison de leur thermosensibilité, certains d’entre eux ne peuvent pas être stérilisés à 134°C sous peine d’être détruits. D’autres dispositifs médicaux voient leur durée de vie considérablement diminuée par les stérilisations répétées à 134°C pendant 18 minutes. La stérilisation à basse température offre les avantages d’une stérilisation, c’est-à-dire une probabilité de retrouver des germes viables après stérilisation inférieure à un millionième, sans pour autant altérer ces dispositifs médicaux. Parmi les procédés de stérilisation à basse température, la stérilisation au peroxyde d’hydrogène est la plus adaptée pour la pratique hospitalière. Deux procédés utilisant ce composé sont validés par l’Agence Nationale de Sécurité du Médicament comme étant inactivants totaux au regard du Protocole Standard Prion. Ce travail permet de faire la synthèse de tous les dispositifs médicaux qui devraient ou pourraient être stérilisés au CHU de Bordeaux par cette technique. En effet, à l’heure actuelle, le CHU de Bordeaux utilise la désinfection qui ne permet au maximum qu’une décroissance de 5 log de la population initiale de micro-organismes. L’acquisition d’un procédé de stérilisation utilisant le peroxyde d’hydrogène apporterait un gain considérable, mais non chiffrable, en termes de qualité et de sécurité des soins apportés au patient.

Book excerpt: La Stérilisation à Basse Température (SBT) permet le traitement des Dispositifs Médicaux Réutilisables (DMR) thermosensibles tel que les endoscopes souples, les optiques de robots chirurgicaux, des sondes d'échographie pour cavités stériles, mais aussi des DMR commercialisés plus récemment pour de nouvelles techniques chirurgicales. Tous les hôpitaux ne sont pas équipés pour faire de la SBT et traitent leurs DMR de catégorie critique thermosensibles par une désinfection de haut niveau (DHN). La Société Française d'Hygiène Hospitalière va publier en novembre 2022 une mise à jour du Guide de bonnes pratiques de désinfection des dispositifs médicaux. Cette mise à jour fait la promotion de la SBT au détriment de la désinfection de haut niveau. L'objectif est donc d'inciter à ne plus utiliser de DMR thermosensible ayant subi une désinfection de haut niveau pour un geste médical ou chirurgical nécessitant l'abord une cavité stérile (abdomen, appareil urinaire ... ). Le principal objectif de cette étude était de voir dans quelle mesure les établissements de santé vont appliquer ces nouvelles recommandations, qu'ils soient équipés ou non d'un stérilisateur à basse température. De ce fait un état des lieux de la SBT en France a été proposé. Un questionnaire a été diffusé à l'attention des services de stérilisation Français et 53 réponses ont été recueillies. Les résultats de cette étude permettent d'évaluer comment les établissements de soins envisagent de s'adapter vis-à-vis des nouvelles recommandations. La parution de la norme NF ISO 22441 relative aux exigences pour la mise au point, la validation et le contrôle de routine d'un procédé de stérilisation [au peroxyde d'hydrogène gazeux] pour dispositifs médicaux, montre bien que le procédé se pérennise et se structure.

Que ce soit la stérilisation ou l'utilisation de dispositifs à usage unique, la sécurité du patient et des utilisateurs doit être au coeur de la discussion. L'une comme l'autre des solutions montrent leur supériorité en matière de risque microbiologique et chimique par rapport à la désinfection. Les établissements évolueront à terme probablement vers ces solutions.
