Le délai moyen de prise en charge médicale est de 01h09 en période ouvrable et de 00h56 en période de garde. Pour les patients thrombolysés, il est de 00h11. Le délai d’obtention de l’imagerie est de 01h09 en période ouvrable, 01h48 en période de garde et 00h22 pour les patients thrombolysés. Le délai moyen de thrombolyse est de 01h18 en heure ouvrable et 01h12 en période de garde. Le NIH pré-thombolyse est en moyenne de 14. Le délai moyen de transit aux urgences est de 04h13 en heure ouvrable et de 03h20 en période de garde.

Le développement des techniques de télémédecine a permis une prise en charge neurovasculaire le plus rapidement possible, à proximité du patient. L'objectif de ce travail était d'étudier la procédure télé-AVC au CH Cambrai sur les 18 premiers mois de mise en place. Méthode: Il s'agissait d'une étude descriptive, monocentrique et rétrospective. Les 182 patients consécutifs admis dans la procédure télé-AVC entre le 02 avril 2012 et le 01 octobre 2013 ont été inclus dans l'étude. Résultats: 24,7% des patients de l'étude ont été thrombolysés. Ils avaient un score NIHSS moyen à 9,6. 70% des patients étaient adressés par le SAMU Centre 15, 76% chez les thrombolysés. Le délai médian entre le début des symptômes et la thrombolyse était de 183 minutes, entre l'arrivée et l'imagerie de 34 minutes dans la population générale de l'étude et de 29 minutes chez les thrombolysés. Les patients arrivaient 88 minutes après le début des symptômes à l'hôpital, ce délai était de 90 minutes chez les thrombolysés. Le délai médian entre l'arrivée à l'hôpital et l'injection (door to needle) était de 81 minutes. Parmi les patients non thrombolysés 44,53% avaient une imagerie cérébrale sans signes d'AVC, 10,95% présentaient un AVC hémorragique, 10,22% avaient une imagerie avec un FLAIR positif. Conclusion: Cette pratique nouvelle par télémédecine a permis d'instaurer la thrombolyse au CH Cambrai et de favoriser celle-ci avec 24,7% des patients inclus dans le protocole télé-AVC qui ont été thrombolysés, dans un délai correct par rapport aux autres centres (auparavant les patients se présentant pour AVC étaient secondairement orientés au CH Valenciennes, créant ainsi une perte de chance compte tenu des délais à respecter pour instaurer la thrombolyse). Reste cependant des délais encore élevés. La multiplication des campagnes publicitaires permettrait une reconnaissance plus rapide des signes faisant suspecter un AVC ainsi qu'une régulation par le SAMU Centre 15, ces 2 paramètres permettant un gain de temps considérable dans la prise en charge des AVC.

Ce travail a mis en avant : une prise en charge inadéquate dès la phase pré-hospitalière avec un transport rarement médicalisé inconstamment accompagné et un délai souvent prolongé, des explorations (IRM, écho-doppler) encore insuffisantes dès la prise en charge aux urgences, des avis spécialisés rares au SAU, un traitement institué aux urgences parfois inadapté et une majorité de patients qui échappent à une filière d'hospitalisation optimale. Les carences de la prise en charge des AVC mises en avant aux urgences du centre hospitalier de Pontoise semblent exister malheureusement dans la majorité des hôpitaux en France. Il est donc important et urgent d'améliorer l'état actuel de la situation pré hospitalière et hospitalière concernant les accidents vasculaires cérébraux. Les perspectives envisagées sont : une formation et une information du public et du milieu médical, une meilleure disponibilité des techniques et des hommes et une formalisation des filières avec le développement des Stroke Centers.

