Bien que les risques restent faibles et limités par l'expérience et la dextérité du chirurgien, des lésions plus ou moins importantes du nerf laryngé peuvent survenir au cours de l'opération. Cela peut entraîner des paralysies laryngées de différents types. Les paralysies récurrentielles résultent de lésions du nerf récurrent (ou nerf laryngé inférieur). Le second type de paralysie, pouvant être isolée ou associée à la première, concerne le nerf laryngé supérieur. Contrairement à la première, la paralysie du nerf laryngé supérieur est beaucoup plus difficile, voire impossible, à identifier au cours de l'opération par le chirurgien et le monitoring du signal électrique nerveux. Des erreurs ou des errances diagnostiques peuvent survenir et biaiser l'orientation thérapeutique de la dysphonie engendrée. Nous supposons que chacune de ces paralysies laryngées peut induire des perturbations acoustiques et donner des symptômes cliniques distincts. Une récupération spontanée peut être observée dans les mois postopératoires. Après une revue de la littérature thématique, cette étude cherche à comparer l'analyse vocale de deux patientes, atteinte pour l'une d'une paralysie récurrentielle et pour l'autre d'une paralysie du nerf laryngé supérieur. Les enregistrements vocaux ont été réalisés 1 et 3 mois après l'opération, suivant un protocole de tâches commun. Dans cette étude, les résultats montrent que les analyses acoustiques à elles seules restent insuffisantes et limitées pour distinguer chaque paralysie, nécessitant de surcroit une analyse subjective et qualitative avec les échelles du GRBASI et du Voice Handicap Index 10. La récupération spontanée semble discutable et variable d'un cas à l'autre. Enfin, ce travail s'achève par l'introduction clinique du travail recommandé pour la rééducation orthophonique, pour chaque paralysie laryngée, allant du travail préparatoire posturo-respiratoire commun aux exercices spécifiques à chaque paralysie.