Book excerpt: Les accidents vasculaires cérébraux représentent la troisième cause de mortalité et la première cause de handicap acquis dans les pays occidentaux. 80% sont d'origine ischémique. Les deux avancées thérapeutiques récentes telles que la prise en charge des patients en unité neurovasculaire et le traitement par thrombolyse intraveineuse uniquement au sein de ces unités dans les trois heures qui suivent l'installation des symptômes font désormais partie des recommandations nationales et placent l'AVC au statut de véritable urgence neurologique. Notre étude rétrospective menée aux urgences du centre hospitalier général d'Argenteuil sur 12 mois a analysé l'épidémiologie et la prise en charge des AVC ischémiques en fonction des recommandations actuelles. Sur les 293 suspicions initiales d'AVC ont été retenues 200 ischémies cérébrales dont 30% étaient transitoires. En pré hospitalier, 34% des patients consultent d'abord leur médecin traitant et seul 15.5% font appel au SMUR. 28% des patients sont admis dans la fenêtre d'opportunité thérapeutique des 3 heures mais leur délai moyen de réalisation du TDM cérébral est de 227 mn. Au total, 87% des patients ont eu un TDM cérébral et 16% un avis spécialisé neurologique. Le délai moyen de prise en charge aux urgences est de 16,7 h pour les patients admis dans les heures et seul 34% d'entre eux ont pu être hospitalisés en neurologie, 12% ont été transférés vers des services non spécialisés et 2 patients admis en unité neurovasculaire. Si leur prise en charge avait été optimale, 12 patients auraient été candidats à la thrombolyse. Au SAU d'Argenteuil, les délais de prise en charge de l'ischémie cérébrale restent trop longs et la course contre la montre est difficilement réalisable à l'heure actuelle. Une organisation des soins est indispensable pour éviter toute " perte de chance au patient " en réduisant le délai de réalisation du scanner et en facilitant l'accès des patients aux structures de soins spécialisées.

Book excerpt: INTRODUCTION : Saint-Martin est une île française située dans la Caraïbe où les AVC sont fréquents. L'AVC est une urgence nécessitant une prise en charge précoce et rapide dans une filière de soins adaptée. L'étude a pour but d'étudier les caractéristiques épidémiologiques d'une population à haut risque vasculaire présentant des AVC et d'évaluer leur prise en charge aux urgences. MÉTHODE : L'étude descriptive rétrospective a porté sur les patients admis aux urgences entre le 1er janvier 2012 et le 31 décembre 2014 pour un AVC. RÉSULTATS : 147 patients (82 hommes et 65 femmes) ont été inclus. La moyenne d'âge était de 60,1 ans. Les caractéristiques démographiques et le risque vasculaire étaient comparables lors des trois années d'étude. 34 % des patients ont bénéficié d'une régulation SAMU et 20,5 % ont été admis dans les trois heures. Il existe une corrélation entre la gravité de l'AVC et les délais pré hospitaliers courts. L'IRM en première intention a été réalisée dans 38 % des cas, et la télémédecine a été utilisée dans 17,7 % des cas. Le délai moyen d'obtention d'une imagerie était de 8,35 heures. L'alerte thrombolyse a été déclenchée chez 33,8 % des patients pour 5,6 % de thrombolyses réalisées. Tous les patients sont restés hospitalisés, avec plus de 70 % en secteur UHCD. Pour 32 % des patients un transfert vers une UNV a été possible. DISCUSSION : Cette étude a mis en évidence l'importance et l'efficacité des campagnes d'information. La création de la filière neurovasculaire avec la Guadeloupe a permis d'améliorer la prise en charge des patients. Une formation régulière des équipes soignantes sont indispensables pour optimiser les soins et raccourcir les délais.

Une étude réalisée en 2004 au centre hospitalier de Pontoise a montré que la prise en charge des patients admis pour ce motif aux urgences n'était pas optimale, avec une application insuffisante des recommandations. Depuis, l'établissement s'est doté d'une unité d'hospitalisation de courte durée. L'étude menée dans cette thèse a eu pour objectif de dégager l'intérêt de cette nouvelle structure dans la prise en charge des patients cérébro vasculaires aux urgences. Nous avons pour cela comparé de façon rétrospective les paramètres de prise en charge des patients admis pour accident vasculaire cérébral aux urgences de Pontoise avant et après l'ouverture du service porte. Les résultats de ce travail ont montré que l'unité d'hospitalisation de courte durée apportait un bénéfice pour la prise en charge de ce type de patients concernant plusieurs critères : l'accès au plateau technique était meilleur avec 1/3 des patients bénéficiant dès les urgences d'un écho doppler cervical, l'expertise neurologique était obtenue chez près de la moitié des patients, l'orientation était meilleure avec des admissions plus fréquentes en service de neurologie (58.9%), un retour plus fréquent à domicile après l'épisode aigu et cela dans un délai plus court. L'unité d'hospitalisation de courte durée a donc représenté, en l'absence d'unité neuro vasculaire à Pontoise au moment de l'étude, une alternative efficace pour mieux coordonner la prise en charge initiale des patients victimes d'accident vasculaire cérébral. L'ouverture récente d'une unité dédiée au sein de l'établissement devrait permettre, à l'avenir, d'améliorer encore la gestion de ces patients
